胃胃吻合术

To evaluate the clinical value of intrathoracic esophagogastric "layer-to-layer" anas tomosis with esophageal mucosa extended varied in plane and to study the effective methods to prevent anastomotic leaking or stricture postoperatively.

目的 评价胸内食管黏膜延长不同平面与胃、食管分层吻合术，探讨预防食管吻合口瘘和吻合口狭窄发生的吻合方法。

Intrathoracic esophagogastric "layer-to-layer" anastomosis with esophageal mucosal extended varied in plane is an ef fective method to prevent anastomotic leaking and stricture postoperatively.

食管、胃黏膜延长不同平面分层吻合术是预防吻合口瘘和吻合口狭窄的有效吻合方法。

Methods:from 1988 to 2004, 26 patients (25 males and 1femal, aged from 28～72 years old) with esophagus rupture were treated. among the 26 cases, 2 cases received conservative treatment; 6 cases' esophaguses were slit to extirpate heterogeneous things; 3 cases received thorax and mediastinal conduction added gastrostomy or jejunostomy; 14 cases received esophagus hernioplasty; 1case was put the metallic bracket into esophagus and then received esophagus resection and esophagus -gastric anastomosis in the second period.

1988～2004年收治食管破裂患者26例，其中男25例，女1例，年龄28～72岁。26例患者中，2例保守治疗，6例切开食管摘取异物，3例行胸腔、纵隔引流加空肠及胃造瘘术，14例行食管修补术，1例行食管内带膜金属支架植入，二期再行食管切除食管胃吻合术。

Three types of reconstruction procedures, including Orr-type Roux-en-Y esophagojejunostomy, P-type jejunal pouch Roux-en-Y esophagojejunostomy and distal jejunal aboral pouch Roux-en-Y esophagojejunostomy, were performed.

全胃切除后消化道重建分别采用全胃切除术后消化道重建Orr式Roux-en-Y食管空肠吻合术、P型空肠袢食管空肠Roux-en-Y吻合术和远端空肠反口贮袋的Roux-en-Y食管空肠吻合术。

Objective\ To find out a good method to reduce or eliminate the chylothorax after resection of esophageal carcinoma.

目的 寻找减少或消除食管癌切除术后乳糜胸发生率的有效方法。方法随机选取 45例食管癌患者，在食管癌切除加胃食管残端吻合时，一期行预防性低位胸导管结扎术(第 8胸椎水平以下)；另 92例作对照组，仅做食管癌切除加胃-食管残端吻合术。

Methods:The incisional infection rates between the manual way (43 cases) in March 1998 and the anastomat way (59 cases) in March 2006 in the esophagogastrostomy after resection fo esophageal carcinoma were compared.

回顾分析我院1998年3月手工吻合43例与2006年3月器械吻合59例病人行食管癌切除食管胃吻合术，比较器械吻合组和手工吻合组术后切口感染率的发生。

Methord 45 patients of the esophageal carcinomawho were, at random, elected had been undergone the ligation of thoracic duct( Low level: under the 8th thoracic vertebrae) during resection of esophageal carcinoma and esophagogastrostomy, while the other 92 patients of the esophageal carcinomahad only been operated the resection of esophageal carcinoma and esophagogastrostomy.

随机选取45例食管癌患者，在食管癌切除加胃-食管残端吻合时，一期行预防性低位胸导管结扎术(第8胸椎水平以下)；另92例作对照组，仅做食管癌切除加胃－食管残端吻合术。

The patients may occur exhaust distention more possibly in BillrothⅡanastomotic operation,that is partial resection of stomach with gastrojejunal anastomosis,than Billroth Ⅰanastomotic operation.

两种不同消化道重建术手后，行毕Ⅱ式吻合术即行胃空肠吻合的患者更容易出现腹胀，应采取有效的护理干预措施，以减少腹胀的发生。

两种不同消化道重建术手后，行毕Ⅱ式吻合术即行胃空肠吻合的患者容易出现腹胀，应采取有效的护理干预措施，以减少腹胀的发生。

Methods 136gastric cancer patients,who underwent total gastrectomy using threereconstruction proceˉdures between1993and2002,were retrospectively evaluated with their digestive symptoms and nutritional status.The three procedures are Braun anastomosis,Roux-en-Yreconstruction and Roux-en-Y with P-shaped anasˉtomosis.Results Both RY and PRY reconstruction have superiority in anti-esophagus reflux than Braun anastomosis.

1993～2002年行全胃切除术的136例胃癌患者，分别采用袢式空肠代胃+Braun吻合、单纯Roux-en-Y吻合和Roux-en-Y吻合+P形代胃三种方式重建消化道，对这些患者进行回顾性分析，以术后的消化道症状和营养状况作为评价依据。
