Methods: 1/2 anterior tibia muscle shifting were used to take the place of long peroneal muscle and short peroneal muscle of paralysis. 13 of 22 cases are congenital talipes equinovalgas, 9 are monstrosity of crossfoot of poliomyelitis.

将胫前肌半腱(外侧1／2)外移代替瘫痪的腓骨长、短肌，配合足部软组织松解或关节融合术，治疗马蹄内翻足畸形。

Anatomical study proved:①.4~8 nutritional vessels of sural nerve –small saphenous vein were the intermuscular perforators of peroneal artery, the furthermost one located at(1.0±1.3)cm above the tip of external malleolus with diameter (0.6±0.2)mm;②.2~6 musculocutaneous perforators were given off by the artery of outer head of gastrocnemius muscle to supply nutrition to the sural nerve-small saphenous vein;③.intermuscular perforators from peroneal artery supplied blood to fibula and outer head of soleus muscle.the perforators with an average outer diameter of （1.3-1.6）cm gave off cutaneous branch ,fascial branch and nutritional vessels to the sural nerve-small saphenous vein,which formed the vessel chain of the sural nerve-small saphenous vein ,sub-fascial and suprafascial vessel net.

结果：1、解剖实验证实：①、腓肠神经-小隐静脉营养血管来自腓动脉肌间隔穿支4～8支，最远的跟外侧动脉穿支距外踝尖上(1.0±1.3)cm，外径(0.6±0.2)mm。②、腓肠肌外侧头动脉沿途发2-6支的肌皮穿支营养腓肠神经-小隐静脉。③、腓动脉肌间隔穿支营养腓骨、比目鱼肌外侧半。上述穿皮支平均外径在0.3～1.6mm，发出皮支、筋膜支、腓肠神经-小隐静脉营养血管，形成腓肠神经-小隐静脉血管链以及深、浅筋膜血管网。

The anterior border of all flaps exceeded crista anterior fibulae, and that of 2 flaps reached to the crista anterior tibiae. The width of flap on anterial compartment of the cnemis ranged from 2.5cm to 7.0cm, averagely 4.0 cm.

行前界扩大的改良腓肠肌外侧头肌皮瓣21例，修复的创面位于小腿上1/3段9例，中1/3段前内侧7例，中1/3段前外侧5例。21例皮瓣前缘均超过腓骨前缘，其中2例达胫骨前嵴，位于小腿前骨筋膜室表面的皮瓣部分宽度为2.5~7.0 cm，平均4.0 cm。

Expose the nerve first at the posteromedial aspect of the distal end of the biceps femoris tendon and trace it distally to where it winds around the neck of the fibula. In this location the nerve is covered by the origin of the peroneus longus muscle. With the back of the knife blade toward the nerve, divide the thin slip of peroneus longus muscle bridging it. Then displace the nerve from its normal bed into an anterior position.

先在股二头肌腱远端后内方暴露腓总神经，顺神经向远端追踪至环绕腓骨头的部分，此处腓总神经被腓骨长肌起点覆盖，保持刀背指向腓总神经，挑断跨越神经的部分腓骨肌纤维，然后将腓总神经自正常的位置转移至前移的位置。
