Abstract］ Objective To evaluate the role of three-dimensional （3D） digital subtraction angiography in the diagnosis and treatment of symptomatic intracranial arterial stenosis.Methods Stent-assisted angioplasty was performed on 30 patients with symptoms of intracranial artery stenosis by 3-dimesion（3D） method.These imagings were reviewed by different locations to get information of optimal projection angle in 2D angiography.

目的 探讨三维重建技术在颅内动脉狭窄的诊断和治疗中的应用方法对使用具有三维重建功能的美国Advantx LCN+型X线血管造影机进行的30例症状性颅内动脉狭窄的患者的支架成形术进行回顾性分析。

结果 糖尿病患者下肢动脉血管管腔的硬化斑块、狭窄和闭塞的发生率明显高于对照组；硬化斑块的发生率以股动脉及腘动脉最为常见，而管腔狭窄、闭塞率以及腘动脉以下的远端动脉血管最为明显。

而Hcy有可能通过激活核转录因子通路增加损伤血管组织炎症因子的表达而加重血管球能损伤后新内膜的过度增生过程，叶酸能对抗Hcy的上述作用，对高同型半胱氨酸血症引起的血管成形术后再狭窄可能有一定的预防作用。

During the guide wire operation, the following cares should be taken:(1) The head of guide wire needs to be bent to a certain degree and the degree of bend depends on the track of the lesion and diameter and characteristics of the vessel;(2) Insert the guide wire gently through the opening of coronary artery and ensure to deliver the guide wire into the vessel without any resistance;(3) Rotate the guide wire while delivering it across the stenosis lesion;(4) Once the guide wire is delivered to the place, no further operation should be performed before it is confirmed by angiography that the wire is in the true lumen of the vessel.

在引导钢丝的操作中应注意以下几点：(1)引导钢丝头部需弯成一定的弯度，弯度的大小应根据病变的走行、血管直径和特点来决定；(2)引导钢丝进入冠状动脉开口时，动作要轻柔，在确保推送引导钢丝无任何阻力情况下将其送入血管内；(3)引导钢丝通过狭窄病变时要边转动钢丝边推送；(4)引导钢丝到位后要造影确认其在血管真腔内，再行操作。
