术后治疗

The post-operational patient on liquid or semiliquid diet injected insulin subcutaneously before three meals,applied inˉsulin according to blood sugar level until end of perioperative period.

目的 探讨糖尿病对耳鼻咽喉—头颈外科患者手术后并发症的影响以及糖尿病患者的围手术期治疗方法对51例接受耳鼻咽喉及头颈部不同类型手术的糖尿病患者，观察其围手术期并发症，并于术 3天改三餐前0.5h皮下注射速效胰岛素治疗，根据血糖的测定结果，开始剂量约为4～8U；对术后禁食需要补液的患者，按葡萄糖3～4g/胰岛素1U的比例给药，并每日监测血糖指标；术后进流质或半流质饮食的患者，于三餐前皮下注射胰岛素，按血糖测定情况给药，直至围手术期结束。

［Objective］To explore the technique of arthroscopy plus radiotherapy in the treatment of pigmented villonodular synovitis of the knee and to evaluate its clinical value［Method］A total of 14 cases of pigmented villonodular synovitis of the knee were treated from January 2000 to July 2003Intraarticular excision under arthroscope with a combined application of routine intrusments and Bipolar radiofrequency was performed on all these 14 cases,and postoperative radiotherapy for diffused pigmented villonodular synovitis of the knee,13 patients were followed up for 10~45 months,and were evaluated for range of motion,pain,swelling effusion［Result］Thirteen cases according to the International Knee Documentation Committeecriteria,subjective knee score was (587+64) and (865 ±57) respectively before surgery and at the last followup［Conclusion］In case of pigmented villonodular synovitis of the knee joint,arthroscopical synovectomy can make good surgical results,and postoperative radiotherapy is an effective treatment for diffused pigmented vilionoclular synovitis of the knee

作者：王晓旭，廖瑛，黄华扬，席雅文，李朝旭，韩志关节镜；放射治疗；膝关节；滑膜炎；色素绒毛结节性摘要：［目的］探讨采用关节镜结合放射治疗膝关节色素沉着绒毛结节性滑膜炎的方法及其临床应用价值。［方法］2000年1月～2003年7月，共收治14例膝关节色素沉着绒毛结节性滑膜炎患者，采用关节镜下常规器械加双极射频进行关节内病变切除，术后弥漫性色素沉着绒毛结节性滑膜炎辅以放疗，13例通过10~45个月的随访，了解患膝活动度、疼痛、肿胀情况。［结果］13例随访患者，术前国际膝关节评分委员会膝关节功能主观评分为（587±64）分，最后随访时，IKDC膝关节功能主观评分为（865±57）分。［结论］通过关节镜能完成膝关节色素沉着绒毛结节性滑膜炎滑膜较彻底切除，对弥漫性膝关节色素沉着绒毛结节性滑膜炎辅以术后放疗是一种有效方法。

The intervention of chemotherapy could worsen the quality of life during the treatment of neo-adjuvant chemotherapy combined with surgery which may be attributed to the side reaction, but such adverse reaction may not affect actually the postoperative subjective feeling; On the other hand, the colorectal surgery may not decrease the quality of life although which could lead more early postoperative uncomforting.

MDT诊治模式下新辅助化疗联合手术综合治疗过程中，化疗干预手段会使生存质量降低，这可能与其副反应相关，但其不良反应可能并不会实际影响到术后患者的部分主观感受；而结直肠癌外科手术干预可能是造成患者术后近期内更多的不适感的主要原因，但并不会额外增加综合治疗后患者的生存质量负担。

Result: the occurrence rate of the chylous fistula after neck dissection was 3.47%; 4 cases of them in operation were cured after ligation in, sewing firm, the ramming of local omohyroid, the chylous fistula after neck dissection did not appear; 6 cases of them after operation was cured by conservative treatment,8 cases were cured through the surgical operation again.

结果：乳糜漏的发生率为3.47%；术中并发乳糜漏4例经手术治疗未出现乳糜漏；术后并发乳糜漏14例中6例经保守治疗治愈，术后8例经再次手术治愈。

Thirty patients with 60 sides who were diagnosed as GC were treated with the minimally invasive operation and rehabilitation detacher.

结果：GC微创综合治疗疗效1周评分平均为9.2分，3周评分为10分，术后4~6天即可恢复出院；GC大转子后切口治疗疗效1周评分平均为6分，3周评分为9分，术后12~14天拆线出院。

Objective:To evaluate the best method for the treatment of postoperative discitis.

目的 ：探讨术后椎间盘炎的最佳治疗方法。方法：回顾分析 5 3例术后腰椎间盘炎的治疗过程。

结果：本组术后腰间盘炎的发生率为1.3%，2例有腰部剧痛、痉挛伴下肢抽搐，余4例呈腰部剧痛伴发热，患者血沉及C反应蛋白均升高，4例行手术治疗，术中取材细菌培养均阴性。2例行保守治疗，6例病人均在术后2个月内症状明显减轻，，3～6个月随腰椎间隙融合而症状缓解。

Result: 276 cases were successfully healed by Duodenoscopy Among 382, the success rate is 72.25%, the difference of the bile collectd on 3,5,7th day after operation between the treatement and control group was significant.

结果：十二指肠镜取石成功276例，成功率为72.25%，治疗组在术后第3、5、7天BD管引流胆汁量与对照组相比较，有显著性差异：治疗组在术后第3、7天TBIL、ALT、ALP与对照组相比较，有显著性差异。

The patients in treatment group received rhGH 10 U hypodermically daily for 10 days 24 hours after liver transplantation.

两组术后均予营养支持及免疫抑制剂治疗，其中治疗组于术后24 h给予rhGH10 U/d皮下注射10 d。

Results Contrast CT or MRI showed prominent irregular circular or nodular enhancement in 14 of 16 cases (64%),can not distinguished from postopertative changes, radioinjury, tumor residue or recurrence.Among them, 9 cases showed tumor residue or recurrence based on significantly increased FDG uptake, 5 were confirmed by pathologic study. The other 5 cases of cerebral necrosis and 2 cases with postoperative cerebral malacia demonstrated FDG uptake defects.

结果 16例中87.5%(14例)CT和MRI表现不规则环形或结节状明显强化，不能准确作出手术后改变和放射治疗后脑损伤、残存肿瘤或肿瘤复发的诊断；其中9例残存肿瘤或肿瘤复发病灶18F-FDG PET影像表现为葡萄糖代谢明显增高，有5例得到手术病理证实；另5例术后放射治疗后脑损伤患者和2例术后软化灶18F-FDG PET影像均显示局部病变区为放射性分布缺损或明显低下。
