［Objective］To study the effect of Endtoside neurorrhaphy for protection target muscle.

摘 要：［目的］研究周围神经端侧吻合后靶肌肉及其运动终板的形态，为端侧吻合的应用提供进一步的形态学依据。

Conclusion］ End to side neurorrhaphy can protect target muscle.

结论］神经端侧吻合能保护靶肌肉及其运动终板。

Abstract〕 Objective: To observe the histological changes and nerve muscle junction in the target muscle after end-to-side neurorrhaphy.

摘要〕 目的：研究神经端侧缝合术后靶肌肉的组织学变化和运动终板再生。

Conclusion］ Endtoside neurorrhaphy can protect target muscle.

结论］神经端侧吻合能保护靶肌肉及其运动终板。

Method］twenty six wistar rats were divided into two groups, with 13 in each group, peroneal nerve was transacted on the right side of all the animals, in one group, the proximal end of peroneal nerve was turned back and ligated, the distal end was sutured to the side of tibial nerve with perineurial window, in another group,both ends of the transacted peroneal nerve were turned back and ligated as contrast, no operation was done on the left side.

方法］wistar大白鼠26只，随机分实验组和对照组，每组13只，实验组右侧腓总神经切断后，近端反转结扎，远端与胫神经外膜开窗处行端侧吻合，对照组右侧腓总神经切断后，两端均反转结扎，两组动物左侧作正常对照，不作处理。术后3个月，行双侧腓总神经、胫前肌组织学检查，胫前肌湿重测定，运动终板检查。

Forty-sixe cases of symptomatic dynamic lumbar spinal instability were selected from the Ninth and Second People's Hospital from March 2003 to March 2007. They received dorsal spinal strenosis surgical decompression, titanium JJXA segmental pedicle screw fixation, intervertebral pace brace, intervertebral disc resection, with the sideward and the forward annulus fibrosus retained, endplate cartilage zone curetted, then received intervertebral bone particles graft, and pedicle screw compression. The patients were followed up for more than 15 months. The good and excellent rate was 91.3%.

选择2003-03/2007-03南阳市第九人民医院和南阳市第二人民医院收治的动力性腰椎不稳患者46例，行后正中入路下的椎管减压手术，钛制JJXA椎弓根螺钉置入，并行椎间隙撑开，双侧切除椎间盘保留侧方以及前方的纤维环，刮除终板的软骨层，行椎间颗粒骨植骨，椎弓根螺钉系统加压固定。46例随访时间≥15个月，优良率91.3%。

Vertebral column were excided through dorsal meso-incision. the removed vertebral column were put on ice as soon as possible and L1/2, L2/3, L3/4, L4/5, L5/6 intervertebral disc tissue were procured.

取下整个腰椎段脊柱迅速放在冰面上，用眼科剪刀、镊剥离椎间小关节和椎体前方的软组织，分别从L1椎体下缘和L2椎体上缘分离上、下软骨终板，取出L1/2椎间盘组织。

The sprouts from endplate and a few contact sites of NCAM with AChR can be seen.C: Untreated group on the 20th day after L5 nerve root compression. There are some AChR plaques of morphological abnormality, and no contact sites of NCAM with AChR although the intensity and scope of NCAM staining is increased.D: Yiqi Huayu Recipetreated group on the 20th day after L5 nerve root compression. All the AChR sites are reoccupied by NCAM and the morphology of AChR are restored. E: Untreated group on the 30th day after lumbar nerve root compression. AChR plaques display irregularity in shape, with a few round or spindle shaped cupulae. Meanwhile, some NCAM stainings are still shown to be around the AChR. F: Yiqi Huayu Recipetreated group on the 30th day after L5 nerve root compression.

随着时间的推移，AChR斑的形态渐趋规则，NCAM反应也逐步增强，至造模后20 d达到最高峰，但益气化瘀方组仍然明显优于模型组，其不仅在数值上超过了胚胎期大鼠，且与AChR全部重叠；而模型组仍可见到不少重叠不全且形态异常的AChR斑。L5神经根受压后30 d，NCAM的表达水平开始下降，基本局限于终板区内，其下降速度和范围在益气化瘀方组明显大于模型组；模型组不仅NCAM的表达明显高于益气化瘀方组，且仍能见到一些成熟不全的AChR斑；而益气化瘀方组AChR斑形态已接近于正常。
