斑点状阴影

The patients should be given synthesized therapy including invaded mechanical ventilate, corrected hypoxidosis, high-dose hormone and antibiotics and complications, when the patients with severe chest trauma appear progressive emptysis, dyspnea, cyanopathy, and stains or pieces of high-density shadow in chest X-ray.

严重胸外伤患者如出现进行性咯血、呼吸困难、发绀及肺部斑点状或片状高密度阴影要考虑到严重肺挫伤并发急性呼吸窘迫综合征的可能，应早期进行有创机械通气、纠正低氧血症、大剂量激素和抗生素及并发症治疗等综合治疗措施。

CT of lung show diffused distribution of plaque and spottiness with high density cavity with inequlity of size and ellipse nodus with sharpness edge and high density. Cases received treatment with Amphotercin B combination fluconazol by vein and intrathecal injection of Amphotercin B.Five cases were cured and five were improved.

5胸片表现为两肺散在分布小片状、小结节阴影，边缘模糊、内见不规则低密度区；CT示两肺广泛分布斑片状和斑点状密度增高影、大小不等的空洞或类圆形边缘光滑高密度结节。

There were nodes, spots and reticular shadows in hilus of lung scatteredly and their diametrers up to 2～5 mm.

肺门及两肺野可见散在的结节或散在的斑点状阴影或边缘不清的网状阴影，直径2～5 mm。

Results:31 cases (34.8%) had bronchovascular shadows with the small spot shadows;32 cases (35.9%) had clouded shadows;26 cases (29.2%) had patchy shadows and pulmonary cyst; We also can find pneumothorax, hydropneumothorax, mediastinal emphysema, subcutaneous emphysema, fracture of costal bone.

结果：肺纹理有改变伴有小斑点状阴影31例(34 8%)；出现云雾状及磨玻璃状阴影32例(35 2 %)；有大片模糊的较高密度影和肺囊肿影2 6例(2 9 2 %)；另外还发现气胸、液气胸、纵隔气肿、皮下气肿及肋骨骨折等。

Results:31 cases (34.8%) had brochovacular shadows with the small spot shaows;32 cases (35.9%)had clouded shadow; 26 cases (29.2%) had patchy shadows and pulmonary cyst ;We alse can find pneumothorax,hydropneumothorax,mediastinal emphysema,subcutaneous emphysema,fracture of costal bone.

结果：肺纹理有改变伴有小斑点状阴影37例（37%）；出现云雾状及磨玻璃状阴影48例（48%）；有大片状模糊的较高密度影和肺囊肿15例（15%）；另外还发现气胸、液气胸、纵隔气肿、皮下气肿及肋骨骨折等。

In 134 cases of infected pneumonia,100 cases showed patchy and puntual opacities in bilateral lung field,cloud margin of heart and diaphragm were seen in 20 cases,complicated pulmonary hemorrhage and heart failure were found in 14 cases.

结果：236例中，吸入性肺炎102例，其中表现为肺门增宽、肺纹理增粗的24例，表现为斑点状及斑片状阴影的68例，并发肺不张的10例；感染性肺炎134例，其中表现为双肺广泛点片状影的100例，表现为心缘及横膈边缘模糊的20例，并发肺出血及心力衰竭的14例。
