objective to raise the level of regarding congnition,prevention,diagnosis and therapy on multiple organ dysfunction syndrome.methods report the process and correlative therapy on one case who occured with mods involving 6 organs after hemorrhagic shock lead to heartbeat and respiration stopping suddenly and resuscitated resulting in left nephrostome titanium nip falled off accidentally from left kidney lymphatic fastening.at the same time,the author reviewed correlative literature and introduce the investigative progress about mods.results one case who happened with mods following hemorrhagic shock,heartbeat and respiration stopped suddenly was diagnozed and resuscitaed.6 organ dysfuncted including brain,heart,lung,liver,kidney and the digestive system.after being given therapy integrated chinese and western medicines,allopathy and nutritional treatment,all circadian parameters of the case came back in gear and discharged at last.conclusion the mechanism about mods is very complicated.because of serious infection,hurt and oxygen-lack,excessive inflammatery reaction activates multifarious cell factors and inflammatery mediums,which improve the happen-rate of mods.wiping off pathogeny,providing life-sustain treatment and recognising sirs and mods in time and adopting corresponding therapy,such as treatment integrated chinese and western medicines,can improve the hit-rate of cure on mods.

作者：王静恩，蔡金芳，王志华，谢晓洪多器官功能障碍综合征；中西医结合；治疗；休克，出血性目的提高对多器官功能障碍综合征（multiple organ dysfunction syndrome，mods）的认识、预防、诊断、治疗水平。方法报告成功抢救1例经腹腔镜左肾淋巴管结扎手术后左肾静脉钛夹意外脱落失血性休克，导致心跳呼吸骤停复苏后再手术并发mods累及6个脏器功能衰竭过程及有关治疗，并复习相关文献，介绍目前关于mods的研究进展。结果确诊1例因失血性休克心跳呼吸骤停复苏后并发mods，累及脑、心、肺、肝、肾、血液、胃肠等6个脏器。经积极采取中西医结合对症、支持救治，各生理指标全部恢复正常，康复出院。结论 mods的发生机制非常复杂，在严重感染、创伤、缺氧等打击下，失控的过度炎症反应激活多种细胞因子和炎症介质的释放，使mods发生率升高。尽早去除病因，尽早给予各种生命支持治疗，尽早对可能发生全身炎症反应综合征、mods识别并给予干预治疗，包括中西医结合治疗能改善mods的救治成功率。

Some electrophysiological index were measured, including atrial effective refractory period, rate adaptability, dispersion of AERP, inducibility and duration of induced atrial fibrillation under different basic pacing cycle length (300, 250 and 200 ms). Results: Contrast with control group, the values of AERP at 6 sites were shortened and maladaptation.

将6对双极记录电极分别缝于高位左、右和低位左、右心房外膜以及左、右心耳，采用心脏程序期前刺激法(S1S2)，分别测量基础起搏周长为300、250和200ms时的心房有效不应期、AERP频率适应性、AERP离散度、房颤诱发率及平均持续时间等电生理指标。

Rats with CHF were divided randomly into two groups, one group was injected dexamethasone(1mg /kg) intramuscularly at the first and forth day respectively,the other group was injected equi-volume saline likewise, and observing the changes of symptoms of rats with CHF; measuring rats hemodynamics index including blood pressure, heart rate and left ventricular end-diastolic pressure 4 days later. Part 2: To measure every group rats myocardialα1、β1、β2、β3-AR density by immunohistochemical method ,and to understand the AR density changes .Taking the first part rats'myocardium tissues for testing myocardialα1、β1、β2、β3-AR density and 5 normal rats for normo-contrast group and taking gray scale of slice as AR density. Part 3: The study enrolled 35 patients with DCHF. Patients were included if they had orthopnea and refractory edema due to acute DCHF that was severe and had taken more 1 week hospitalization and intravenous therapy in addition to diuretics.

