1999年至2004年，我院连续203例肾动脉严重狭窄的患者，其中男性128例(63.1％)，女性75例(36.9％)，年龄13-82(54±18)岁，病因分别为动脉粥样硬化123例(60.6％)、大动脉炎62例(30.5％)和纤维肌性结构不良18 例(8.9％)，因严重高血压或伴有肾功能不全而进行了肾动脉支架置放术，并随诊6个月，观察手术对患者血压、肾功能的影响。

Abstract］ objective to summarize the methods and experience and analysing of curative effects of open operation combined eswl treating renal staghorn calculi.methods therapeutic results of 8 cases of renal staghorn calculi were analysed retrospectively.results the lithous horn of 8 cases were broken by eswl before operation,then the renal calculus were removed by pyelolithotomy or extended pyelolithotomy.conclusion the method was a good choice of renal staghorn calculi with high stone-free rate,little trauma and less complication.

摘要］ 目的总结开放性手术结合eswl治疗鹿角状肾脏结石方法经验并进行疗效分析。方法回顾分析了8例鹿角状肾脏结石患者的治疗结果。结果 8例患者术前eswl将结石角粉碎，然后应用肾盂切开或肾窦扩大切开手术取出肾盂和肾盏内结石。结论该方法对于鹿角状肾脏结石治疗，结石取净率高，对肾脏损伤小、并发症少，是较为适合的方法。

Methods Twenty four patients with chronic renal diseases and 48 healthy volunteers were examined with SS-EPI DWI at 3.0T MR. In chronic renal disease group, RIF of 14 patients who received renal biopsy was determined as grade Ⅰ and Ⅱ based on the tubule- interstitial damage degree, while RIF of 10 patients with uremia, who did not receive biopsy but had nephrogenic renal atrophy, was categorized as grade Ⅲ.

对照组（48例正常志愿者）和慢性肾病组（24例慢性肾脏疾病患者）在肾脏常规MR扫描后行DWI扫描，慢性肾病组14例行穿刺活检，根据病理结果将肾小管间质损害程度分为Ⅰ、Ⅱ级；另10例肾脏体积缩小的慢性肾功能衰竭尿毒症期患者定为Ⅲ级。

Methods: Forty-four specimens of renal tissues were harvested from urolithic patients,control tissues were harvested from non-urolithic patients (6 specimens) and renal tumorpatients performing nephrectomy (21 specimens). Immunohistochemical technique wasestablished to study the location of VKORC1 in renal tissues. 方法收集肾脏皮质组织标本71例，其中草酸钙结石组44例，对照组27例(肾盂输尿管交界处狭窄和输尿管结核组6例，皮质厚度小于1.0cm；肾肿瘤皮质组21例，皮质厚度大于1.0cm)，应用免疫组织化学法检测VKORC1在组织中的表达部位，应用免疫印迹蛋白半定量法，检测各组VKORC1蛋白的表达，并分析其表达与肾脏积水及皮质厚度等因素的关系。

Methods A total of 36 patients with advanced renal cancer were definitely diagnosed by IVP, US, CT or MRI examination. A 5F Yashiro catheter was introduced into the femoral artery with Seldinger's technique under local anesthesia, abdominal aortagraphy and renal arteriography in both sides of kidneys were performed routinely to display the blood supplying arteries of tumor, thereafter, renal embolization was performed under superselective catherization for chemotherapeutic embolizing. Then the clinical effect was observed, the amount of blood loss and the operation time were recorded. All these results were compared with that of randomly selected 17 patients before surgical operation with advanced renal cancer admitted during the recent 3 years but without embolization.

36例中晚期肾癌患者，均经临床检查、静脉肾盂造影、B超、CT或MRI检查而确诊；采用Seldinger's技术，局部麻醉下经一侧股动脉穿刺插管，引入5F Yashiro导管，常规行腹主动脉及双肾动脉造影，以显示肿瘤的供血动脉，再将导管超选择插管或尽可能接近肿瘤病灶，进行化疗栓塞，观察其临床改善情况，计算术中失血量及手术时间，并与随机抽取我院近3年所做的17例术前未行肿瘤供血动脉栓塞的肾癌患者进行对照。

This may be explained by follows: First, Decoction Shenkangling can regulate the homeostasis of NEI network and reduce the increased activity of NF-κB to restrain the action of inflammatory-sclerotic media; Second, it can redress the disequilibrium of the ratio of TXA2/PGI2 to reform the hypercoagulable state; Third, it can enhance albumin and reduce cholesterol and triglyceride; Fourth, it can amend the renal damage and protect the function of renal tubule and stroma to prevent the transformation from MCD to FSGS; Last, it can lessen urine protein in AN rats markedly and relieve the side effects of prednisone.4. NF-κB, TXA2 and PGI2, along with the renal pathomorphology and so on can all be the indexes of the diagnosis of Symptom of "Kidney Deficiency and Blood Stasis" and the application of Methods of "Benefiting the kidney and Promoting blood circulation" in TCM.5. The curative effects on AN rats by treatment of integrating Traditional Chinese Medicine with prednisone are much better than using prednisone only. And the curative effects on AN rats by treatment of matching Decoction Shenkangling, having the function of "Benefiting the kidney and Promoting blood circulation", with Prednisone are much better than using Prednisone only.6. When it referred to decreasing urine protein, cholesterol and triglyceride and increasing albumin, Decoction Zhibodihuang excesses Decoction Taohongsiwu. However, when it referred to regulating the disequilibrium of TXA2/PGI2 and meliorating renal lesion, Decoction Taohongsiwu exceeds Decoction Zhibodihuang.7. The curative effects on AN rats by treatment of matching Methods of "Benefiting the kidney and Promoting blood circulation" with Prednisone overmatches using Prednisone matching Method of "Benefiting the kidney" or Method of " Promoting blood circulation" only.

益肾活血中药肾康灵能显著改善AN鼠的肾虚血瘀证的临床表现和实验指标，这可能与中药肾康灵能调理AN鼠机体的NEI网络功能稳态，降低异常升高的NF-κB活性，抑制免疫炎症/硬化介质环节，纠正失衡的TXA2/PGI2，改善AN鼠血液高凝状态，提高AN鼠的血清Alb，降低Ch和TG，减轻AN鼠的肾脏组织病理学变化，保护肾小管－间质功能，阻止MCD向FSGS转化；显著减轻AN鼠的尿蛋白水平以及减轻激素副作用有关。4、NF-κB、 TXB2和6-keto-PGF1α以及肾脏病理形态学改变等，都可作为中医肾虚血瘀证诊断和益肾活血法应用的客观指标。5、中西医结合干预AN鼠疗效优于单纯激素治疗；强的松配合益肾活血中药肾康灵组的综合疗效显著优于单纯强的松组。6、知柏地黄汤组在降低尿蛋白、提高血清Alb、降低Ch和TG疗效上优于桃红四物汤组；桃红四物汤组在纠正失衡的TXA2/PGI2和减轻肾脏病理损害方面优于知柏地黄汤组。7、激素配合益肾活血法干预AN鼠在一定程度上优于激素配合益肾法或活血法。
