脊髓纵裂

Objective. To show retrospective analysis of 21 consecutive patients who underwent simultaneous surgical treatment for progressive spinal deformity and coexisting intraspinal pathologies (tethered cord and/or diastematomyelia).

目的：回顾分析21个病人，这些病人因为进展性的脊柱畸形和共存的椎管内病变（范围束缚和/或脊髓纵裂）同时接受外科治疗。

According to the classification, there were single-tube in 25 and double-tube in 96 patients. One hundred and two patients with diastematomyelia were treated by operation and ninteen by conservative treatment. The clinical results were retrospectively analyzed according to scoring neural function and monitoring the peak P40 of posterior trivial nerve cortical somatosensory evoked potential before and after treatment.

方法］对1978年5月～2006年1月收治的有完整资料的121例单管型及双管型脊髓纵裂手术及非手术治疗的病例进行回顾性分析，并通过临床神经功能评分及胫后神经皮层体感诱发电位P40波峰监测，评价其手术治疗的疗效。

One hundred and two patients with diastematomyelia were treated by operation and ninteen by conservative treatment. Results: Patients with diastematomyelia w ere categorized into osseous, cartilaginous, fibrous and mixed types according to the CT and operative findings.

结果： 根据CT影像与手术所见将脊髓纵裂间隔分为骨性、软骨性、纤维性和混合性。

ConclusionOne-stage posterior hemivertebrae and osseous divide resection combined with transpedicular instrumentation and bone graft can achieve a satisfactory result for the treatment of scoliosis of thoracic hemivertebra and diastematomyelia of adolescent.It can be recommended in clinical practice because of a good stability of fixation and fine spinal fusion.

结论一期后路半椎体及骨嵴切除、椎弓根螺钉内固定植骨融合术是治疗青少年胸椎半椎体脊柱侧凸合并脊髓纵裂的有效方法，具有较好植骨融合和稳定性，值得推广应用。

Amongst 23 diastematomyel ia in 20 patients, there were 18 osseous, 2 cartilaginous, 1 fibrous and 2 mixed types. With the exception of one case with fibrose septum, all th e CT findings were in keeping with the operative findings.

20例患儿23处脊髓纵裂中，骨性18例，软骨性2例，纤维性1例，混合性2例，除1例纤维性间隔CT平扫未能显示外，其余的CT影像与手术所见相符。
