Objective To explore the relationship between the functions and ultrastructural changes of platelets in patients with acute cerebral infarction.

目的 探讨脑梗死急性期血小板超微结构改变与血小板功能变化的关系。

The first part of our study focused on the interaction of platelets and their microparticles with leukocytes.

There are more than 95% in the 34 patiens who have skin ecchymosis, petechia and epistaxis at different degrees. Females also suffere from menorrhagia. When their PT, APTT and FIB were norma and the possibility of DIC was excluded, 3 patients died of cerebral hemorrhage caused thrombocytopenia, accounting for 8.8% of the total death cases.

单纯血小板低引起出血与死亡的关系本组34例患者中95%的都有不同程度的皮肤瘀点，瘀斑、齿龈出血和鼻衄，女性患者可有月经过多，其中有3例患者在PT、APTT、FIB正常，排除DIC的情况下确诊为血小板低导致脑出血而死亡，占总死亡率的8.8%。

Result :(1)the percentage of NTG tolerance in Group Ⅱ and Group Ⅲ were lower than that of Group Ⅰ( P ＜0.05);(2)Captopril and non-sulfhydryl ACEI could inhibit the re-elevation of SBP and the decrease of platelet cGMP at the later stage during the NTG infusion;(3)Platelet cGMP could be an indicator of nitrate tolerance;(4)ACEI could decrease the production of vasoconstrictor substances such as Ang Ⅱ,ET and increase the production of vasorelaxant substances such as BK.

结果：①Ⅱ组和Ⅲ组耐药比例均较Ⅰ组低( P ＜0.05)；②加用Captopril和非巯基ACEI能使NTG滴注过程后期的SBP回升及血小板cGMP下降得到抑制；③血小板cGMP能反映NTG耐药；④ACEI能在NTG滴注过程中使缩血管物质减少、舒血管物质增多。
