To observe the efficacy and safety of balloon dilatation using a flexible bronchoscope in the management of benign tracheobronchial stenosis.

目的 观察经支气管镜球囊扩张术治疗良性气管支气管狭窄的疗效和安全性。

Results: This group included 31 TCCF patients with the characteristics: the incidence rate of this disease for male is higher than that for female; the average age is 38.2 year-old, the male average age is lower than that of female. The common clinical symptoms are pulsatile intracranial bruit, pulsating exophthalmos and congested and edema conjunctiva, which usually appeared in 7 days after the trauma. These symptoms even appeared two months after the trauma in exceptional patients. Among the 31 patients, 13 patients didn't get timely diagnosis and treatment when they had presented typical symptoms.

临床症状主要表现为搏动性突眼、颅内杂音和结膜充血水肿，症状出现时间主要在外伤后7天内（25/31），个别患者在伤后近两个月出现症状。31例患者中，13例患者出现首发症状后没有得到及时诊断和治疗。31例患者均行DSA确诊，其中29例患者采用球囊栓塞瘘口，1例行弹簧圈填塞海绵窦，1例行覆膜支架封闭瘘口。

但Massoud模型目前存在以下主要缺点：第一，模型的建立需要复杂的神经介入技术和昂贵的介入材料(球囊、微弹簧圈、血管内电凝等)，操作费时，又大大增加了研究费用；第二，Massoud模型虽然首次具备了畸形血管团，但它远不是我们临床上常见的含有高流量动静脉瘘的脑AVM；第三，颈动静脉吻合口自发性血栓形成进而导致引流静脉闭塞的机率很高；第四，Massoud模型的长期自然过程和动静脉吻合口的长期通畅性尚不清楚，目前仅仅作为急性期模型。

Objective To assess the clinical efficiency of the trilumen single balloon catheter newly designed for percutaneous transvenous mitral commissurotomy.

The percutaneous balloon mitral valvuloplasty was performed in the patient with single thickened tendinous cordae.The valvular closed commissurotomy was performed in the patient with thickened non-calcified tendinous cordae.The ATC stereoscopic images that revealed by dynamic 3-D reconstruction technique could be not acquired in two-dimensional image technique.

结果：本组瓣膜病患者，有两组腱索增粗并钙化者及腱索稀疏与断裂者均行瓣膜置换术，仅单组腱索增粗者行瓣膜球囊扩张术，两组腱索增粗而无钙化者行瓣膜闭式分离术，动态三维重建技术展示了二维显像所不能获得的房室瓣腱索的立体图像。

From indirect comparison of case series eidence, the procedures appear to proide similar gains in pain relief while for balloon kyphoplasty there is better documentation of gains in patient functionality and quality of life.

Stent should be soft, with good lateral bending property, and diameter of the stent should be 1.0-2.0 mm longer than the actual stenosis. Implantation technique should be improved. These can reduce extrusion and cutting action of the atheromatous plaque due to balloon dilatation and stent expansion, and prevent plaque defluxion during implantation. Filter protection device is used to filter tissue debris and to prevent microemboli entering cerebral circulation, which can elevate safety of carotid stent implantation.

选择相对柔软的，侧弯性能较好，且直径应比狭窄处实测直径大1.0~ 2.0 mm的支架，提高置入操作技术，可以减轻支架置入过程中球囊扩张及支架膨胀时对粥样斑块的挤压与切割作用，防止斑块脱落，同时使用滤器脑保护装置以过滤较大组织碎片，阻止较大微栓子进入脑循环，提高颈动脉支架置入的安全性。

Surface tear of the endomembrane and atherosclerotic plaque induced by balloon dilatation, mechanical expansion and carotid sinus draw-off can adjust compliance of the carotid wall, change sensitiveness of carotid artery receptor, cause instability of hemadynamics and defluxion of plaque fragments, lead to hypotension, bradycardia, hypertension and vascular embolism. How to control and reduce occurrence of above-mentioned complications is a main problem in present studies.
