Objective To explore the metergasis of patients with rectal cancer before and after Dixon.

目的：探讨行直肠癌前切除术直肠癌患者的术前术后直肠和肛管功能变化。

Research of ultramicro- structure of anal sphincter in the cases of patients with fecal incontinence shows : shortness of smooth muscle cells of anal sphincter , destruction of normal structure of the rest cells , elongation of elastic tissue and increase of collagen fibers. Thus the most familiar cause of copracrasia is Degeneration of finespun smooth muscle of anal sphincter which mantain anal sphincter closed .One of the pathomechanism of fecal incontinence is decrease of anal resting pressure .

对大便失禁者肛门内括约肌的超微结构研究发现：肛门内括约肌的平滑肌细胞缺失、剩余细胞的正常关系破坏、弹性组织拉伸、胶原纤维成分增加，因而大便溢漏最常见的原因是维持括约肌关闭的肌肉-肛门内括约肌的纤细平滑肌的退行性病变，肛管静息压降低是大便失禁发病的一个病理学机制。

Preventing and controlling infection, in addition, used the Chinese native purgative ( compound prescription decoction DA CHENG QI ): Chinese rhubarb 20g ( post - decocting ), mirabilite 15g , officinal magnolia, Fructus aurantii immaturus , the peach kernel , the giant knotweed , the herbaceous peony , the dandelion , fumitory each 15g , fresh licorices 6g , the water dococtum 200ml treatment , by stomach tube pours into group every day two , each time 100ml , retains for two hours , by high retention enema group every day one , each time 200ml , when enema , the buttocks raise 10cm , the liquid level are apart from anus 50—60cm , 7days are a treatment course .

胃管注入组每日两次，每次100ml，保留两小时，期间暂停胃肠减压(中药胃管注入后胃肠减压器即予夹管，两小时后放开)；高位保留灌肠组每日一次，每次200ml，采用左侧卧位，灌肠时臀部抬高10cm，液面距肛门50～60cm，插入肛管约25～30cm，药液温度保持在40～42℃，7天为一个疗程。

Total proctocolectomy and ileostomy is a cure for UC,total proctocolectomy and ileal pouch-anal anastomosis improves defecation control but followed by a high incidence of stomal ulcer.

ResultsThere are no significant differences of general curative effect and recurrence of anal fistula. There are significant differences of anal deformation, anal resting pressure, anal maximal systolic pressure, postoperation complications (leakage of gas and liquid, key-hole deformation).

结果： 两组各15例病人在总疗效、肛瘘复发等方面没有显著性差异，而在肛门变形、肛管静息压和最大收缩压变化及并发症方面有显著性差异。

General curative effect, anal deformation, anal resting pressure, anal maximal systolic pressure, postoperation complications (leakage of gas and liquid, key-hole deformation) and recurrence of anal fistula were observed in the two groups after operation.

术后对两组病人的总疗效、肛门变形、肛管静息压和最大收缩压变化、并发症、肛瘘复发进行观察比较。
