［Objective］To explore the technique of arthroscopy plus radiotherapy in the treatment of pigmented villonodular synovitis of the knee and to evaluate its clinical value［Method］A total of 14 cases of pigmented villonodular synovitis of the knee were treated from January 2000 to July 2003Intraarticular excision under arthroscope with a combined application of routine intrusments and Bipolar radiofrequency was performed on all these 14 cases,and postoperative radiotherapy for diffused pigmented villonodular synovitis of the knee,13 patients were followed up for 10~45 months,and were evaluated for range of motion,pain,swelling effusion［Result］Thirteen cases according to the International Knee Documentation Committeecriteria,subjective knee score was (587+64) and (865 ±57) respectively before surgery and at the last followup［Conclusion］In case of pigmented villonodular synovitis of the knee joint,arthroscopical synovectomy can make good surgical results,and postoperative radiotherapy is an effective treatment for diffused pigmented vilionoclular synovitis of the knee

作者：王晓旭，廖瑛，黄华扬，席雅文，李朝旭，韩志关节镜；放射治疗；膝关节；滑膜炎；色素绒毛结节性摘要：［目的］探讨采用关节镜结合放射治疗膝关节色素沉着绒毛结节性滑膜炎的方法及其临床应用价值。［方法］2000年1月～2003年7月，共收治14例膝关节色素沉着绒毛结节性滑膜炎患者，采用关节镜下常规器械加双极射频进行关节内病变切除，术后弥漫性色素沉着绒毛结节性滑膜炎辅以放疗，13例通过10~45个月的随访，了解患膝活动度、疼痛、肿胀情况。［结果］13例随访患者，术前国际膝关节评分委员会膝关节功能主观评分为（587±64）分，最后随访时，IKDC膝关节功能主观评分为（865±57）分。［结论］通过关节镜能完成膝关节色素沉着绒毛结节性滑膜炎滑膜较彻底切除，对弥漫性膝关节色素沉着绒毛结节性滑膜炎辅以术后放疗是一种有效方法。

Results Postoperative follow-up lasted for 1.5~9 years, and audition increased more than 30 dB in 17 patients, increased 10~30 dB in 47, no increase in 4 (including 2 cases of artificial auditory ossicles expelling through tympanum because of postoperative infection).

结果：术后患者随访时间1.5~9年，听力提高30dB以上者17耳，听力提高10~30dB者47耳，听力无提高者4耳（其中因术后感染致人工听骨经新鼓膜排出者2耳）。

The intervention of chemotherapy could worsen the quality of life during the treatment of neo-adjuvant chemotherapy combined with surgery which may be attributed to the side reaction, but such adverse reaction may not affect actually the postoperative subjective feeling; On the other hand, the colorectal surgery may not decrease the quality of life although which could lead more early postoperative uncomforting.

Results All patients were followed up for 6 to 18 months after the operation, with an average of 9.2 months. According to Harris' evaluation standard, the excellent and good rate of postoperative hip joint function was 90.8%. In these following up cases, coxa vara was found in 3 cases, delayed union was in 2 cases. No death,no postoperative infection,no deep venous thrombosis,and no fracture nonunion or internal fixation breakage exist.

Methods: Between November 2003 and December 2005, 67 patients with renal cell carcinoma underwent retroperitoneal laparoscopic radical nephrectomy (34 patients, group A) and open radical nephrectomy (33 patients, group B). Operating time, blond loss, amount of postoperative drainage, time to ambulation, recovery of intestinal function after operation, the postoperative hospital stay, use of antalgesic and transfusion blood and plasma were compared between group A and group B, retrospectively. Results: In group A, the operations of 31 patients were successful and 3 cases failed, then conversed to open surgery. Firstly, one case were not decented the prerenal fascia and the other two cases occurred complications, one case was due to injurying genital gland vein, with blood loss of 500m1, another fatty renal capsula was too thick to explose the renal pedicle. The operations of group B were all successful, only one case was died of intracerebral hemorrhage after two monthes.

回顾性分析和比较后腹腔镜肾癌根治性切除术(Retroperitoneal laparoscopic radical nephrectomy， RLRN)34例与开放性肾癌根治性切除术(Open radical nephrectomy， ORN)33例，比较两种方法的手术时间、术中出血、术后引流、术后最早下床活动时间、肠功能恢复情况、术后住院时间、术后镇痛剂使用量、术中术后输血、血浆量、术后随访时间及手术效果结果：A组中1例因最初对解剖结构难于辨认，无法分离肾前筋膜至肾门而改行开放手术，1例因损伤精索静脉出血难以控制而改行开放手术，1例因肾周脂肪过多，无法将肾脏掀起显露肾蒂而行开放手术，31例手术成功。B组手术均获成功，1例术后2个月因大面积脑出血死亡，余均无瘤生存。

There were 12 cases of postoperative complication in group B(42.9%).Conclusion Both hip arthroplasty and AO compressive cannular are reasonable choices for treatment of displaced femoral neck fracture in the elderly, but the results of group A is better than the group B. The rate of postoperative complication of group A is lower than the group B.
