AbstractAIM: To assess the efficiency of preventing iris anterior synechia with incision suture in postoperative pediatric cataract.

Postoperative reaction in anterior chamber was distinctive in group Ⅱ, iris posterior synechia in 17 eyes and temporary intraocular hypertension in 8 eyes.

组患者术后前房反应较大，17例患者发生虹膜后粘连，8例患者发生一过性高眼压。

Results The incidence of postoperative peripheral anterior synechia of iris was 87.1% and that of the secondary glaucoma (mainly the angle-closure) was 11.3%.

Since it has not much been used in clinic, and never used in China, to investigate its application for cataract extraction surgery, this study use Erbium: YAG laser for continual circular capsulorhexis on patients of senile mature white cataract, hyper-mature cataract or miotic pupil cataract with posterior synechia, which are difficult for capsulorhexis.

由于迄今临床应用甚少，国内尚未开展此项技术，因此本研究利用Er：YAG激光对采用常规手法撕囊困难的老年性成熟期的白色白内障、过熟期白内障及有虹膜后粘连的小瞳孔老年性白内障患者进行连续环形撕囊，来探讨其在白内障摘除术中的应用。

第一部分为利用Er：YAG激光对老年性成熟期的白色白内障、过熟期白内障及有虹膜后粘连的小瞳孔老年性白内障患者进行连续环形撕囊，并与使用截囊针或撕囊镊等进行手法撕囊技术作比较。

The associated factors, clinical signs, treatment and prognosis were discussed. Result: Nine patients (6 female and 3 male) were included in this study. All of the patients were associated with myopia, ranging from -0.5 diopters to -5.0D. Gonioscopy revealed a dense homogeneous pigmented band seen over the entire circumference of the trabecular meshwork in all patients; however, no patients with iris transillumination defects were detected. Two of the patients developed pigmentary glaucoma and were treated with β-blocker, miotics and argon laser trabeculoplasty.

结果：九病例中有六位女性及三位男性，所有的病例皆有近视，分布於负零点五至负五点零屈光度，所有的病例在隅角镜检查皆可观察到浓厚的色素沈积在小梁组织，然而在这些病例并没有发现到有典型之虹膜透光缺损：其中有两例后来发展成色素性青光眼，以乙型阻断剂、缩瞳剂和氩氮雷射小梁整形术治疗后，病情控制稳定。
