Objective To evaluate the diagnostic value of high frequency ultrasound in cubital and carpal tunnel syndrome.

目的 探讨高频超声对肘管、腕管综合征的诊断价值。

高频超声在肘管、腕管综合征的诊断及鉴别诊断中具有重要价值。

Then sonorgraphic examination was carried out in 20 patients with cubital tunnel syndrome and 10 patients with carpal tunnel syndrome before operation.Results Not only the structure of cubital and carpal tunnel cauld be showed clearly,but also the cause of the cubital and carpal tunnel syndrome could be distinct.

结果 高频超声不仅能清晰显示构成肘管及腕管的骨质、软组织及其内容物，而且能够明确肘管和腕管综合征的病因以及尺神经和正中神经的形态学变化。

The length and width of the arcuate ligament,the depth of the cubital tunnel,the extension of the ulnar nerve and the distance from medial epicondyle to olecranon as the elbow flexed from neutral to 90 degrees were measured.

观察60个肘关节标本肘管的构成，测量肘管弓状韧带的长宽和肘管中部的深度，以及肘关节屈曲90°时肱骨内侧髁和尺骨鹰嘴距离的变化，尺神经被牵拉的幅度。

Methods :60 elbows of adult cadavers were dissected.The length and width of the arcuate ligament,the depth of the cubital tunnel,the extension of the ulnar nerve and the distance from medial epicondyle to olecranon as the elbow flexed from neutral to 90 degrees were measured.

观察60个肘关节标本肘管的构成，测量肘管弓状韧带的长宽和肘管中部的深度，以及肘关节屈曲90°时肱骨内侧髁和尺骨鹰嘴距离的变化，尺神经被牵拉的幅度。

Methods High frequency ultrasonography of normal cubital and carpal tunnel were performed in 30 healthy volunteers.

应用高频超声观察30例正常人的肘管及腕管的超声解剖并对20例肘管综合征、10例腕管综合征患者术前行超声检查。

结论］肘部的创伤及慢性劳损可以导致肘管弓状韧带出现肥厚增生，引起尺神经卡压磨损，这是肘管综合征最常见的病因；其他病因还包括慢性骨关节炎，占位病变和先天异常；尺神经可被机械性卡压和磨损，出现慢性缺血缺氧，导致肘管综合征的发生；详细的体格检查和术前的肌电图检查是诊断肘管综合征的主要手段，在诊断时应注意该病与其他部位迟发性尺神经麻痹的鉴别。

Results:The anterior bundle of the UCL starts from anterior inferior part of the medial condyle,and ends on the sublimis tubercle in front of the coronoid process of ulna,locating in the deep layer of the flexor carpal ulnar muscle.The distance between ulnar nerve and UCL is (0.9±0.1)cm in the center of cubital tunnel,and (0.6±0.3)cm in the distal foramen of cubital tunnel.The distance from the point of ulnar nerve entering the flexor carpi ulnar muscle to the medial epicondyle is (3.7±0.8)cm.The distance between the closest branch innervated flexor muscle of the median nerve and the approach is (1.6±0.5)cm at the level of medial epicondyle,and (0.9±0.4)cm at the level of ulnar tubercle.

结果：UCL前束起于肱骨内侧髁的前下方，止于尺骨冠突内侧的小结节，UCL前束位于尺侧腕屈肌肱头、尺头之间的深层；尺神经在肘管中部与尺侧副韧带前束的平均距离为(0.9±0.1)cm，在肘管的远侧出口，距尺侧副韧带的尺骨止点(0.6±0.3)cm；尺神经至尺侧腕屈肌尺头肌支的入肌点距肱骨内侧髁的距离为(3.7±0.8)cm；正中神经最靠近切口的分支距切口的距离，在肱骨内侧髁和尺骨结节水平分别为(1.6±0.5)cm和(0.9±0.4)cm。