观察心衰大鼠的症状，4天后对两组大鼠进行血流动力学检测，检测指标包括血压、心率和左室舒张末压；第二部分：取第一部分实验动物大鼠的心肌组织，并取5只正常大鼠的心肌组织作为正常对照，用免疫组化法检测大鼠心衰前后及糖皮质激素治疗前后大鼠心肌组织肾上腺素能受体α1、β1、β2、β3受体的变化，取镜下切片的灰度值反应受体的密度；第三部分：研究对象为临床难治性心衰病人35例，病人的选择标准包括由于急性失代偿的心力衰竭出现的端坐呼吸，难治性水肿，患者病情严重，住院超过1周，包括利尿剂之外药物的静脉治疗。

The ECG shows evidence of severe left ventricular hypertrophy with prominent precordial voltage, left atrial abnormality, lateral ST-T abnormalities and a somewhat leftward QRS axis (-15 degrees).

Methods: Randomly divided 80 cases with chronic cardia failure into two groups, regular therapy group and bisoprolol group with 40 cases per group. Regular anticardiac failure therapy, including cardiac and diuretic therapy, was employed to patients in regular therapy group; while the combined therapy of regular therapy and bisoprolol was employed to patients in bisoprolol group. Compared the heart rate, left ventricular diastolic dimension, left ventricular systole dimension, left ventricular ejection fraction before and after therapy and efficiency of both groups.

80例慢性心力衰竭患者随机分为两组，常规治疗组40例，采用强心、利尿等抗心力衰竭常规治疗，富马酸比索洛尔治疗组40例，在常规抗心力衰竭治疗的基础上加用富马酸比索洛尔治疗，对两组临床疗效及治疗前、后心率、左心室舒张末内径、左心室收缩末内径及左心室射血分数的变化进行比较。

There were no nick infection, chorioamnionitis and other complications. Eight/sixteen fetal lambs were undergone intro-utero cardiac intervention. The values of body weight and Hct of lambs which were executed. pre-term and full-term delived were (1.77±0.14) kg vs.(2.15±0.23) kg vs.(2.41±0.19) kg and 29%-33% vs. 27%-35% vs. 37%-41%. In autopsy, hydropericardium was found in 4/8 with 1-2.5 mL and 1/8 with 5 mL. Besides 1/8 with interventricular septum centesis injury, there was no centesis damage to endocardium, valve, chordae tendineae, papillary muscles and aortic tunica intima.

病理解剖发现施行手术的8例胎羊/新生羊胸壁穿刺点均位于左侧心前区，心脏进针部位在左室心前区近心尖部位6例，近流出道1例，近室间隔1例；4只少量心包积液(1~2.5 mL)，1只心包积液5 mL，3只未发现心包积液；除1只胎羊有室间隔穿刺伤外，其余胎羊心内膜、瓣膜、腱索、乳头肌及主动内膜均未发现损伤；未发现肺、肝等胸腹腔脏器穿刺损伤；均未发现心包、胸腔感染迹象。

Results: Thrombus within right ventricle and main pulmonary artery was detected in 6 patients, right atrium and RV enlargement was found in 19 patients, right ventricular hypokinesis was found in 21 patients, hypokinesis of interventricular septum was detected in 13 patients, MPA and right pulmonary arterial dilation was found in 22 patients, marked tricuspid regurgitation was detected in 23 patients, pulmonary hypertension was found in 23 patients.

右心异常改变者26例（包括超声直接检出血栓的6例），其中右房室增大19例；右室收缩功能减退21例；室间隔左移、左室短轴切面室间隔向左室膨突呈&D&型改变1例；室间隔运动幅度减低13例；主肺动脉及右肺动脉增宽22例；三尖瓣中量以上返流23例；肺动脉收缩压增高23例，范围在33~81mmHg (62.87±23.46 mmHg)，治疗后肺动脉压明显下降，范围在21~52 mmHg (34.26±13.52 mmHg)。

观察20例正常人和37例冠心病患者胸骨旁左室乳头肌短轴观、心尖四腔、两腔观各节段心肌舒张期心内膜位移引起彩阶的变化，并运用ICK定量分析软件计算出心内膜于舒张早期30﹪时间内位移的面积占整个舒张期内膜位移面积的百分比，即心内膜位移指数。
