Methods:9 patients including 8 male and 1 female were included in this study.2 patients suffered from both side of ANFH,5 were righe side and 2 were left side.X-ray,CT and MRI were performed before the surgery.4 hips were classified as ARCO stage 1,7 hips were stage 2.All the patients were treated as follows:the area of NFH and cystis degeneration were detected and bouche decompression was done through X-ray with C-arm,and the necrotic bone was cleared for the pathologic diagnosis under the monitoring of arthroscopy.And then the non-cell based tissue engineering bone, which was the complex of autologous red bone marrow and osteoinductice absorbing material containing BMP,was transplanted into the body.

本组9例，男性8例，女性1例；双侧股骨头坏死2例，右单侧股骨头坏死5例，左单侧股骨头坏死2例；术前均行X线摄片、CT、MRI 检查，ARCO分期：Ⅰ期4髋，Ⅱ期7髋；C型臂X线透视下行股骨头坏死区和囊性变区定位、髓芯减压，关节镜监视下，刮除坏死骨送病理；植入由自体红骨髓与含BMP的骨诱导活性材料复合成的非细胞型组织工程骨，并用空心钛钉支撑股骨头软骨下骨质。

自体红骨髓组织工程复合物植入结合临床应用髓芯减压、空心钛钉支撑，在减轻疼痛、改善股骨头内循环、促进骨修复、防止或延缓股骨头塌陷有极好的作用，为临床治疗成人ARCOⅠ期、Ⅱ期股骨头缺血性坏死一种有效的新方法。

Results all of the 20 x-ray cases demonstrated the trabeculae bone hazy with pathy high density in the weight-supporting zone .all of the ct cases demonstrated the collapse of the femoral head. and the 4 cases demonstrated the high density, fragment of the femoral head and cystic lucency zone in the high density area and mixed density could be seen in 3 cases. the 4 mri cases demonstrated cystic hypointensity in t1wi and hyperintensity in t2wi. the 3 mri cases demonstrated mixed intensity in t1wi and t2wi,the 3 mri cases demonstrated irregular hypointensity with collapse of the femoral head in t1wi and t2wi.

结果 x线平片20 例，均表现为股骨头持重区骨小梁增粗且模糊不清，持重部位呈斑片状或斑点状密度增高影，本组均可见（占100%）。ct扫描20 例，平扫均表现为股骨头形态塌陷，其中显示高密度硬化4 例（占20%），股骨头碎裂，显示高密度区内有不规则透亮区，呈高低混杂密度改变有3 例（占15%）。mri扫描10 例， t1wi呈斑点状或小囊状低信号，t2wi为高信号，本组4 例，t1wi与t2wi均为不规则低、等、高混杂信号，本组3 例，股骨头塌陷、变形，t1wi与t2wi均为不规则形低信号3 例。

老年患者股骨颈骨折保守治疗的骨不愈合率、股骨头缺血坏死率及卧床并发症发生率很高，常需要行手术治疗，目前应用人工关节置换疗效较好，能早期负重行走，生活质量明显提高，已得到骨科医师的普遍认同，其中又以人工股骨头置换术和人工髋关节置换术为主，但这些术式的适应证及利弊尚存在诸多争议，我们通过比较双动人工股骨头置换与全髋关节置换治疗老年人股骨颈骨折的平均手术时间、平均输血量、平均住院时间、平均住院费用、早期并发症、Harris评分、牛津髋关节评分标准评价来探讨双动人工股骨头置换与全髋关节置换在老年人股骨颈骨折治疗中的优势与劣势。

Results The fractures affected the paretic side in the 23 cases,the 24 cases were treated with hemiarthroplasty including bipolar arthroplasty in 10 cases,the 23 cases were followed up for 4 weeks to 1 years,the Harris scores were beyond 80 points in all cases.

结果 2 3例股骨颈骨折发生在偏瘫同侧，2 4例接受人工股骨头置换手术，10例双动股骨头，其余患者为单极股骨头；2 3例接受随访，时间为 4周至 1年，Harris评分均在 80分以上。
