Objective To explore the therapeutic effect of total pelvic exenteration for advanced rectal carcinoma.Methods 15 patients with advanced rectal carcinoma were operated with TPE,including total rectal,distal end of sigmoid colon,bladder,posterior urethra,anus,intrapelvic sexual organ and side lymph node,and the survival rate was estimated.

目的 探讨全盆腔脏器切除治疗晚期直肠癌的临床疗效方法采用Miles 手术方式基础，包括肿瘤在内全部切除直肠、乙状结肠远侧段、膀胱、远侧输尿管、后尿道、肛门及盆腔内的生殖器官，同时行盆腔内的侧方淋巴结清扫，并进行随访。

"In summary, we have found that uninsured and Medicaid patients have a significantly elevated risk of being diagnosed with advanced-stage colorectal cancer, and this was seen consistently through different regions of the colon, as well as the rectosigmoid junction and rectum," he said.

他表示，总而言之，我们已经证实没有保险与接受医疗补助的病患，被诊断罹患末期大肠直肠癌的风险较高，且不论是在大肠、乙状结肠与直肠的哪里个位置都是一样，我们相信针对弱势族群改善筛检与提供其它照护资源可能可以降低这种差异。

Results Triology was present in 8 cases(66.7%) and the double signs in 4 cases(33.3%).4 patients(33. 3%)died of malignant degeneration of colonic polyp in 10 years, all occurring in rectum and sigmoid colon,the oldest one was 74 years and the youngest 35 years.

结果 出现三联症者8例(66.7%)，出现二联症者4例(33.3%)。10年追踪观察，其中4例(33.3%)因结肠息肉发生癌变而死亡，息肉癌变的部位均发生在直肠与乙状结肠，死亡年龄最大74岁，最小35岁。
