The patient presented with hemoptysis. The chest images and bronchoscopic washing cytology showed bilateral lung involvement, so chemotherapy was prescribed, as stage IV non-small cell lung cancer with lungto-lung metastasis was suspected. After 3 cycles of chemotherapy with docetaxol and carboplatin, the bilateral lung tumors responded discordantly. An aggressive surgical intervention was performed, and the pathology showed synchronous squamous cell carcinoma and a carcinoid tumor.

病人一开始以咳血表现，因为胸部影像学检查及支气管镜抽洗液细胞学检查显示两侧肺部皆有癌侵犯，所以在第四期非小细胞肺癌合并有肺部转移的情形下开始接受化学治疗，经过三个疗程的克癌易及卡铂之化学治疗，两侧肺肿瘤的反应不一致，因此采取积极的手术处理，而病理报告显示两侧肺部肿瘤分别为鳞状上皮癌及类癌。

ConclusionGR mRNA upregulation in lung tissue could be one of the mechanism of glucocorticoid's effective cure for pulmonary fibrosis,and GR mRNA downregulation in lung tissue in renalyang asthenia syndrome could be one of cause of glucocorticoid's noneffective therapy for pulmonary fibrosis,in addition,the Chinese traditional medicine with warming Yang effect could be effective therapy for pulmonary fibrosis.

结论肺组织GRmRNA表达的上调是糖皮质激素防治肺纤维化之所以有效的机理之一，而肾阳虚状态下肺纤维化肺组织GRmRNA表达下调是临床上许多肺纤维化病人对糖皮质激素不敏感的原因之一，同时，温阳药也极可能具有防治肺纤维化的作用。

METHODS: Pulmonary function tests were performed in 20 children with asthmatoid bronchitis (2 months-2.5 years of age) before and 30, 60, and 120 minutes after salbutamo and ipratropium bromide inhalation. 20例2个月至2岁6个月的喘息性支气管炎患儿，在吸入药物沙丁胺醇+溴化异丙托品前、吸入后30，60，120min各行一次潮气流速容量环检查，并进行肺功能各项指标比较，包括潮气呼吸流速容量环的形态，反映小气道功能的敏感指标到达潮气呼气峰流速时的呼气量/潮气量，呼出75%潮气量时的呼气流速/潮气呼气峰流速(25/PF)，潮气呼气峰流速，反映大气道功能的指标潮气呼气中期流速/潮气吸气中期流速，潮气呼吸状态下的通气功能，包括呼吸频率，每公斤体重潮气量。

objective to evaluate the main points and complications of ct-guided percutaneous pneumocentesis in patients with lung lesions.methods cook 18～21g puncture needle was used to inserting aspiration in the optimal level,situs,angle and depth.and had made of multidrop.fifty-nine patients with lung lesions underwent ct-guided percutaneous pneumocentesis and follow-up.results the successful puncture rate was 100%,accurate diagnose rate was 92%(54/59).conclusion ct-guided pneumocentesis for chest lesion is a efficient and safe diagnostic method.but it is important to improve the doctor′s operating skill to lessen vascular complication.

目的 探讨ct引导下经皮肺穿刺活检的技术要点及并发症。方法采用cook公司18～21g自动弹射活检枪，选择最佳层面、进针点、进针角度和深度，行多点取材。回顾性分析59例肺内病变ct引导下经皮肺穿刺活检术。结果穿刺成功率100%，活检诊断准确率为92%（54/59）。3例发生轻度气胸，2例穿刺后咯血痰。结论 ct引导下经皮肺穿刺活检术成功率高，是安全的，但操作医生熟练的技术方法非常重要，并可以减少并发症的发生。
