Results: six patients with vagal reflection were cured by intravenous atropine, 2 with pericardial hemorrhage were treated by pericardiocentesis, 1 patient developed hypotension due to occlusion of the mitral valve orifice by the catheter was corrected by shortening the duration of the catheter in left ventricle, the blood pressure of 1 patient with rapid atrial flutter was elevated by intravenous cedilanid, the blood pressure of 1 patient with hypotension of unknown origin was elevated by vasopressor and atropine.

结果：其中迷走神经反射6例，经静脉注射阿托品纠正；心包出血2例，心包穿刺抽液血压至正常；球囊导管阻塞二尖瓣口1例，缩短导管停留在左心室时间，可以避免低血压反应；快速心房颤动1例，使用西地兰减慢心室率，血压回升；原因不明1例，经使用升压药及阿托品后血压改善。

The causes were right auricle injury by violently placing a electric catheter to it (n=2),coronary sinusinjury by violently placing a electric catheter to distal CS (n=3) and by intracardiac electric defibrillation using electric catheter electrodes of high right atrium and CS as current loop(n=1),perforations of right atrial wall (n=3) and left atrial wall(n=2) by sliding up of the transseptal set,and perforation of left auricle by misplacing the mapping catheter of left superior pulmonary vein to it (n=1). 10 cases of acute tamponade occurred during operation, and 2 after operation.12 patients were treated successfully with urgent pericardiocentesis, and no patients required emergency surgery.

引起心包填塞的原因有：①放置电极导管至右心耳时用力过大致右心耳损伤(2例)；②放置电极导管至冠状静脉窦时，因导管不易进入远端而用力过大致CS损伤(3例)；③心内电除颤时采用高位右房和CS电极作回路，致CS损伤(1例)；④房间隔穿刺系统向上滑动穿破右房(3例)和左房(2例)；⑤放置左上肺静脉标测电极时导管误入左心耳致其穿孔(1例)。10例急性心包填塞出现于术中，另2例术后发生。12例患者经紧急心包穿刺引流均成功治愈，无1例需外科急诊手术。

A central venous catheter was inserted for hemodynamic monitoring and blood transfusion. Cardiac tamponade occurred secondary to perforation of the superior vena cava by the central venous catheter. The patient's hemodynamic status became stable after pericardiocentesis. One week later she was discharged with no sequelae during the three month follow-up.

患者术后装置中央静脉导管以利於血行动力的评估及输血，病患进行中央静脉导管术时因上腔静脉穿刺破裂导致心包膜填塞及休克，经紧急照会心脏科医师后立即给予心包膜穿刺术，术后患者血压渐趋稳定情况好转於一星期后出院，经过三个月的追踪亦无后遗症。

Once correctly recognized, pericardiocentesis started via subxyphid routine under flouscopy, including insertion of 145 cm guide wire, sheath and pigtail catheter.

处理方法：患者取平卧位，用18号静脉穿刺针带注射器及造影剂，在X线透视下于剑突下进行心包穿刺。穿刺针进入心包后立即注射5 ml造影剂，若造影剂沿心包分布则肯定穿刺针入心包。然后送入145 cm导引钢丝，并经钢丝送入鞘管及猪尾导管。引流完毕后留置猪尾导管24小时。
