The available operation range in clivus and anterior cranial base by transsphenoidal opproach was also observed during the operation under the help of neuronavigational pointer. Forty-one patients underwent sublabial, transseptal, transsphenoidal approach to the pituitary region guided by Surgical Microscope Navigation was studied. Microadenomas or cystic tumors perhaps can be operated even in a narrow exposure, but large or giant solid adenomas may need a proper evaluation including changing the operative approach or performing a bilateral submucous inferior conchotomy or posterior ethmoidectomy firstly.

手术中还利用CT导航系统观察棒研究了经口—鼻蝶窦入路在斜坡区和前颅底能够达到的实际有效操作范围，为扩大经蝶窦入路手术在斜坡区和前颅底的应用奠定基础，在国内外尚未见同类报道。41例垂体瘤患者梨状孔平均宽度27.2mm±3.1，下鼻甲根部附着处间距30mm±4.1，中鼻甲根部附着处间距27.9mm±4.3，这是经口—鼻蝶窦入路手术中，对Hardy双瓣鼻窥器张开限制的主要骨性结构，若患者这些径线值明显小于平均值，则可能因为鼻窥器打开不充分而影响手术暴露，当然对于垂体微腺瘤和以囊变为主的垂体瘤可能影响不大，但对于大和巨大垂体瘤必须要有充分的准备，包括评估是否改用其他入路，或先行鼻甲切除再行经蝶手术。

Results There were 8 angiomyolipomas, 2 leiomyomas, 2 multilocular cystic nephromas, 1 hemangioma and 1 mesoblastic nephroma. Only multilocular cystic nephroma and leiomyoma have special characteristics that can be determined from RCC.

典型的平滑肌瘤和多囊性肾瘤在CT上有可与肾癌鉴别的特征性表现：平滑肌瘤为规则圆形，边界清楚，密度均匀，CT值略高于肾实质，无钙化，弱强化；多囊性肾瘤为边界清楚的包块，可见边缘锐利含多个分隔的肿块，分隔之间为囊腔，囊内容物的CT值与水相似或稍高，而错构瘤、海绵状血管瘤、慢性肾脓肿、中胚层肾瘤在CT上无法与肾癌鉴别。

Results:In this group there were 5 mucinous cystadenoma; 4 serous cystadenoma; 4 teratoma; 2 lager ovarian cyst; 4 subserous leiomyoma of uterine. The massive cystic-solid tumor with irregular cystic wall was seen in mucinous cystadenoma and serous cystadenoma; Teratoma appeared non homogenous density with fat density mass; Subserous leiomyoma of uterine showed large cystic tumor with clear edge, and the size of uterine was normal.

结果：5例卵巢浆液性囊腺瘤，4例黏液性囊腺瘤，CT表现为腹盆腔内巨大囊性或囊实性肿瘤，囊壁厚薄不均；4例卵巢畸胎瘤，CT表现为盆腔巨大肿瘤内部密度不均，可见脂肪密度影；卵巢巨大囊肿2例，CT表现为腹、盆内巨大单囊状肿物；4例浆膜下子宫肌瘤，CT表现为盆腔内巨大囊性肿物，边界清晰，子宫大小正常；1例阔韧带平滑肌瘤，CT表现为密度不均匀的实性肿物，增强扫描强化不均。
