Objective To summarize the clinical experience of video-assisted thoracoscopy combined with minimal incision pulmonectomy in the treatment of pulmonary tuberculosis.

Methods 36 pulmonary tuberculosis patients who had operative indications were treated by video-assisted thoracoscopy combined with chest minimal incision surgery,including 18 cases of pulmonary wedge resection, 15 cases of pulmonary lobectomy and 3 cases of compound pulmonectomy.

方法对36例有手术指征的肺结核患者运用电视胸腔镜辅助小切口进行肺切除术，其中肺楔形切除术18例、肺叶切除术15例、复合肺切除术3例。

Objective:to evaluate the effect about 11 antigenpreparations used for serological differential diagnosis of pulmonary tuberculosis and lung cancer.

目的：对11种分枝杆菌抗原制备物用于肺结核和肺癌血清学鉴别诊断的效果进行评估。

MPR and VR were applied in the imaging post-processing. Results: All cases were divided into four groups according to the thoracic CT findings and pathologic results as follows:① spheral caseous pneumonia;② tuberculous granuloma;③ fibrous caseous nodule;④ Focal pulmonary consolidation with cavitation. The spheral lesion appeared inhomogeneous density (44/58). cavitation (14/58) or satellite foci (21/58). Contrast scanning was carried out in 31 cases, of which peripheral or ringshaped enhancement was seen in 15 cases, no enhancement in 9 cases and extensive enhancement was seen in 7 cases.

结果：依据影像学表现、随访结果和病灶的病理改变，将58例球形肺结核分为四种类型：①球形干酪肺炎11例，体积较大（最大直径3~5cm），边缘模糊或毛糙，可见晕征，无强化或环行强化；②结核性肉芽肿19例，软组织密度，形态规则或不规则，边缘清楚或光整，强化模式为均匀强化或厚壁强化；③纤维干酪结节14例，规则球形，边缘光滑锐利，内部密度不均匀，可见钙化，多无强化或薄壁环行强化；④结核空洞14例，干酪空洞或纤维空洞，内壁不光整，可见到引流支气管（2例）。

EMB-resistance in 18 isolates was caused by the mutation at coden 306 in embB gene with positive rate of 62.1%. The PCR result was negative in 20 specimen from pulmonary diseases without active TB and 5 negative control strains of no MTB. The data in this study showed high specificity of 100%. Only one specimen in 7 specimen with negative result in acid-fast stain and cultivation was shown alteration in coden 306 in embB gene.

结果：130例活动性肺结核患者痰标本抗酸染色阳性38例，阳性率为29.2%(38/130)；罗氏培养阳性为79例，阳性率为60.8%(79/130)。79例培养阳性痰标本中耐乙胺丁醇29例，其中有18例在embB基因met306ATG突变，基因突变率为62.1%。20例非结核肺部疾病患者痰标本和5株非结核分枝杆菌菌株经套式PCR扩增全部为阴性，特异性为100%。7例涂阴培阴PCR扩增阳性的序列中有1例在embB基因306位密码了发生突变。
