Methods:Twelve patients with fresh transverse fracture of patella were reviewed in the study.The hemarthrosis,blood clot,fracture fragments and soft tissues between them were cleared with arthroscopy.With the help of C-arm X-ray,the fracture was reduced with manipulation method.After the articular facet reached anatomical reduction,two Kirschners were inserted percutaneously from proximal end to distal end of fracture to fix the fragments,Two hollow screw were put along the approach of Kirschners,and tension band fixation was performed with the shape of "8".Results:All the patients were followed up averagely 12.4 months.

12例新鲜髌骨横骨折患者通过关节镜清理关节腔内的血凝块、碎骨块和骨折块之间嵌顿的软组织，直视下观察骨折复位情况以指导骨折复位固定，在C形臂X线透视下帮助骨折复位，关节镜观察关节面解剖复位后，经皮从近折端向远折端平行打入2枚克氏针固定骨折块，顺克氏针拧入2枚中空螺钉，拔除克氏针，顺螺钉空心导入钢丝，行张力带&8&字固定。

Methods 18 fresh humeri were used to reproduce T-type intercondylar fracture, and three different fixation appliances were applied, including cross Kirschner wire with addition of "8" wire, single "Y" type plate, 1/3 tubular plate with reconstruction plate.

采用18具新鲜肱骨标本，制成远端&T&型髁间骨折损伤模型，用交叉克氏针加8字钢丝、&Y&型钢板、三分之一管形钢板及重建钢板3种方法固定后，进行生物力学研究。

Kirschner wire plus tension band fixation ate inferior to transverse fixation, but in comminuted fracture it can induce wore complications: despite of wide application, nickel titanium patella fixator is unsuitable for comminuted fractures due to its hypersensibility and carcinogenicity; silk or steel wire loop fixation fits various comminuted fractures, hut metal cerclage is predisposed to induce infection and breakage because of insufficient fixation force.

克氏针＋张力带固定粉碎性骨折不如横行骨折固定确切，且并发症较多；镍钛－聚髌器适用范围较广，但是其材料的过敏性与致癌性限制了其应用，并且它也无法适用于各种粉碎性骨折；丝线或钢丝环扎可适合各种类型的粉碎性骨折，但是其固定力量不足，金属环扎尚容易产生感染、断裂等并发症。

The Kocher Langenbeck, ilioinguinal, combined anterior and posterior approach were used according to different fracture types.

We present a rare case of type II post-traumatic AARF in an adult treated by posterior transpedicular screw fixation with interspinous bone graft wiring after open reduction.
