结果 4例髋臼骨折中 3例行手术治疗，2 0例胫骨平台骨折中 15例行手术治疗(12例行植骨)，11例距下关节骨折 7例行手术治疗。

Results:When unilateral anterior superior iliac spine loaded by transient stress,0-10 ms,stress distributed from posterior superior iliac spine to anterior superior iliac spine,and on the last 10 ms,there were greater stress distribution around cacroiliac joint,acetabulum and pubic branch.When unilateral positive backside of ilium loaded,between 0 ms and 20 ms,stress transmitted along vertical direction of ilium,and after 20 ms,there were greater stress distribution on cacroiliac joint,both pubic branch,ischium branch and acetabulum.

结果：冲击载荷作用于单侧髂前上棘时，0～10ms主要的应力沿髂后上棘到髂前上棘分布，在冲击后期10ms以后，骶髂关节、髋臼和耻骨支都会产生较大的应力分布；作用于单侧髂骨正后方部位时，0～20ms主要的应力沿髂骨纵行传导，应力分布并没有在冲击峰值10ms时达到最大，而是从20ms之后，应力开始向骶髂关节、双侧耻骨支、坐骨支以及髋臼等部位传导并可见到明显的应力分布。

Current application for hip arthroscopy include management of labral tears, osteoplasty for femoroacetabular impingement, thermal capsulorrhaphy and capsular plication for subtle rotational instability and capsular laxity, lateral impact injury and chondral lesions, osteochondritis dissecans, ligamentum teres injuries, internal and external snapping hip, removal of loose bodies, synovial biopsy, subtotal synovectomy, synovial chondromatosis, infection, and certain cases of mild to moderate osteoarthritis with associated mechanical symptoms.

目前髋关节镜主要应用于盂唇撕裂的处理、股骨髋臼的撞击症的骨成形、隐匿性髋关节旋转不稳的热关节囊紧缩或关节囊皱折术，治疗软骨损伤、剥脱性骨软骨炎、圆韧带损伤、内源性或外源性弹响髋，清除游离体、滑膜活检、滑膜次全切、滑膜软骨瘤病、感染和某些伴有机械症状的轻度至中度骨性关节炎。

Thirty-six patients had radiographic evidence of femoroacetabular impingement at the time of revision. Revision procedures included 34 (95%) for femoroacetabular impingement, 32 (87%) for labral lesions, 26 (70%) for a chondral defect, 23 (62%) for lysis of adhesions, and 13 (35%) for previously unaddressed instability.

翻修术时发现有36例患者在股骨头髋臼接触面存在侵犯，关节唇损害32(87%)例，关节软骨缺损26 (70%)例，需要粘连松解术的23 (62%)例，以前关节存在不稳定因素的有13 (35%)例。

Recent research shows hip arthroscopy can provide early diagnosis and treatment hip unexplained pain, such as femoroacetabular impingement, labral team, chondral lesions, furthmore hip arthroscopy shows good prospects in the treatment avascular necrosis of femoral head, synovial abnormalities, post hip arthroplasty discomfort.

研究表明，镜关节镜对于既往不明原因的髋痛如髋关节撞击征、髋臼盂唇、髓臼软骨损伤可以早期诊断和治疗。此外，髋关节镜在股骨头坏死、股骨头圆韧带损伤、髋关节置换后的应用也展现出极大的应用前景。

The authors found that the prealence of protrusio increased to approxi- mately 35.9% by the age of twenty years and then it plateaued; howeer, its presence did not correlate with osteoarthritic changes as measured with the Iowa hip score in patients more than forty years of age.

作者发现髋臼前突的发病率在20岁时增加到35.9%然后进入平台期，但其表现与骨关节炎改变并不成比例（IOWA髋指数，年龄大于40岁）；对有症状的患者来说，髋关节前突与髋部疼痛之间呈正相关关系，关节间隙宽度与髋前突间呈负相关关系，提示髋关节前突的力学因素对关节退变有一定作用，但可能不是主要因素。

Evaluate MRI representation:(1) segregation condition: transfixation band, segregation between cartilage and bone, segregation between bone and bone;(2) four stages of pathologic classification based on MRI signal character;(3) texture in weight bearing zone;(4) add Bone Marrow Edema score and joint effusion score as stasis index.

术前MRI评估：(1)股骨头内分离情况：贯穿带、软骨与骨分离，软骨下骨与骨分离；(2)股骨头内信号的病理分期；(3)冠状正中位的髋臼负重区下结构；(4)将股骨头骨髓水肿评分和关节积液评分相加记为关节的瘀积指数。

Methods From March 1994 to December 2002, on the basis of the ordinary operative method, Pemberton method was improved for 48 cases of DDH 49 articulatio coxae which had superficial and small acetabula but big heads of femur. Osteotomy points were moved higher to enlarge area of bone flap turned over. A piece of full thickness ilium including periosteum was removed as a free bony graft. Capsula articularis growing thicker was made into 2 layers, of which the inner layer was used to tighten articularis and the outer layer was used to stabilize bone flap.

1994年3月～2002年12月，对DDH髋臼浅而小、股骨头大的48例49个髋关节，在通常术式的基础上，对Pemberton截骨术式进行改良，将截骨点上移，扩大翻转骨瓣面积；取髂骨全层骨板连带附着的骨膜植骨；增厚的关节囊制成两层，内层紧缩关节，外层稳定骨瓣。
