畸胎瘤

Results: 4 cases were geminoma, 3 cases were teratoma, 1 case was choriocarcinoma, 4 cases were glioma, 2 cases were meningiomas, pineoblstoma, neuroblastoma, dermoid cyst and arachnoid cyst each had one case. It maybe very difficult if we diagnosed only through MR signals. Due to the multitude origin of histology, we can diagnose through some special signals, such as adipose, cystic degeneration, hemorrhage and implant dissemination. We can also combined with clinical examination which is help to differential diagnosis.

结果：生殖细胞瘤4例，畸胎瘤3例，1例绒毛膜癌，胶质瘤4例，脑膜瘤2例，松果体母细胞瘤、神经母细胞瘤、皮样囊肿及蛛网膜囊肿各1例，仅通过MR信号较难诊断，由于肿瘤组织学来源多，可通过一些特殊信号如脂肪、囊变、出血、种植播散等，并结合临床检查来进行鉴别诊断。

There were 36 males and 40 females,with the average age of 47.7 years.36 patients presented with sciatica and perineal numbness,11 patients had painless mass and the other 20 were identified due to other causes.Among 67 benign neurogenic tumors,54 were originated from S1-S3 nerves,3 from S4-S5 nerves and 11 from the presacral space without caudal involvement.Giant cell tumor was found in 60 patients,which included 24 males and 36 females,with an average age of 32 years.Upper sacrum（S1-S2） was involved firstly.There were 56 patients（average,37.7 years） diagnosed as other primary benign bone tumors,which included 24 cases of teratomas,epidermoid cyst or dermoid cyst.Other malignant tumors,including chondrosarcoma （17）,Ewing′s sarcoma/PNET（14）,multiple myeloma（12）,lymphoma（6） and osteosarcoma（6） were found in 64 patients.

其中脊索瘤95例，男62例，女33例，平均年龄55.7岁，骶尾区疼痛是主要症状（82例），半数以上患者伴有坐骨神经痛，肿瘤多先累及低位骶骨（S3~S5）；神经源性肿瘤76例，男36例，女40例，平均年龄47.7岁，神经纤维瘤43例，神经鞘瘤24例，恶性神经鞘瘤9例，主诉多为坐骨神经痛、会阴部麻木（36例）和无痛性包块（11例），20例患者为查体或其他原因检查时发现，67例骶骨良性神经源性肿瘤起源于S3以上神经者54例，起源于S3以下神经者3例，发生于骶前未累及骶管者10例；骨巨细胞瘤60例，男24例，女36例，平均年龄32岁，腰骶尾部不适（37例）、坐骨神经痛（21例）是主要的临床表现，肿瘤多先累及上位骶骨（S1~S2）；骶骨其他原发良性肿瘤及瘤样病变56例，包括畸胎瘤、皮样囊肿、表皮样囊肿共24例；其他原发恶性肿瘤64例，包括软骨肉瘤17例，尤文肉瘤14例，多发性骨髓瘤12例，成骨肉瘤6例等。

From 1961 to 1978, 40 cases of intrathoracic terato-dermoids were operated upon in Capital Hospital, among which 10 cases of giant tumors were analyzed.

首都医院1961～1978年共手术切除40例胸内畸胎瘤及畸胎囊肿，本文分析其中巨大的10例，直径均大于17厘米，最大的1例囊内液体达5000毫升。

Results: Malignant transformation of MCT amounted to 1.4% of all MCT. Of the 11 cases, 7(7/11) were squamous cell carcinoma, 3(3/11) adenocarcinoma, and 1 (1/11) malignant struma ovarii.

结果：卵巢成熟囊性畸胎瘤恶变占良性畸胎瘤的1.4％，其中鳞癌变7例（7／11），腺癌变3例（3／11），卵巢甲状腺肿恶变1例（1／11）。5年平均生存率80％，Ⅰ期为100％，Ⅱ～Ⅲ期为0。

Results; MRI showed such components as fat,fibre,and liquid in teratoma,and large vessels in fibrolipomatous angioma;lytic bone destruction in giant cell tumor...

结果：畸胎瘤含脂肪、纤维和液体等成分；纤维脂肪血管瘤含有粗大血管；骨巨细胞瘤呈溶骨性破坏，1例在T2WI有液-液平面；神经源性肿瘤通过骶孔与骶管内病变相连，神经节母细胞瘤和神经外胚层瘤破坏骶孔；9例脊索瘤在T2WI有条状低信号纤维结构和高信号粘液基质；未分化癌和淋巴瘤T1WI呈略低信号，T2WI呈略高和较高信号；19例转移瘤有多处骨骼破坏，5例只有骶椎侵犯。

The postoperative pathology confirmed that cortical carcinoma was found in 5 cases and pheochromocytoma in 20 cases, malignant pheochromocytoma in 4 cases, malignant fibrous histiocytoma in 1 case, neurocytoma in 1 case, myelolipoma in 1 case, dysembryoma in 1 case, dysembryo-sarcoma in 1 case, well-differentiated liposarcoma in 1 case respectively.

术后病理提示皮质癌5例；嗜铬细胞瘤20例；恶性pheo 4例；恶性纤维组织细胞瘤1例；节神经细胞瘤1例；髓样脂肪瘤1例；畸胎瘤1例；畸胎肉瘤1例；高分化脂肪肉瘤1例。

Results 531 cases postoperatively diagnosed as endometriotic cyst,254 cases teratoma,139 inflammatory masses,121 serous cystademona,67 cases mucinous cystadenoma,128 parovarian,140 simple ovarian cyst,3 cases brenner tumor,1 case borderline mucinous cystadenoma,1 case mucinous cystadenocarcinoma,1 case endometrioid carcinoma and 1 case malignant brenner tumor.

结果术后诊断卵巢内膜异位囊肿 5 3 1例、卵巢畸胎瘤 2 5 4例、炎性肿块 13 9例、卵巢浆液性囊腺瘤 12 1例、卵巢粘液性囊腺瘤 67例、输卵管系膜囊肿 12 8例、卵巢单纯性囊肿 14 0例、卵巢纤维上皮瘤 3例、粘液性交界性囊腺瘤 1例、粘液性囊腺癌 1例、卵巢子宫内膜样癌 1例、卵巢恶性纤维上皮瘤 1例。

Objective To analyze teratomas and teratoid cyst of the head and neck.

目的 头颈部畸胎瘤及畸胎样囊肿分析、总结。

Mature teratomas of benign tumors, and said, skin samples cyst is one of the most common ovarian tumor, ovarian tumors by 10%— 20%, germ cell tumors of 85%— 97%, 95% teratomas.

成熟畸胎瘤属良性肿瘤，又称皮样囊肿，是最常见的卵巢肿瘤，占卵巢肿瘤10%～20%，占生殖细胞肿瘤85%～97%，占畸胎瘤95%以上。

Objective To evaluate the ovarian teratomas that misdiagnosis or missed diagnosis by sonography and discuss the ultrasound characteristics in the rare ovarian teratomas and reasons for misdiagnosis.

目的 分析超声误诊或漏诊的卵巢畸胎瘤，探讨少见组织类型畸胎瘤的超声表现及复杂盆腔条件下畸胎瘤的误诊、漏诊原因。
