下鼻甲

Objective To compare and appraise concha nasalis inferior partial excision methods and under the nose endoscope the pliers clamp with plasma treatment for chronic hypertrophic rhinitis.

目的 比较和评价下鼻甲部分切除术和鼻内镜下钳夹法联合等离子治疗慢性肥厚性鼻炎的效果。

Methods: Among these 32 patients with narrow nasal valve,10 were operated with anterior inferior turbinectomy,4 with deviation orthopedic of protruding trian gular outer cartilage (2 on the left,2 on both sides),6 with bone-widen operation of osseous pyriform aperture(2 on the left,4 on both sides).The nasal ventilation before and after operation was observed.

32例鼻瓣膜区狭窄患者，行下鼻甲前端切除术10例，鼻外侧软骨凸出矫正术4例(左侧2例、双侧2例)，骨性梨状孔狭窄扩大术6例(左侧2例、双侧4例)，观察手术前后鼻通气改善情况以判定疗效。

Methods:①To observe important landmarks of bony of PPF、the neurovascular structure and its relations with the surrounding structures via endoscopic middle meatal transpalatine approach、middle meatal transantral approach and inferior turbinectomy transantral approach respectively in 10 fresh cadaveric heads(20 sides).

①通过对10具(20侧)成人新鲜尸头分别采用鼻内镜下中鼻道经腭骨入路、经上颌窦入路、下鼻甲切除经上颌窦入路进行解剖，观测手术径路中穿经结构以及重要血管神经的毗邻关系。

Results The pathology of all 10 cases of nasal lymphomas were T cell Non-Hodgkin's lymphoma, Ann Arbor stage Ⅰ～Ⅱin9 and 1 in stage IV. The locations of 10 lymphomae were in the anterior portion of one of the nasal cavities, and the lymphomae extended posteriorly along the inferior and/or media nasalis concha in different extent. No bone structure destruction was found in all stage I cases and the bone of the media nasalis concha as well as the uncinate process was partially absorbed in the stage IV nHL. The skin of the external noses adjacent to the lymphoma of the whole 10 cases were swollen in various severity with disappearance of the subcutaneous fat.

结果 10例鼻腔淋巴瘤病理均为T细胞非何杰金淋巴瘤，9例为Ann arbor I～II期，1例Ann Arbor IV期。10例病变均发生于一侧鼻腔前部，沿下鼻甲向后呈不同程度蔓延。9例I期病变均无骨质破坏，相邻鼻背侧皮肤不同程度肿胀，皮下脂肪消失，1例IV期病变可见患侧中鼻甲及钩突骨质部分吸收，同侧眶内肿瘤侵犯。

Methods Total 349 patients, 265 case with chronic hypertrophic rhinitis and 203 case underwent two partial inferior turbinectomy and 62 case underwent partial inferior and middle turbinectomy, 46 case with nasal polyps and chronic sinusitis and underwent nasal polypectomy and transnasal ethmoidectomy, 38 case with deviation of nasal septum and underwent septoplasty.

349例鼻腔术后患者（慢性肥厚性鼻炎265例，行双侧下鼻甲部分切除术的患者203例，行中下鼻甲部分切除术患者62例；鼻息肉摘除、筛窦开放术患者46例；鼻中隔矫正术患者38例）随机分为两组，A组181例采用&层叠式&方法鼻腔填塞并分次抽取，B组168例采用传统的&口袋式&方法鼻腔填塞及抽取。
