AIM: To observe the expression and distribution of C5a receptor(C5aR) on epiretinal membranestissue from patients with proliferative retreous body retina lesion and cultured retinal pigmentary epithelial cells.

目的 ：检测手术中取出的增生性玻璃体视网膜病变患者的视网膜前膜组织及培养的人视网膜色素上皮细胞中的C5a受体(C5aR)的表达及分布，阐明C5a受体在发生PVR的病理过程中可能的作用。方法：手术取出ERM组织，常规制备石腊切片。

In this study, we evaluated the efficacyof preanesthetic proton pump inhibitor (PPI; oral rabeprazol)use in patients receiving regular H2-antagonisttherapy for more than 4 wk. Forty-eight patients with assumedcomplete tolerance to H2 antagonists undergoing elective surgerywere recruited and randomly assigned to receive either a preanestheticPPI (rabeprazol 20 mg; n = 24) or H2-antagonist (H2 group; roxatidine75 mg; n = 24) at 9:00 pm on the day before surgery and 2 hbefore the induction of anesthesia.

本次研究，作者评价了长期规则使用氢离子拮抗剂（口服法莫替丁，疗程大于4周）治疗的病人，麻醉前使用质子泵抑制剂（ PPI ；口服雷贝拉唑）的有效性。48位择期手术的病人假设能够完全耐受氢离子拮抗剂，随机分为两组，一组接受 PPI （口服雷贝拉唑20mg ； n ＝24），另一组接受质子泵抑制剂（口服罗沙替丁75mg ； n ＝24），服药时间分别为，术前一天晚上9点和麻醉诱导前2小时。

常见的并发症和合并伤依次为：休克、颅脑损伤、胸部损伤、神经血管损伤、脂肪栓塞、腹部脏器损伤和泌尿系损伤。3、多发性骨与关节损伤的治疗以早期内固定效果为好，72h内手术并进行早期功能锻炼的患者远期功能恢第一军医大学2004届硕士学位研究生毕业论文复情况优于超过上述时限手术的患者。4、重视院前急救及院内各科室的急诊救治工作，有效的降低了死亡率，提供了早期处理骨关节损伤的良机。5、早期内固定治疗便于早期开展功能锻炼，可以明显改善伤肢功能，减少了威胁生命的并发症。

The recoery rate was calculated as follows;(JOA score at follow up_preoperatie JOA score)/(17 _ preoperatie JOA score) _ 100,15 and the surgical outcomes were classified into four grades based on the calculated recoery rate; excellent: recoery rate 75% or higher, good: 50–75%, fair: 25–50%, and poor: lower than 25%. Ealuation was made on radiographic findings, including the range of motion and the jaw diameter (a measured line from the posteroinferior corner of the ertebral body to the anterior aspect of the subjacent lamina) at the adjacent leels in group A and the affected leels in group B and the spinal canal diameter at the C4 leel in both groups. The C4 leel was chosen for this measurement, because the spinal canal is usually narrowest at this leel.

恢复率如下计算；（随访JOA评分-术前JOA评分）/（17-术前JOA评分）*10015，按计算的恢复率将手术结果分4级；优：恢复率75%或更高，良：50-70%，中：25-50%，差：低于25%。X线片结果评价，包括活动范围和颌直径（从椎体下后角到下邻椎板前方的测量线）在A组相邻水平与B组受影响水平及两组C4水平椎管直径。C4作为该侧量的选择是因为通常在此平面椎管最为狭窄。

Abstract］ objective to explore the experience in the diagnosis and treatment of small bowel diverticular surgical complication.methods 24 cases of small bowel diverticular were retrospectively analysed.results the diagnosis was made preoperatively in 4 duodenal diverticula and only 1 jejunal diverticula.clinical complications included obstruction in 11 cases,bleeding in 4 cases,perforation in 4 cases.23 cases were cured.1 case was died of mof.all the diverticular with complications had histopathological signs of inflammation and 3 had signs of ulcer and 4 had the resected.conclusion the small bowel diverticular is rare.it may diagnose except common reason underwent gastrointestinal bleeding or obstruction.the gi is an effective way of examinations.operation is effective for small bowel diverticular disease.

回顾性分析24例小肠憩室病外科并发症的临床资料。结果术前6例十二指肠憩室中4例确诊，18例空回肠憩室仅1例确诊。小肠憩室伴小肠梗阻11例，憩室出血4例，憩室穿孔4例。23例治愈，1例死于多脏器功能衰竭。有并发症的憩室病理都有炎症表现，5例有溃疡，4例有异位黏膜组织。结论小肠憩室病比较少见。在胃肠出血或梗阻的情况下，可采取对常见原因排除的方法进行诊断。在不需要急诊手术的情况下，钡餐造影是最有效的检查。手术是可靠有效的治疗方法。

手术操作不当是导致骶前大出血的主要原因，按盆腔解剖结构正确操作是预防出血的关键，术中压迫法和术后纱布填塞法是处理骶前大出血最有效的方法。

结果：1、骶前止血钉发射器及骶前止血钉的制造是成功可行的。2、实验组术中止血时间为12.25±10.69秒、对照组术中止血时间26.00±11.52秒，实验组显著短于对照组(p=0.01)；实验组术中止血效果比对照组好(p=0.03)；实验组与对照组术中出血量、手术相关死亡率及术中所用止血钉数之间的实验结果比较无统计学意义。
