Results (1)Of 104 patients with trigeminal neuralgia,75 patients had neurovascular compression or contact of the cisternal portion of the trigeminal nerve at MR imaging and 69 patients were verified by operation;14 patients had C-P angle tumors,13 cases were diagnosed by MRI and the remaining one was verified by the operation and pathology;5 patients had vascular lesions at MR imaging which were verified by microvascular decompression.

The sensitivity of 3d-tof-mra for diagnosing vascular compression or contact was 88.2%, and the accuracy compared with the surgery was 91.8%. conclusion 3d-tof-mra plays an important role in demonstrating the relationship of the cisternal portion of facial nerve with its surrounding vasculature, and has high sensitivity and accuracy in judging neurovascular compression or contact, which may help to select the proper method for treating patients with hfs. 3d-tof-mra appears to be an important imaging test for determining pathogenesis of hfs.

Methods: We selected 71 patients with acute hemorrhagic stroke as the study group who were hospitalized within 24 hours after onset in the 1st 3rd 5th 7th 9th 11th 13th 15th day of hospitalization and prior to the usage of dehydrant. We selected 30 normal people as the control group. We collected the data of cerebrovascular hemodynamic index including cerebral blood flow and velocity, cerebrovascular resistance, cerebrovascular elasticity ,dynamic resistance and critical pressure for each patient . We compared the data between the study group and control group ,also we compared the data in the different time after disease onset and the data of affected side and non-affected side by using SPSS software.
选择急性出血性脑血管病患者71例为观察组,正常人30例作为对照组,观察组以发病时间为起点,分别在发病后第1、3、5、7、9、11、13、15天且在使用脱水药前行脑循环动力学检测仪检测获得脑循环动力学参数,包括血流速度、血流量、血管弹性指标,血管阻力、血管调节功能指标动态阻力及临界压力数值,对观察组和对照组的各项数值进行比较,及观察组在发病后的不同时间进行前后比较,且对患侧和健侧数值之间进行比较,所得数据使用SPSS软件处理。

Essential hypertension caused by neurovascular compression of the left ventrolateral medulla was associated with malbalance of vasomotor center in the dorsal medulla. The possible pathogenesis was the following:① An irritation of the left RVLM by pulsatile compression of an ectatic vessel would increase activity of central sympathetic neuron.② Neurovascular compression at the REZ of the cranial nerves Ⅸ and Ⅹ decreased excitation of parasympathetic nerve.③ Decreased sensitivity of afferent inputs to neuron of nucleus tractus solitarii in the sensory area lead to hyperactivity of central sympathetic nervous system due to pulsatile compression of the left RVLM.④ Neurocompression of the left RVLM and REZ in the left cranial nerves Ⅸ and Ⅹ lead to an overactivity of central renin angiotensin system.
3,结论:左侧延髓腹外侧Ⅸ、Ⅹ颅神经REZ动脉血管压迫所导致的高血压的发生与左侧延髓血管运动中枢对血压的调节失衡有关,其可能机制是:①左侧RVLM在搏动性血管压迫刺激下,中枢交感神经元活性增高;②左侧Ⅸ、Ⅹ颅神经REZ受压导致副交感神经兴奋性降低;③感受区孤束核神经元接受迷走神经传入冲动的敏感性减低,导致中枢交感神经活性增高;④左侧RVLM及Ⅸ、Ⅹ颅神经REZ受压引起中枢RAS活性增加。
