术后治疗

TUVP was undertaken for 310 patients with benign prostatic hyperplasia and 12 cases had severe postoperative hemorrhage, bleeding occured in 1 -25 days following operation, most occurred in 2, 3 weeks, among the 12 patients 6 received conservative treatment, 5 received hemostasis by endoscopic cautery and 1 received open operation.

结果310例行TUVP术患者中，12例发生术后大出血，出血发生于术后1-25d，多发生于2、3周。经保守治疗止血6例，经电切镜止血5例，经开放手术止血1 例。

Results On postoperative day 10, the cumulative nitrogen balance and the level of plasma fibronectin was significantly higher in rhGH group than in the placebo group (P 0.05). 手术人员一方面必须非常熟悉胃十二指肠及其临近器官的正常解剖结构，并在此理论基础指导下进行手术，另一方面还必须具备扎实过硬的操作和对病人高度负责的职业态度，这样才可能尽量避免术中邻近脏器的损伤和线结被剪断，术后缝线脱落，以及减少出血等情况的发生。在施行胃大部切除术时，对切除胃的范围还没有一个精确的定量手段，只能根据临床经验进行掌握。胃切除过多，术后会导致病人胃肠功能不足，出现营养不良，甚至可以出现粒细胞减少而引起腹泻。切除胃过少又可能达不到治疗目的，出现原发病的复发，如溃疡复发，残胃癌等。

对117例术后患者行336例次扩张治疗，扩张时间为术后1～6个月。

术后两组患者结石残留率经统计分析无明显差异，KUB复查提示两组疗效相当，差别无显著性意义，提示术后短期内(7天)结石残留率取决于手术情况，中医介入治疗不能改变短期结石残留率，但长期疗效有待进一步研究。
