肺状的

CT of lung show diffused distribution of plaque and spottiness with high density cavity with inequlity of size and ellipse nodus with sharpness edge and high density. Cases received treatment with Amphotercin B combination fluconazol by vein and intrathecal injection of Amphotercin B.Five cases were cured and five were improved.

5胸片表现为两肺散在分布小片状、小结节阴影，边缘模糊、内见不规则低密度区；CT示两肺广泛分布斑片状和斑点状密度增高影、大小不等的空洞或类圆形边缘光滑高密度结节。

Results 6 patients with PAP presented with ground-glass opacification of the bilateral lungs on HRCT. Typical chest HRCT scan showed "map-like" lesions in 3 cases and "cobble stone-like" lesions in 3 cases. Multifocal consolidative opacities in the lungs were seen on HRCT in 3 cases. Lung biopsies from 6 patients with PAP revealed a prominent positive periodic acid-Schiff intra-alveolar exudates. There were normal alveolar spetasis in 3 patients while interstitial thickening, fibroblast hyperplasia, interstitial fibrosis in the others.

结果 6例患者在不同层面可见磨玻璃影，其中2例可见小结节影；3例表现为两肺斑片状磨玻璃影与周围肺组织分界清楚，呈地图样改变；3例因磨玻璃影与小叶间隔增厚交织成铺路石样改变；3例可见肺泡实变融合成密度较高的斑片状阴影，1例在肺泡实变区可见&空气支气管征&。6例患者活检肺组织在光镜下显示肺泡腔内充满大量块状或颗粒状嗜伊红物质，PAS染色阳性，AB染色阴性，其中3例肺泡间隔正常，3例肺泡间隔增宽，可见慢性炎性细胞浸润、成纤维细胞增生和胶原沉积。

Results:31 cases (34.8%) had bronchovascular shadows with the small spot shadows;32 cases (35.9%) had clouded shadows;26 cases (29.2%) had patchy shadows and pulmonary cyst; We also can find pneumothorax, hydropneumothorax, mediastinal emphysema, subcutaneous emphysema, fracture of costal bone.

结果：肺纹理有改变伴有小斑点状阴影31例(34 8%)；出现云雾状及磨玻璃状阴影32例(35 2 %)；有大片模糊的较高密度影和肺囊肿影2 6例(2 9 2 %)；另外还发现气胸、液气胸、纵隔气肿、皮下气肿及肋骨骨折等。

Results:31 cases (34.8%) had brochovacular shadows with the small spot shaows;32 cases (35.9%)had clouded shadow; 26 cases (29.2%) had patchy shadows and pulmonary cyst ;We alse can find pneumothorax,hydropneumothorax,mediastinal emphysema,subcutaneous emphysema,fracture of costal bone.

结果：肺纹理有改变伴有小斑点状阴影37例（37%）；出现云雾状及磨玻璃状阴影48例（48%）；有大片状模糊的较高密度影和肺囊肿15例（15%）；另外还发现气胸、液气胸、纵隔气肿、皮下气肿及肋骨骨折等。

Resujts:① The distribution of lesions are mostly symmetrical, extensive, predominant in the middle and lower lung fields, and posterior areas in peripheral;② There are so many HRCT findings of lung, ILD is predominant: intralobular interstitial thickening 46 cases (100%), ground-glass opacity 41 cases (89.13%), peribronchovascular or centrilobular interstitial thickening 40 cases (86.96%), interlobular septal thickening 38 cases (82.61%), irregular linear opacity 37 cases (80.43%), small nodular opacity 34 cases (73.91%), subpleural line 27 cases (58.70%), bmnchiectasis or bronchiolectasis 19 cases (41.30%), patch opacity 18 cases (39.13%), expiratory mosaic sign 15 cases (32.61%), interface sign 14 cases (30.43%), honeycombing 12 cases (26.09%), emphysema or bulla 3 cases, cystic airspace suspected 1 case, and atelectasis suspected 1 case;③ Mediastinum and pleura: multiple small lymphonodi in mediastinum 41 cases (89.13%), pleural thickening or rough 38 cases (82.61%), esophagoectasis 11 cases (23.91%), unilateral little pleural fluid 1 case, and mediastinal emphysema 1 case.

