A review of the literature in Taiwan revealed a lack of recent systematic analysis on dysphasia. The purpose of this article was to discuss dysphasia within the concept analysis framework provided by Walker and Avant (2004). This article verified and confirmed the conceptual characteristics of dysphasia; identified antecedents and consequences; and elaborated on previous cases (including model, borderline, contradictory, and related cases).

虽然目前有不少的文献探讨吞咽障碍的评估与处置，但并无针对吞咽障碍作有系统的概念分析，故本文依Walker及Avent(2004)提出概念澄清与分析方法来进行吞咽障碍的概念分析，广泛性地查证此概念之定义及确认吞咽障碍的概念性特徵，并且列举典型、边缘与相反案例作说明，同时确认吞咽障碍发生的前因与后果并阐述相关实证资料。

RESULT: 1.Ouabain act on lens sodium-pump,under the LM,lens anterior capsular membrane discontinuous,epithelium cells clustered,occluding zonule seperated,lens fiber layers fractured.Under the EM,cells totally hollowed,mitochondria swelling,myelin figure appeared.RT-PCR examine the expression condition of αsubunit of sodium pump on mRNA level,α1、α2 and α3-isoform are all decreased.2.Digoxin act on lens sodium-pump,under the LM,lens cell oedema,linkage distructed,extensive exfoliation.Under the EM,plasma appeared little half-transparant hollow region,mitochondria swelling and ridge disappeared. RT-PCR examine,α1、α2 and α3-isoform are all decreased.3.Amphotericin B act on lens sodium-pump,under the LM,lens epithelium cells linked tightly,arranged in-line,lens fiber layers arranged tightly and regularily.Under the EM,abbundant cellular organes,exuberant cells function indicated. RT-PCR examine the expression condition of αsubunit of sodium pump on mRNA level,α1 and α3-isoform are increased significantly,demonstrated isoform-specific action.4D-thyroxine act on lens sodium-pump,under the LM,lens plasmalemma integrated,cells arranged tightly and regularily.Under the EM,nucleus fission appeared,desmosome half-desmosome and tensile microfilaments linked the cells. RT-PCR examine,α2 and α3-isoform are increased, also demonstrated isoform-specific action.5.Vitamin E act on lens sodium-pump,under the LM,lens anterior capsular membrane continuous and smooth,epithelium cells tightly linked,lens fiber layers appearede hollow region occasionally.Under the EM,lateral membrane high density belt appeared,abundant nucleolus. RT-PCR examine,onlyα1-isoform are increased, demonstrated significantly isoform-specific action.6.DMSO act on lens sodium-pump,under the LM,lens anterior capsular membrane slightly thicker,cells linkage partly distructed.Under the EM,plasmalemma denaturation,mitochondria swelling.RT-PCR examine,α1、α2 and α3-isoform are all altered slightly and haven't significant meanning.

结果：1、哇巴因作用于晶状体钠泵后，光镜下晶状体前囊膜断裂、上皮细胞聚积、闭合连接分离、纤维板层裂隙，电镜下全层细胞空泡化、线粒体肿胀出现髓样结构，RT-PCR法检测晶状体钠泵α亚单位α1、α2及α3三种重整异构体在mRNA水平的表达均减弱。2、地高辛作用于晶状体钠泵后，光镜下晶状体细胞水肿、细胞连接破坏、广泛剥离，电镜下胞质见少许半透明空化区、线粒体肿胀嵴消失，RT-PCR法检测晶状体钠泵α亚单位α1、α2及α3三种重整异构体在mRNA水平的表达均减弱。3、两性霉素B作用于晶状体钠泵后，光镜下晶状体上皮细胞紧密连接、线状排列、纤维板层紧密规整，电镜下细胞器丰富、细胞功能旺盛，RT-PCR法检测α1及α3表达显著增强、具有一定的重整异构作用特异性。4、D甲状腺素作用于晶状体钠泵后，光镜下晶状体质膜完整、细胞排列紧密规整，电镜下胞核见分裂像、细胞间有桥粒、半桥粒及张力微丝，RT-PCR法检测α2及α3表达均增强、亦有一定的重整异构作用特异性。5、维生素E作用于晶状体钠泵后，光镜下晶状体前囊膜连续光滑、上皮细胞紧密连接、纤维板层偶见空化，电镜下囊侧膜内有高电子密度带、细胞核仁丰富，RT-PCR法检测仅有α1的表达显著增强、具有极强的重整异构作用特异性。6、二甲基亚砜作用于晶状体钠泵后，光镜下晶状体前囊膜轻度增厚、细胞连接部分破坏，电镜下质膜变性、线粒体肿胀，RT-PCR法检测晶状体钠泵α亚单位α1、α2及α3三种重整异构体在mRNA水平的表达无显著改变。

Methods: Between November 2003 and December 2005, 67 patients with renal cell carcinoma underwent retroperitoneal laparoscopic radical nephrectomy (34 patients, group A) and open radical nephrectomy (33 patients, group B). Operating time, blond loss, amount of postoperative drainage, time to ambulation, recovery of intestinal function after operation, the postoperative hospital stay, use of antalgesic and transfusion blood and plasma were compared between group A and group B, retrospectively. Results: In group A, the operations of 31 patients were successful and 3 cases failed, then conversed to open surgery. Firstly, one case were not decented the prerenal fascia and the other two cases occurred complications, one case was due to injurying genital gland vein, with blood loss of 500m1, another fatty renal capsula was too thick to explose the renal pedicle. The operations of group B were all successful, only one case was died of intracerebral hemorrhage after two monthes.

