脓肿

Objective To analyze MR findings of liver abscesses and to evaluate diagnosis and differential diˉagnosis value of MR.

目的 分析肝脓肿的MR表现，探讨MR的诊断和鉴别诊断的价值。

These cases included 55 cases of ITP,33 cases of HS,5 cases of MA,5 cases of PHT with splenic function accentuation,3 cases of splenic cyst,1 case of splenic abscess,3 cases of splenic angioma,2 cases of splenic angiolymphoma,2 cases of accessory spleen,and 1 case of splenic rupture.

病例构成为原发性血小板减少性紫癜(idiopathic thrombocytopenic purpura，ITP)55例，遗传性球形红细胞增多症（hereditary spherocytosis，HS）33例，地中海贫血(mediterranean anemia，MA)5例，肝硬化门脉高压脾亢5例，脾囊肿3例，脾脓肿1例，脾血管瘤3例，脾淋巴管瘤2例，副脾2例，脾破裂1例。

The incidence rates of wound infection and ankylenteron and hospital durations in the two groups were observed and compared with each other.

结果引流组与非引流组切口感染率分别为19.0％和8.0％（P〈0.05）；肠粘连发生率引流组与非引流组分别为10.7％和4.5％（P〈0.05）；腹、盆腔残余脓肿的发生率分别为1.1％和1.2％（P〉0.05）；引流组与非引流组病人术后住院天数分别为（9.3 2.7）d和（5.1 1.9）d（P〈0.05），引流组还存在拔管恐惧、脱管等并发症，结论穿孔性阑尾炎术后不放置腹腔引流更明显地降低术后并发症，且明显缩短住院天数。

Results: Of the 26 cases, there were 8 cases with acute osteomyelitis, 12 cases with chronic osteomyelitis and 3 cases with Brodie's abscess and chronic sclerosing osteomyelitis, respectively.

结果：26例中急性骨髓炎8例，慢性骨髓炎12例，骨脓肿和硬化性骨髓炎各3例，术前诊断正确仅20例，3例误诊为恶性骨肿瘤，3例误诊为良性骨肿瘤或肿瘤样病变。

The diffusive XGPN shows cystic-solid multi-occupying with the whole kidney involved and kidney renal pelvis and calices enlarged. The lesions were extensive with unclear border which made the perirenal space dim renal fasciae thicken and with the abscess formation of psoas and abdominal wall. The localized XGPN shows mass in the renal parenchyma with low density clear border after enhancement and rim enhancement.

弥漫型累及全肾，肾影增大，肾盂肾盏扩大积液，肾实质幕粥发囊实性占位，病变分界不清，向肾外扩展，肾周间隙模糊，肾筋膜增厚并累及腰大肌，腹壁脓肿形成；局灶型表现为肾实质内局限性占位，病灶区呈略低密度灶，增强后边界清晰，病灶内见环形强化，而略低密度灶者强化较明显，但仍低于肾实质。

8Neoplastic bone or neoplastic calcified cartilage: the former demonstrate ill-defined patched, radial or conglomeratic high density area in CT; the latter demonstrate spotty or circled high density in CT.

MR检查显示软组织肿胀I度2例，n度6例，nl度6例。3例显示脓肿样囊腔，在MR DWI上均呈明显高信号。1例显示脂液平面。1例显示软组织肿块。14例MR检查均未见软组织内气体。70例恶性骨肿瘤病人中，54例行CT检查，49例行MR检查。
