内疝

Objective To assess the value of helical CT on the diagnosis of intestine entocele.

目的 探讨螺旋CT对小肠内疝的诊断价值。

Baby two years old my family that much, but often this happens, I also go to hospital from her reading, but also consulted the Doctor, general lead to a child has a stomachache so several reasons: one, intestinal cramps. 2, intestinal parasitic diseases. 3, mesenteric lymphadenitis, appendicitis, diverticulitis Meikle. 4, congenital malrotation. 5, inguinal hernia, internal hernia (diaphragmatic hernia, obturator hernia) 6, intussusception, volvulus.

我家孩子两周岁多，也常有这种情况发生，我也特意去医院领她看过，也咨询过医生，一般引发宝宝子肚子疼的原因有这么几种：1、肠痉挛。2、肠寄生虫病。3、肠系膜淋巴结炎、阑尾炎、米克尔憩室炎。4、先天性肠旋转不良。5、腹股沟疝、内疝6、肠套叠、肠扭转。

Results: Both sides of elder hernia sac became constrictive because of fibrosis caused by A ring, C ring chronic inflammation, which made hiatal hernia sac dilate like a expanded balloon, 7 cases were manifested stenotic at the two ends. Lithiasis in the hiatal hernia sac display of filling defect the same as the smoothly sac without aerogenic powder. Long axis of filling defect were accorded with that of esophagous, the sign could be described as "cast mould" of esophagous hiatal hernia sac.

结果：由于老年性疝囊两端的A环、C环在长期慢性炎症的刺激下纤维化而狭窄，使疝囊膨胀表现为&吹气球征&，7例疝囊上下两端狭窄。7例疝囊内结石未服用产气粉前表现为与疝囊同等大小且边缘光滑的充盈缺损，充盈缺损的长轴与食道长轴方向基本一致，呈现&食管疝囊铸型征&。

Spiral CT and image reconstrution have impotant value on the diagnosis of intestine entocele.

螺旋CT扫描和图像重建对诊断小肠内疝有重要的价值。

Methods The CT results of 10 case of intestine entocele which have proved by operation were reviewed. The main reconstruction techniques was MPR. Result The main phenomenon of intestine entocele: Orificium hernialis were found in 8 cases.

对10例经手术证实且术前有CT资料的小肠内疝病例进行回顾性分析，4例行CT平扫，6例行CT平扫和双期增强扫描，并进行多平面重建。

Methods :25 cases of traumatic hydrocephalus were analysed retrospectively.Among them,3 cases showed brain contusion,4 with subrarchnoid hemorrhage,5 with subdural effusion,2 with epidural hematoma,1 with subdural hematoma,1 with fracture of skull,3 with malacia lesions,3 with cerebral infarction,and 2 with cerebral hernia.The other 3 cases were not associated,with intracranial traumatic anomaly.

回顾分析25例外伤性脑积水，其中分别伴颅脑损伤有3例脑挫伤、4例蛛网膜下腔出血、5例硬膜下积液、1例颅骨骨折、3例脑内软化灶、3例脑梗塞、2例硬膜外血肿、1例硬膜下血肿、2例脑疝(大脑镰下疝及钩回疝各1例)，另3例颅内未见外伤性异常改变与其并存。

Results One case with transverse mesocolon pore hernia, one with obturator hernia and one with paracecal hernia were confirmed by exploratory laparotomy. There were six cases whose hernias resulted from the fall of export jejuna into import jejuna after Billroth II anastomoses in front of colons, five from the pelvic bottoms' peritoneal fissure and four from the fissure appearing between descending colon and lateral abdominal wall after Miles' operation, one from greater omental adhesion to sigmoidal wall after sigmoidostomy, and one from inflammatory adhesion on the basis of Meckel's diverticulum. The other 17 cases' hernias (48.7%) were caused by the fissure among celiac adhesive bands, peritonea, celiac organs, enteric walls and intestines.

结果 剖腹探查手术证实为横结肠系膜裂孔疝1例；闭孔疝1例；盲肠旁疝1例；毕Ⅱ式结肠前吻合术后输出段空肠疝入输入段空肠与横结肠系膜间隙6例；Miles术后盆底腹膜裂开所致内疝5例；Miles术后降结肠与侧腹壁形成的间隙致内疝4例；大网膜与乙状结肠造口肠壁粘连形成内疝1 例；1例在梅克尔憩室基础上发生炎性粘连形成腹内疝；因腹腔粘连带与腹膜、腹腔脏器、肠壁、肠与肠之间的孔隙改变形成内疝17例（48.7%）。

Methods 28 cases with BPH and inguinal hernia, 10 cases underwent suprapubic transvesical prostatectomy and preperitoneal inguinal herniorrhaphy through hypogastric median incision and 18cases underwent Filling prosthesis of no -tension hernia through oblique hernia incision and TURP.

其中10例下腹正中切口，盆腔内腹膜前疝修补术及耻骨上经膀胱前列腺摘除术；18例腹股沟斜疝切口，无张力疝修补术及经尿道前列腺切除术。

Clustering displacing of small朾owel loops were found in 9 cases. Reduced enhancement of intestion wall were found in 5 cases. Demonstrated as a saclike mass between stomach and pancrase were found in 2 cases.

结果 小肠内疝主要表现为：8例有疝口，9例有肠管聚集、肠系膜走行异常，5例显示肠壁强化减弱，2例显示胃和胰腺之间囊实性肿块；肠梗阻、腹水。

Setting an observation hole in navel position, puncturing the needle in the projection shadow position of the hernia inner loop, high position ligature of hernia sac, injecting 1ml biological albumin glue into the hernia sac before tightening the catgut, tightening the catgut, interring the knot under the skin.

于脐孔处置观察孔，在患侧疝内环口体表投影处穿刺进针，行高位结扎疝囊，收紧缝线前疝囊内注入生物蛋白胶1ml，收紧缝线、线结埋于皮下。
