Poyu Tongluo Decoction is composed of Buyang Huanwu Decoction withRhizoma Acori Graminei, Hirudo and Bombyx Batryticatus, and its effect on the plasma tumornecrosis factorlevel in patients with acute cerebral infarction was observed.

目的：观察破瘀通络汤(补阳还五汤加石菖蒲、水蛭、白僵蚕)对急性脑梗塞患者治疗前后血浆中肿瘤坏死因子的影响。

We evaluated 350 tumors: 136 ES, including 64 conventional ES, 64 proximal-type ES, and 8 with hybrid features of conventional and proximal-type ES; 54 metastatic carcinomas (22 from lung, 6 breast, 6 stomach, 5 colorectum, 5 kidney, 5 prostate, 5 pancreas); 12 metastatic testicular embryonal carcinomas; 20 metastatic melanomas; 20 epithelioid mesotheliomas; 20 epithelioid angiosarcomas; 10 epithelioid hemangioendotheliomas; 24 epithelioid malignant peripheral nerve sheath tumors; 22 myoepithelial carcinomas of soft tissue; 7 anaplastic large cell lymphomas; 5 histiocytic sarcomas; and 10 control MRT of infancy (4 brain, 3 liver, 2 soft tissue, 1 kidney).

我们总共研究了350例肿瘤：136例ES，其中包括64例寻常型ES、64例近端型ES和8例混有前两型特征的混合型ES；54例转移癌（22例来自肺、6例来自乳腺、6例来自胃、5例来自结直肠、5例来着肾、5例来着前列腺、5例来着胰腺）；12例转移性睾丸胚胎性癌；20例转移性黑色素瘤；20例上皮样间皮瘤；20例上皮样血管肉瘤；10例上皮样血管内皮瘤；24例上皮样恶性外周神经鞘膜瘤；22例软组织的肌上皮癌；7例间变性大细胞淋巴瘤；5例组织细胞肉瘤；10例作为对照的婴幼儿MRT（4例位于脑、3例位于肝、2例位于软组织、1例位于肾）。

Tumor mass could express CD133 and nestin. The karyoplasmic ratio of CD133+ brain glioma stem cells reached 2/3-3/4, and there were a few processes, a few organelles and a plenty of ribosome. These were accorded with the characteristics of stem cells.

肿瘤球能同时表达干细胞标志物CD133和巢蛋白，CD133+脑胶质瘤干细胞核浆比例达2/3~3/4，突起少，胞浆中线粒体等细胞器较少，核糖体丰富，未见胶质丝等分化结构，符合干细胞超微结构特点。

Results:There were 12 cases of peripheral sensory neuropathy,12 cases of myasthenic syndrome,6 cases of spinal cord compression,4 cases of brain stem encephalitis,3 cases of subacute cerebellar degeneration.3 cases were diagnosed in 1 week after appearing neurological symptoms,and 3 cases were diagnosed in 1 month,the other 16 cases were diagnosed after many months or years while the neuropathy was existing.

结果：癌性感觉性周围神经病12例，肌无力综合征8例，脊髓压迫症6例，脑干脑炎4例，亚急性小脑变性3例。3例在出现神经系统症状后1周内确诊，3例在1月内确诊，其余16例均在神经病变持续存在数月至数年后经多次检查确诊为肿瘤。

The results showed that 3 cases were alive longer than 5 years and 2 cases died from acute myocardiac infaction and intracerebral hemorrhage more than 1 year after operation respectively.

随访1~10年，1例手术后1年死于脑出血，1例肿瘤复发再次术后1年5个月死于急性心肌梗塞，余3例随访5年以上均健康存活。

结论应用电磁刀配合显微手术能够明显提高脑和脊髓肿瘤的全切率，并能减少重要神经和血管的损伤。

Distant metastases to the lungs (n=4), pleura (n=2), brain (n=3), and peritoneum (n=1), and paraaortic, paratracheal, and mediastinal LNs (n=2) were observed. Six of 9 patients (66%) died from tumor dissemination, all with a median overall survival of 4.3 years (range: 1 to 9 y).
