Objective To investigate the methods and treatment of internal fixation with Titanium mini-plate in the treatment of metacarpus and phalange fracture in metaphysic.

目的 探讨国产微型钛板钉内固定治疗手部掌骨、指骨干骺端骨折的治疗方法及效果。

Results Metaphysis osteoporosis was observed on X-ray and CT in acute stage, the midsection of infarct of bone was isointensity on T1WI and iso/hyperintensity signal on T2WI, the edge of infarct of bone was ring-shaped hypointensity on T1WI and hyperintensity signal on T2WI. Small worm-eaten change and stippled calcification was observed on X-ray and CT in subacute stage, the infarct of bone was iso/hypointensity on T1WI and iso/hyperintensity signal on T2WI, the edge of infarct of bone was ring-shaped hypointensity on T1WI and hyperintensity signal on T2WI. Irregular and wriggle osteosclerosis was observed on X-ray and CT in chronic stage, was hypointensity on T1WI and T2WI.

结果 骨梗死急性期X线、CT表现为干骺端骨质疏松，MR T1WI骨梗死灶中央部分呈等至高信号，T2WI呈高信号，T1WI骨梗死灶边缘呈环形低信号，T2WI呈环形高信号；亚急性期X线、CT表现为小的虫噬样改变，伴有斑点状钙化，MR T1WI骨梗死灶呈等或低信号，T2WI呈等或稍高信号，T1WI骨梗死灶边缘呈环形低信号，T2WI呈环形高信号；慢性期X线、CT表现为不规则状、蜿蜒状骨质硬化，MR T1WI和T2WI均呈低信号。

Methods: Between 1980 and 2002, forty-four skeletally immature prepubescent children and adolescents who were in Tanner stage 1 or 2 (with a mean chronological age of 10.3 years) underwent physeal sparing, combined intraarticular and extra-articular reconstruction of the anterior cruciate ligament with use of an autogenous iliotibial band graft.

从1980年到2002年，我院共收治44例骨骼发育中的Tanner1期或2期青春期前儿童及青少年患者（平均实际年龄10.3岁），采用干骺端保守治疗结合关节内及关节外前十字韧带重建术，同时取自体髂胫束移植。

Physeal sparing, combined intra-articular and extra-articular reconstruction of the anterior cruciate ligament with use of an autogenous iliotibial band graft in skeletally immature prepubescent children and adolescents provides excellent functional outcome with a low revision rate and a minimal risk of growth disturbance.

干骺端保守治疗结合关节内及关节外前十字韧带重建、自体髂胫束移植术治疗骨骼发育中的青春期前儿童及青少年能提供良好的功能恢复，且二次手术率低，并能把医源性生长紊乱的危险降到最低。

MATERIALS: The demineralized bone was prepared with cancellated bone of metaphysis from newborn calf via treatment of decalcification, defatting, acellular, remove antigen, and deproteinization.

材料：取新生小牛四肢长骨干干骺端的松质骨置于脱钙液中脱钙、脱脂、去细胞、去抗原和去蛋白等程序处理制备成脱钙骨支架。

Xrays of limb bones may show reduced density and thin cortices and in some cases fractures in metaphyses.

对四肢骨骼的X光检测显示出骨量减少，皮质变薄，甚至干骺端碎裂。

Method］twelve cases with extremities invasion bone tumor were reviewed,including 6 male and 6 female.the average age was 37.1 years old.according to the mri,typeⅰtumor located in metaphysic and growth eccentricity,and was treated with outside tumor osteotomy with reservation of one side as integumentale,the cut line was shape of "匚".type ⅱ tumor was with invasion part articular surface,and was operated with part joint osteotomy with reservation of one side as integumentale,the cut line was shape of"l".type ⅲ was with invasion total joint surface and as integmentale,all tumor segment including joint could be cut,but the shape of cut close to the diaphysis was echelonment.

方法］12例侵袭性骨肿瘤患者男6例，女6例；平均年龄37.1岁。根据mri表现选择不同截骨方法，ⅰ型肿瘤位于干骺端偏心性生长，采用保留一侧皮质骨的肿瘤外截骨，截骨线呈&匚&形。ⅱ型病例肿瘤侵蚀部分关节面，采用保留一侧皮质的部分关节截骨，截骨线呈&l&形。ⅲ型病例肿瘤侵蚀整个关节面和骨皮质，可切除包括关节在内的整个瘤段骨，只是近骨干侧截骨呈阶梯状。
