剔除

Objective:To discuss the operative method and skill,postoperative curative effect and clinical value of laparoscopic hysteromyomectomy and laparoscopic accessorial hysteromyomectomy.

目的：探讨腹腔镜下子宫肌瘤剔除术和腹腔镜辅助下子宫肌瘤剔除术的手术方法、技巧、术后疗效及临床应用价值。

Results: 8 cases of patients were vaginal delivery; cesarean section delivery in parallel with hysteromyomectomy were 21 cases;1 case was multiple fibroids and the diameters were＞5 cm, with more bleeding and a larger wound during the operation d, so we took total hysterectomy to avoid infection; 1 case of cervical fibroids, which did not be removed in the cesarean section because of its special position.

结果：患者阴道分娩8例；行剖宫产术分娩并行子宫肌瘤剔除术21例；1例为肌瘤多发，且直径均＞5 cm，术中出血较多，且创面较大，为避免感染和出血行全子宫切除；1例为子宫颈肌瘤，因其特殊位置，剖宫产时未行剔除术。

Mid-pregnancy treatment of uterine fibroids:① myoma less than six centimeters in diameter, and asymptomatic, and regular prenatal checkups, the vast majority do not need special treatment;② leiomyoma diameter greater than 6 centimeters, with the the growth of uterine fibroids may continue to increase, while the large red-like leiomyoma easy change for the sake of stimulating the uterus to contract or peritoneal irritation, obstetricians refer only recommended in patients with bed rest and application of analgesic agent to conservative treatment, it is less recommended during pregnancy line hysteromyomectomy surgery, only as a last resort circumstances do myomectomy.

妊娠中期子宫肌瘤的处理：①肌瘤直径小于6厘米，且无症状者，定期产前检查，绝大多数不需特殊处理；②肌瘤直径大于6厘米，随着子宫的增长肌瘤还可能继续增大，而大型肌瘤易有红色样变而刺激子宫收缩或有腹膜刺激症状，介时产科医生只是建议患者卧床休息及应用止痛剂等进行保守治疗，很少建议在妊娠期行子宫肌瘤剔除手术，只有在不得已情况下才行肌瘤剔除术。

Results 128 patients were successfully treated by transvaginal surgery, without anyone tranformed to ventrotomy, and any trauma of proximate organs, such as bladder, rectum, in which there were 52 cases of total hysterectomy, 76 cases of myoma removal with 486 myomas removed. The avarage time of surgion was 68 minutes (56~112 minutes), that of gastrointestinal function was 23 hours (11~35 hours), that of hospitalization was 7.1 days (6~8 days).

结果 128例病人全部经阴道手术成功，无1例中转开腹，无邻近脏器损伤，其中，完成子宫切除术52例，施行肌瘤剔除术76例，共剔除肌瘤486枚，手术时间平均68min(56~112min)，术后经过均良好，胃肠功能恢复时间平均23h(11~35h)，无1例感染，住院时间平均为7.1d(6~8d)。

It also eliminates the invalid paths between statuses considering the restriction of minimum continuous time of on and off , and tamps rates , thereby reducing the number of paths in dynamic programming and attaining the aim of reducing dimensions . After confirming the running or suspended status of units , the approach is used to distribute powers among units , thereby increasing the efficiency of dynamic programming greatly .

通过比较各时段负荷及机组参数，剔除各时段下不满足要求的组合状态，从而减少动态规划中的状态点数；根据机组的最小连续运行，停运时间限制，计及机组功率上升，下降速度的约束，剔除了状态点间的无效路径，从而减少了动态规划的路径个数，达到降维的目的；在确定机组启停状态后，再采用解析法进行机组的功率分配，可以大大提高动态规划方法的效率。
