The location and severeness of osteolysis around aseptically loosened hip prostheses have certain relation with the migration pathway and collection site of wear particles originated from articular surface.Cement and cementless prostheses have some different roentgenographic feature of osteolysis around the components.

Results Among the 62 cases, 23 were simple orbital wall fracture and the other 39 were complex orbital fracture.

After 24 weeks,the operation area of group A was more smooth,and the surrounding normal cartilage naturally straight flush,transparent form new cartilage,subchondral bone formation in good condition;Group B restoration surgery the basic integrity of the cartilage tissue, center is not yet fully integrated,there was slight depression;CollagenⅡimmunohistochemistry of cartilage that was new brown area.Group C has no formation of articular cartilage.The growth and the intergration of subchondral bone of group A and B were better.

术后24周取材，见A组山羊手术区关节表面较为光滑，与周边正常软骨自然连续平齐，透明的新生软骨组织形成，软骨下骨形成完好；B组山羊手术区修复的软骨组织基本完整，中心部位仍未完全融合，有微小凹陷；Ⅱ型胶原免疫组化示新生软骨组织呈棕黄色。C组术区关节凹陷，无关节软骨组织形成。A组和B组，软骨下骨的生长及与周围组织的结合均较好，无植入物脱落现象的发生。

It is concluded:①The method does not traumatize the periosteum of the hard palate,thus alleviating postoperative maxillary deformity;②The buccal musculomucosal flap is generally long enough to reach across the incisive foramen and maintain good blood circulation;③The method benefits reconstruction of the levator sling and facilitates velopharyngeal closure.

该方法有以下优点：①手术不剥离硬腭区粘骨膜，对上颌骨的损伤很小，减轻了上颌骨的发育障碍；②采用两块带蒂颊粘膜肌瓣重建了腭帆提肌功能，促使异位的提肌悬吊再形成；③血运良好的颊粘膜肌瓣插入于软硬腭交界处，能使软腭充分后退，实现腭咽闭合，而且可有效地防止瘘孔的发生。

The screw entry point was located at the tip of transverse process. The average inclination angle should be 10°-20° headward and 30°-40° toward neutral line respectively. The screw was inserted and entered into the vertebral body along the external cortex of pedicle. The integrity of cortical bone around screws, the distance to segmental blood vessels and the penetration of cortex were observed after operation to evaluate the accuracy and safety of screw placement. Among 160 thoracic pedicle screws that were implanted in 34 cases, 148 screws (92.5%) were accurately inserted inside the costotransverse unit, and 12 screws (7.5%) were misplaced according to the image of X-ray and CT thin-slice scan.

应用椎弓根外内固定技术2004/2007年在中山大学附属第三医院骨科治疗34例患者，固定前采用CT加密扫描测量进钉点、进钉点至椎体前缘的深度、进钉角度和直径，固定中椎弓根钉入点为横突尖，进钉方向为平均向头侧倾斜10°~ 20°、与中线成角30°~40°，顺椎弓根外侧皮质进入椎体，固定后CT加密扫描观察螺钉周围的皮质骨是否完整、是否靠近节段血管、是否穿透皮质骨，评价置入钉的准确性和安全性。34例患者共置入160枚胸椎椎弓根螺钉，固定后CT加密扫描和X射线片观察到148枚(92.5%)螺钉置入准确，12枚(7.5%)螺钉发生错置。