结果：①皮肌炎肺部病变分布呈对称、广泛、偏中下、偏外后的特点；②肺部HRCT表现多样，以肺间质性改变为主：小叶内间质增厚46例(100%)，磨玻璃影41例(89.13%)，支气管血管束增宽或小叶核心增大40例(86.96%)，小叶间隔增厚38例(82.61%)，不规则纤维索条影37例(80.43%)，结节影34例(73.91%)，胸膜下线27例(58.70%)，支气管或细支气管扩张19例(41.30%)，斑片影18例(39.13%)，呼气相马赛克征15例(32.61%)，界面征14例(30.43%)，蜂窝影12例(26.09%)，肺气肿或肺大泡3例，单纯囊状气腔1例，肺不张l例；③纵隔及胸膜：纵隔小淋巴结影41例(89.13%)，胸膜增厚或毛糙38例(82.61%)，食管扩张11例(23.91%)，单侧少量性胸腔积液1例，纵隔气肿1例。

Two third of pts with unilateral lung oligemia had plethora on the contralateral side.

三 继发改变中，14.7％表现为外围肺的实变灶或肺内残留条索状、盘状不张或纤雏化。

Result: A 77-year-old female presented at our hospital for blurred vision in her right eye. The intraocular pressure was 46 mmHg and slit lamp examination showed a protruding mass with papillomatous appearance near pupil margin. On chest X-ray examination, only prominent left hilum was noted. Iris tumor excisional biopsy was performed. Pathology investigation of the surgical specimen revealed carcinoma and immunohistochemical study demonstrated thyroid transcription factor-1 (TTF-1) positive. Chest-computed tomography showed left upper lung tumor and primary pulmonary adenocarcinoma was confirmed after CT-guided biopsy. She was placed under treatment with Genfitinib. Recurrent iris tumor mass was noted during follow-up, and chest CT revealed shrinkage of adenocarcinoma.

结果：一位七十七岁女性因右眼视力模糊至本院就诊，当时眼压为46毫米汞柱，细隙灯检查发现虹膜靠近瞳孔边缘有一乳突状肿块，胸部X光摄影仅见左肺门稍明显并未发现明显病灶，患者接受虹膜肿瘤切除手术，病理报告显示为恶性肿瘤，且免疫组织染色为thyroid transcription factor-l阳性，进一步的胸部电脑断层显示左上肺肿瘤，经电脑断层指引之切片检查证实为原发性肺腺癌，她接受Genfitinib药物治疗，后续追踪发现虹膜肿瘤复发，胸部电脑断层扫描显示肿瘤体积缩小。

Results Thirty-one(7.64%) of 406 patients showed abnormal ventilatory function, while 165 of 404 patients showed diffusing abnormality. Of the 434 patients who had received HRCT scans,151(34.79%) showed abnormalities including subpleural and basal ground-glass and reticular attenuation,nodular septal thickening and bronchiectasis. Of the 395 patients who had received both lung function test and HRCT scanning,55(13.92%) had abnormalities both in the lung function and HRCT.

结果 在4 0 6例进行肺通气功能检测的患者中，31例(7 6 4 %)异常；4 0 4例进行肺弥散功能检测，其中16 5例(40 84 %)异常；4 34例进行胸部HRCT检查，15 1例(34 79%)异常，表现为磨玻璃密度阴影、网格状阴影和胸膜下弧线影等；395例同时接受肺弥散功能检测和胸部HRCT检查，5 5例(13 92 %)两项均异常，且HRCT显示双肺病变典型者的肺弥散功能较HRCT正常者有显著的下降，两组差异有统计学意义。

Results:There were multiple small nodules in bilateral lungs in 9 cases.Bullerfly wing-like shadow was showed beside bilateral lung hili in 3 cases.Great piece shadow can be found in both inferior lungs in 3 cases.

结果：15例中9例表现为双肺广泛分布的小结节状阴影，3例表现为以肺门为中心，向外放射的蝶翼状阴影，3例表现为双下肺野大片状致密阴影。
