［Purpose］ To summarize the experience of bronchoplasty and pulmonary artery reconstruction in the treatment of central NSCLC.

［目的］支气管肺动脉成形术中央型非小细胞肺癌的体会。

Methods］ Thirty?鄄two cases with central NSCLC treated with bronchoplasty and pulmonary artery reconstruction in our hospital from June 2000 to June 2005 were analyzed.

方法］ 分析我院胸外科自2000年6月至2005年6月采用支气管肺动脉成形术治疗中央型非小细胞肺癌32例的临床资料。

我院胸外科自2000年6月至2005年6月采用支气管肺动脉成形术治疗中央型非小细胞肺癌32例的临床资料。

The pathology of bronchoscopic tissue biopsy showed small cell lung cancer.

经支气管镜切片的病理报告为小细胞肺癌。

Results 6 patients with PAP presented with ground-glass opacification of the bilateral lungs on HRCT. Typical chest HRCT scan showed "map-like" lesions in 3 cases and "cobble stone-like" lesions in 3 cases. Multifocal consolidative opacities in the lungs were seen on HRCT in 3 cases. Lung biopsies from 6 patients with PAP revealed a prominent positive periodic acid-Schiff intra-alveolar exudates. There were normal alveolar spetasis in 3 patients while interstitial thickening, fibroblast hyperplasia, interstitial fibrosis in the others.

结果 6例患者在不同层面可见磨玻璃影，其中2例可见小结节影；3例表现为两肺斑片状磨玻璃影与周围肺组织分界清楚，呈地图样改变；3例因磨玻璃影与小叶间隔增厚交织成铺路石样改变；3例可见肺泡实变融合成密度较高的斑片状阴影，1例在肺泡实变区可见&空气支气管征&。6例患者活检肺组织在光镜下显示肺泡腔内充满大量块状或颗粒状嗜伊红物质，PAS染色阳性，AB染色阴性，其中3例肺泡间隔正常，3例肺泡间隔增宽，可见慢性炎性细胞浸润、成纤维细胞增生和胶原沉积。

The consolidation and ill-defining diffuse nodule appeared in 8 cases, air bronchogram in 11 cases, bulging of the interlobar fissure in 4 cases,heterogeneous consolidation in 7 cases, cystic airspaces in 6 cases, and the CT angiogram sign in 2 cases, the consolidation were peripherally distributed in 15 out of 18 cases.

结果：18例肺炎型肺癌影像学表现两肺多发性分布7例，叶性分布8例，段性分布3例，所有病例均有实变存在，外周分布的15例，伴有不规则小结节的为8例，空气支气管征的11例，叶间裂向外突出的4例，实变影不均匀分布7例，不规则小囊腔6例，CT血管征2例。

Imaging finding of pneumonic-type bronchioloalveolar carcinoma shows a peripheral consolidation, air bronchogram, bulging of the interlobar fissure, CT angiogram,cystic airspaces and so on.

肺炎型肺癌表现为典型的外周分布，空气支气管征、不规则小结节、叶间裂外突、多发小囊腔样改变等，但与感染性肺炎很难区分。

Results Mottling and patchy infiltrats were observed in 42 patients, irregular shape cavities in 41 patients, 28 patients had satellity lesions, 8 had fibrotic stripes, 10 had mottling califacations, 2 had lymphy node enlargement.

结果得到42例均见斑片状高密度灶影，其中41例不规则空洞影，4例与支气管相通；28例卫星灶影，8例纤维索条影，1例仅见小斑片状影伴卫星灶影，10例见斑点状钙化影，2例淋巴结肿大等。

结果9例患者呈实体瘤样生长，多数分界清楚、质地均匀、带有毛刺和分叶的软组织密度影，阴影内缺乏支气管充气征和小泡征。

Results After 2 or 6 hours of LPS intravenous administration, severe lung injury and positive signals of NT occurred with the latter located in the pulmonary macrophages, endothelia, subendothelial and muscular layers of the pulmonary artery.
