Objective To explore the changes of metatarsal arrangement and their association with forefoot adduction after modified Mckay procedure.

目的：探讨先天性马蹄内翻足患儿的跖骨排列改变以及其与改良Mckav术后前足内收的关系。

Methods: 14 cases (24 feet) of rigid clubfoot were treated by complete subtalar release.

距下完全松解术治疗重度先天性马蹄内翻足14例24足。

OBJECTIVE: Western-blot technique is utilized to further validate FHL1 and HOXD13 gene expression in protein levels in muscle tissue of patients with congenital clubfoot, the in vivo HOXD13 and predicted binding sites in embryonic foot development was verified using chromatin immunoprecipitation technology.

目的：应用Western-blot技术进一步验证先天性马蹄内翻足患者足部肌肉组织中FHL1，HOXD13基因蛋白质水平的表达，应用染色质免疫沉淀技术验证在胚胎足部发育时在体内HOXD13和预测结合位点的结合作用。

METHODS: Muscle tissues were samples from 15 children with congenital clubfoot, in the Department of Pediatric Surgery, at the Second Affiliated Clinical Hospital of China Medical University, 3 copies of normal children foot muscle tissues at the same age were provided by the Forensic Medicine College of China Medical University, 1 case of aborted embryo at pregnancy 13 weeks were offered by Department of Obstetrics and Gynecology, at the Second Affiliated Clinical Hospital of China Medical University.

15份先天性马蹄内翻足患儿肌肉组织标本由中国医科大学附属第二临床医院小儿外科提供，3份同年龄组正常儿童足部肌肉组织由中国医科大学法医学院提供，1例孕13周流产胚胎由中国医科大学附属第二临床医院妇产科提供。

Methods: 1/2 anterior tibia muscle shifting were used to take the place of long peroneal muscle and short peroneal muscle of paralysis. 13 of 22 cases are congenital talipes equinovalgas, 9 are monstrosity of crossfoot of poliomyelitis.

将胫前肌半腱(外侧1／2)外移代替瘫痪的腓骨长、短肌，配合足部软组织松解或关节融合术，治疗马蹄内翻足畸形。

Methods: From June 2002 to June 2007, 202 patients (298 feet) with congenital talipes equinovarus admitted in our hospital, and the operations such as the Achilles tendon lengthening+solution of soft tissue, the solution of metatarsus fascia, the Achilles tendon lengthening+cuboid bone osteoectomy or tibialis anterior tendon transfer, the Achilles tendon lengthening+talus excision, the Achilles tendon lengthening+fusion of trijoints+talus excision etc, were adoptted.

回顾性分析202例（298足）先天性马蹄内翻足患者的临床资料，根据患者的年龄、内翻足畸形和僵硬程度的不同分别采用跟腱延长+软组织松解、跖筋膜松解、跟腱延长+骰骨截骨或胫前肌外移、跟腱延长+距骨切除、跟腱延长+三关节融合+距骨切除等手术方式进行治疗。

In 6 patients with asymmetric lower extremities and feet, symptoms disappeared in 4 cases, and improved in 2. In 4 patients with urinary and fecal incontinence, 2 achieved complete control, and 2 improved. Three patients with talipes equinovarus recovered after orthopaedic treatment, and 2 with incomplete paralysis in lower limbs improved a little.

结果 本组病儿随访疗效良好。6例双下肢及双足不对称中病人中，恢复正常4例，改善2例。4例大小便失禁病人中能自主控制大小便2例，症状略改善2例。3例马蹄内翻足由骨科矫形治疗后均痊愈，2例双下肢轻瘫病人肌力略改善。

新微型足三维矫形外固定器治疗婴幼儿先天性马蹄内翻足效果良好。

OBJECTIVE: A new three-dimensional external mini-frame was designed to observe its therapentical effects for congenital talipes equinovarus in infants.

设计、时间及地点：回顾性分析2004-01/2007-06河北医科大学第三医院小儿骨科用自制新微型足三维矫形外固定器治疗婴幼儿先天性马蹄内翻足效果。

The Ilizarov technique was used in 9 patients (11 feet) with the severe rigid talipes equinovarus deformity, in which one patient with syringomyelia, and the orthers with congenital club foot. Prospective goals of correction were made in all the cases.

方法］作者在2000年3月～2005年3月间，使用Ilizarov技术矫正9例11足重度僵硬性马蹄内翻足畸形，将连接于胫骨、跟骨、跖骨的外固定环互相连接、组合成复杂的三维外固定架，通过逐渐调整外固定架矫正畸形，从而使患足达到或接近正常足的外形和功能。
