Methods we developed a surgical technique to correct short nose by increase tissue volume in nasal tip ,nasal alae and the columella. The technique contain three steps:① Wide dissection and prolongation of the outer skin envelope to cover the lengthened framework by a "V" formed surgical approach from columellar base to inner edge of alae.②Silicone or expanded polytetrafluoroethylene implant is used to correct saddle nose and to length nasal framework.③A papilionaceous concha composite graft containing a elliptic ear cartilage and a onlay "V" formed skin and subcutaneous tissue is grafted to inner edge of columella and alae. Results we performed the procedure on 8 patients.

通过8例短鼻缺陷受术者采用蝶形复合耳郭组织游离移植到鼻尖鼻翼以增加鼻尖鼻翼的组织量以达到延长短鼻的目的，手术分为三个步骤：首先从鼻小柱基底到两侧鼻翼内缘做V形切口，广泛分离鼻尾端皮肤粘膜；然后，通过硅胶或ePTFE假体材料间插并缝合于两侧大翼软骨之间延长鼻尖部软骨支架；最后，将蝶形耳后复合组织块移植到切口部以增加鼻尖鼻翼部的组织量，术后观察该法延长短鼻的实际效果。

Methods 30 cases of nasal soft tissue de fects were reconstructed by different kinds of local flaps according to modified Burget′s nasal subunit. The Rintala flaps were used above the lowest level of low eyelids; at the area between low eyelids and the alar groove, the dorsal def ects were covered by ax-like local flaps and defects on side wall were reconstru cted by flaps rotated from the contralateral wall covered by skin graft; when it was below the highest level of alar groove, nasolabial flaps with subdermal ped ical were applied to nasal tip defection, and nasolabial local flaps to alar def ects, axial nasodorsum flaps supplied by the lateral nasal artery to defects inv olving columellae.

Results The flaps totally survived in 30 patients,the distal part necrosed and secondary free-skin graft were further conducted in 2 cases.All soft tissue defects were repaired and their accompanied bone and tendon exposure healed.

结果 30例皮瓣全部存活，2例远端部分坏死，经游离植皮后创面愈合，所有软组织缺损均修复，骨骼、肌腱外露均覆盖。

After transecting the pedicle, implants were placed with standard method on the alveolar through the skin graft.

Results: Three of 6 patients had cervical girdle sensation. Their cervical girdle sensation were obviously relieved when both cervical parts were performed with eschar open decompression. The eschar open decompression were also performed in rest 3 patients in prevention. All 6 patients bad no airway obstruction in the follow treatment. The incision wounds of 4 cases healed spontaneously and rest 2 via skin graft. No bleeding and infection or other severe complication were observed.

结果：6例患者中3例在面颈部出现紧缩感时行双颈部焦痂切开减压，切开后颈部紧缩感全部缓解，3例在未出现紧缩感时即进行预防性切开；6例患者在随后的治疗中未出现气道梗阻，未进行气管切开，均成功救治，4例颈部切开创面换药后自行愈合，2例植皮修复，未出现出血、感染等并发症。
