肠切除术

Emergency operation were performed on 21 patients,including embolectomy on 9 cases, embolectomy and intestinal resection on 8 cases, intestinal resection and anastomosis on 3 cases, intestinal resection and enterostomy on 1 case.,expectant treatment on 6 cases.

急诊行手术治疗21例，其中单纯行肠系膜上动脉取栓术9例，肠系膜上动脉切开取栓加肠切除吻合术8例，单纯肠切除吻合术3例，肠切除加肠造瘘术1例。保守治疗6例。

The average age was 27.8 years, and there were 47.5% patients with malignant change, the average age of them was 35.9 years. The choice of operative procedures included total proctocolectomy with ileostomy in 60 cases (26.0%), subtotal colectomy+rectal polyposis electrocautery in 63 cases (27.3%), total colectomy or proetocolectomy with ileo-anal anastomosis in 12 cases (5.2%), total colectomy or proctocolertomy with ileal pouch-anal anastomosis in 19 cases (8.2%), subtotal colectomy+rectal mucoscctorny, through the muscular sheath of rectum ileo-anal anastomosis in 22 cases (9.5%), ileal pouch-anal anastomosis in 47 cases (20.3%), and subtotal proctoeoleetomy in 8 cases (3.5%).

术式选择全结肠直肠切除＋末端回肠腹壁造口术60例(26.0%)，全结肠部分直肠切除＋回肠直肠吻合术63例(27.3%)，全结肠直肠切除＋回肠肛管吻合术12例(5.2%)，全结肠直肠切除＋回肠储袋肛管吻合术19例(8.2%)，全结肠部分直肠切除＋残留直肠黏膜剥脱＋经直肠肌鞘回肠肛管吻合术22例(9.5%)，全结肠部分直肠切除＋残留直肠黏膜剥脱＋经直肠肌鞘回肠储袋肛管吻合术47例(20.3%)，部分结肠或直肠切除术8例(3.5%)。

Radical excision 153 cases,of which 128 cases in pancreatoduodenectomy,20 cases in the expansion of pancreatoduodenectomy,and 5 in total pancreatectomy; 102 cases with palliative operation,of which 33 cases in RouxY biliaryenteric anastomosis,69 cases in the Rouxen bridge T tube drainage.

根治性切除术153例中，胰十二指肠切除术128例，扩大胰十二指肠切除术20例，全胰切除术5例；姑息性手术102例中，胆肠RouxY吻合术33例，胆肠T管架桥内引流术69例。

Cases were undergone choledocholithotomy with T-tube drainage , 39 cases cholangiojejunostomy with T-tube drainage , 28 cases lobectomy of liver, 16 cases plastic repair of hepatic portle duct with cholangiojejunostomy.

其中肝胆管切开取石T管引流术47例，肝胆管切开取石+胆肠吻合术39例，肝部分切除术28例，肝门部胆管切开成形+胆肠吻合术16例。

Results 114 cases were follow-up surveyed from 6 months to 10 years, After operation,the superior rate and residual stone rate were respectively: it was 67.57%、54.05% in the group of choledocholithotomy with T-tube drainage ,78.38%、32.43% in cholangiojejunostomy with T-tube drainage , 96.15%、7.69% in lobectomy of liver, 85.71%、14.29% in plastic repair of hepatic portle duct with cholangiojejunostomy. The total superior rate was 79.82% and total residual stone rate was 31.58%.

结果 114例得到随访，随访6 月～10年，术后优良率及残石率分别：肝胆管切开取石T管引流术67.57%、54.05%，肝胆管切开取石+胆肠吻合术78.38%、32.43%，肝部分切除术96.15%、7.69%，肝门部胆管成形+胆肠吻合术85.71%、14.29%，总优良率79.82%、残石率31.58%。

They were male in 8 eases and female in 14. Pancreatic cystoadenoma was in 14 cases (63.30%) and pancreatic cystoadenocarcinoma in 8(36.3%). It was 1.7:1 between two groups. Tumor at the head of pancreas was 6 cases (27.21%). at body-tail 12 (54.5%). at tail 4 (18.1%). Local resection was performed in one case, pancreatic body-tailectomy pancreatoc1uodenectoni in 3, central pancreatectomy in 2 cases.

男8例，女14例。14例(63.3%)为胰腺囊腺瘤（浆液性6例、黏液性8例），8例(36.3%)为胰腺囊腺癌，两者之比为1.7：l肿瘤位于胰腺头部6例(27.2%)，胰腺体尾部12例(54.5%)，胰腺尾部4例(181%)行胰腺肿瘤局部切除术l例，肤腺体尾部切除术14例，胰头十二指肠切除术3例，胰腺中段切除术2例，囊肿－空肠Roux-Y吻合术1例，T管架桥胆－肠内引流术1例。

Results:All 55 patients were diagnosed and treated by laparoscopic procedure successfully including 7 laparoscopic intestinal local resections,10 laparoscopic intestinal diverticular resections,37 laparoscopic partial intestinal resections and 1 laparoscopic right hemicolectomy.

结果：腹腔镜探查不仅明确了所有患者小肠出血的原因，而且同时实施了相应的腹腔镜手术，其中包括7例完全腹腔镜下小肠肿瘤局部切除术；10例腹腔镜下小肠憩室切除术；37例腹腔镜辅助下小肠肠段切除术；1例腹腔镜辅助下右半结肠切除术。

Surgical procedures included eight proximal pancreatectomies, three total pancreatectomies, nine distal pancreatectomies, and three tumor enucleations from the pancreatic head. Three patients had superior mesenteric vein reconstruction, six had liver resection, one had an extended periaortic node dissection, 11 had splenectomy, and one had left nephrectomy.

手术的过程包括8个近端的胰脏切除，三个全胰脏切除，九个末梢的胰脏切除，和三个进行肿瘤切除，三个患者有较佳的中肠静脉重建术，六名进行肝脏再切除，一名患者有广泛的进大主动脉节切除，1人进行脾脏切除，一名患者进行左肾切除术。

The split between the two groups can hardly be papered over.
