Here we present a case of 21 year-old male who suffered from recurrent swelling in left side of mandible which had received surgery for ossifying fibroma in another hospital when he was 19 years old. Obvious facial asymmetry was noted but no sensory dysfunctions were detected. The lesion showed well-demarcated, multilocular radiolucency but radiopacity centrally. We applied segmental resection via extra-oral approach and reconstructed with anterior iliac crest bone graft. After removal of MMF in six week, the wound healed well and joint function was good without malocclusion. After follow-up of one year, no recurrences were noted.

本病例为－21岁之男性，在19岁时因为左下颚骨角区肿大在某医院接受外科手术处理，术后病理报告为骨化纤维瘤，因为复发的肿胀而来本院寻求治疗，临床检查并无疼痛及下唇区感觉异常的问题，放射线影像为周界清晰，多腔室放射透过性合并中央放射不透过性之不规则小块，手术方式为从口外颚下切线，做下颚骨片段式切除，并以肠骨脊海绵骨移植修补骨缺损加以颚间固定六周，术后伤口恢复及愈合良好，颚骨关系稳定无咬合异常的情况，无张口受限的问题。

Comclusion: transpetrous presigmoid retrolabrinth approach was rational approach for surgery on petroclivus and -4- CPA; it抯 important for accomplish the surgery to study and master the anatomy and variance of this approach; there was relative anatomical relationship among those structure on posterior of petrous bone; AAMP was the extracranial mark of inflexion of TS and SS; LAM and PSC were obvious bone marks for operation; we can confirm the abraded range of petrous bone lean upon LAM and/or GSS beside abrading step by step as literature, and it抯 unnecessary and unrational to emphasize to show PSC if the surgical space was enough; preceding sigmoid sinus and high jugular bulb often appeared at right and would bring disadvantage effect; interspace ,between CN V and CNVII or CNVII and CNJXI, were available to reach clivus and manage the mass on the range.

经SS前迷路后入路是处理岩斜坡区、桥小脑角区病变理想的手术入路；研究和熟悉手术入路中的解剖结构及部分变异对指导手术十分必要；岩骨内测面的骨性结构之间有着相对稳定的空间关系；顶乳缝前角是SS和TS转接处在颅外的标志；IAM和PSC等结构可以作为手术中的重要骨性标志；以PSC确定岩骨的磨除范围是较安全和可靠的，但亦可以结合影像检查，以内耳道和乙状窦前缘为基点判定切除范围，如果操作空间足够，过分强调磨出半规管是不适当的；右侧GSS的宽度和深度较对侧大，SS前置和颈静脉球高位较对侧多见，这些都对右侧入路有不利的影响，但仍可以进行手术操作；三叉神经和面神经间隙及面l 神经和舌咽神经间隙方便到达斜坡并处理病变。l

The anteroposterior projectionof the foot, utilized to examine the foot disease in the past was referred as to the freely-stressed method. However, most of the patients on suspicion of Hallux Valgus get pains while standing with weight-bearing. Additionally, the patients' real conditions can not be diagnosed without standing. it is helpful for us to apply both the foot weight-bearing and the foot non weight-bearing so as to probe the individual difference between the HVA and the IMA 1(superscript * intermetatarsal angle and further to provide the informative resources concerning the assessment of performing surgery operation. The study include 40 patients who we having Hallux Valgus of different degree, The findings have revealed the significant difference among 40 patients after the method-weight-bearing AP of the foot projection given.

过去一般常用诊断足部疾病的足部摄影(anteroposterior projection of the foot； AP projection of the foot)是指未施加压於病人足部的拍摄方法，然而多大脚趾外翻的病患常於站负重时感觉疼痛舒服，而未负重摄影却无法呈现出病人站时的真实态，因此我们用足部前后位负重摄影(weight-bearing AP projection of the foot)与足部未负重摄影(non weight-bearing AP projection of the foot)的影像比较其大脚趾外翻角(Hallux valgus angle， HVA)及第一跖骨间的角1(上标 st intermetatarsal angle， IMA的差，进而提供外科手术的估评。