回顾性分析和比较后腹腔镜肾癌根治性切除术(Retroperitoneal laparoscopic radical nephrectomy， RLRN)34例与开放性肾癌根治性切除术(Open radical nephrectomy， ORN)33例，比较两种方法的手术时间、术中出血、术后引流、术后最早下床活动时间、肠功能恢复情况、术后住院时间、术后镇痛剂使用量、术中术后输血、血浆量、术后随访时间及手术效果结果：A组中1例因最初对解剖结构难于辨认，无法分离肾前筋膜至肾门而改行开放手术，1例因损伤精索静脉出血难以控制而改行开放手术，1例因肾周脂肪过多，无法将肾脏掀起显露肾蒂而行开放手术，31例手术成功。B组手术均获成功，1例术后2个月因大面积脑出血死亡，余均无瘤生存。

Methods Thirty ASA I women who underwent gynaecologic operation and requested epidural analgesia were enrolled in the study.Epidural catheter was placed at L2-3 and advanced for 3cm in the epidural space in a cephalad direction.

方法选择30例妇科手术患者，在术前硬膜外给予罗哌卡因10 ml，第1例患者罗哌卡因的浓度为0.660%，下1例患者的药物浓度根据前1患者有无运动阻滞情况来上调或下调1个浓度级。

Fifteen patients undergoing hysteromyomectomy with 0 75% bupivacaine epidural anesthesia were studied to observe the alterations of the levels of plasma atral natriuretic peptide, renin activity, angistensin Ⅱ and aldosterone systems.

研究了15例子宫肌瘤切除术患者应用0.75％布比卡因硬膜外麻醉血浆心钠素和肾素活性、血管紧张素Ⅱ－醛固酮系统的变化及相互调控。结果表明，血浆ANP水平在麻醉10min后各时点明显低于麻醉前（P＜0.05），PRA在麻醉10min后亦明显低于麻醉前（P＜0.01），而AⅡ和AL无显著改变（P＞0.05）。

Methods: Thirty ASA I nulliparous women were administered 10 ml bolus of epidural ropivacaine in the first stage of labor. A concentration of 0.5% ropivacaine was applied to the first woman, and the dosage to the next one was up-regulated or down-regulated one level in accordance to the previous woman's response to the concentration.

选择30例单胎足月初产妇，在进入第一产程活跃期时行硬膜外给予罗哌卡因10ml，第1例罗哌卡因的浓度为0.5%，下一产妇的药物浓度根据前一产妇有无运动阻滞情况来上调或下调1个浓度级。

因此可以确定的说：一定毫无实实在在的我；只有诸识在自己无始开始存在以来，於分段生死中相续不断，自己的存在无始以来前灭后生，在因因果果、果果因因中相续不断，因著业力的虚妄熏习，出现假我的种种现象，就误以为有我，故无知的愚者自以为有ㄧ个实我。（韦达在此将《成唯识论》的「诸识」翻成「几种不同的识」，指的是种子识、末那识、眼、耳、鼻、舌、身、意等八个识，在不同时空分段生死中，所起的外延与内涵、作用不同，所以才用「不同的」various一词。其次，他把「无始时来」翻成「since before the beginning of time」，就尚德对佛法的了解而言，佛法所指的「无始」不是指「时间的本身」，而是指众生生命开始的那个时刻，叫做「无始」。----------译者

In an attempt to evaluate the safety and efficacy of Healon in trabeculectomy, 20 eyes with Healon and 20 eyes without Healon injection after trabeculectomy are compared. Trabeculectomy using Healon will not induce transient rise of intraocular pressure. By filling the anterior chamber with Healon, a tamponing effect results which will maintain a deep chamber and reduce complications such as flat anterior chamber, choroidal detachment and striate keratopathy in the early postoperative period. In long-term evaluation there is no difference in pressure control, conjunctival scarring and corneal endothelial cell loss between Healon group and Non-Healon group.关键词 trabeculectomy , flat anterior chamber , choroidal detachment , intraocular pressure

为了评估Healen在小梁切除术中之疗效及安全性，选20位接受小梁切除术者，同时注射Healon於前房为实验对象，另20位接受传统小梁切除术为对照组，发现使用Healon并不会引起一过性眼压上升的现象，Healon注入前房，产生填塞作用，最大的好处是可以维持前房深度，减少术后合并症如前房消失，脉络膜剥离及角膜水肿等，然长期追踪，对於降眼压的幅度，不因使用Healon而可达到低眼压状态，也不会减少结合膜瘢痕化而促进房水引流，然Healon系一无毒，无炎性，无抗原性高黏稠性的物质，不因使用而发生炎性反应，同时有保护角膜内层细胞的功能。
