The unstable fracture of scapular neck can be divided into two main types: absolute instability and relative instability, based on the stability of the glenoid fragment in the scapular neck fracture. The former needs early operation.

根据不稳定性肩胛颈骨折中的关节盂骨折块的稳定程度，可将其分为关节盂骨折块绝对不稳定和相对不稳定两类。

Method］from may 2000 to november 2007,14 patients(10 females,4 males;mean age 34.6 years)with scapular fractures were treated by open reduction and internal fixation through a posterior approach.eight patients were injurred in traffic accidents,3 due to falling,2 by direct violence and 1 due to cut.there were eleven patients with close fractures and 3 with open fractures.according to hardegger classification system,there were 8 cases of scapular body fracture,9 of scapular spine fracture,6 of scapular acromion,10 of scapular neck fracture,6 of glenoid rim fracture and 4 of glenoid fossa fracture.

车祸伤8例，高处坠落伤3例，直接暴力伤2例，刀砍伤1例。闭合性骨折11例，开放性骨折3例。根据hardegger的分型方法，体部骨折8处，肩胛冈骨折9处，肩峰骨折6处，肩胛颈骨折10处，盂缘骨折6处，盂窝骨折4处。其中混合型骨折10例，合并全身多发伤5例；伴有腋神经损伤1例，肩胛上神经损伤2例。通过后入路手术，对不同类型的肩胛骨骨折使用重建钢板或拉力螺钉和钢丝等固定。［结果］14例患者获得随访，随访时间6个月～3年，平均18个月。

From October 2006 to March 2007, 31 shoulders (13 men and 18 women with average age of 54.3) with subacromial impingement syndrome, from our department, were selected to form the patient group, An age and gender-matched of 40 asymptomatic shoulders (18 men and 22 women with average age of 52.8) formed the control group. The acromion index was measured on the Grashey view. The distance from the glenoid plane to the lateral border of the acromion was divided by the distance from the glenoid plane to the lateral aspect of the humeral head to calculate the acromion index.

方法］对本科2006年10月~2007年3月就诊的肩峰下撞击综合征患者31肩（平均年龄54.3岁，男13肩，女18肩）的G rashey位片，计算肩峰指数即肩峰最外侧缘至肩关节盂平面的距离与肱骨近端最外侧缘至肩关节盂平面的距离的比值，通过肩峰指数对肩峰肱骨头相对位置进行定量评估，与正常人群40肩（平均年龄52.8岁；男18肩，女22肩进行比较、分析。
