Objective:to study the epidemiologic characteristic of mortality of malignant tumors in zhongshan city.method:selecting the investigated datum of mortality of malignant tumor in zhongshan city as sample uses sas statistics software to analyse and forecast the mortality by time series.results:morlality of male malignant tumor and both male and female malignant tumour showed the risen trend from 1970 to 1989.the trend will be kept in future.both will be separately risen to 121.05/105 and 83.92/105 in 2 000.mortality of female malignant tumour will be kept in about 43.59/105 in 2000.couclusion:mortality of both male and female malignant tumor takes on the trend of diffirent change.the male showed risen trend;the female showed the fluctuation of non-trend random.rising mortality of malignant tumor greatly threaden the crowd health.

目的：研究中山市恶性肿瘤死亡率的流行学特点，为制定防治计划提供依据。方法：以中山市恶性肿瘤死亡率调查资料为样本，应用sas统计分析软件对恶性肿瘤死亡率进行时间序列分析与预测。结果：1970～1989年男性恶性肿瘤死亡率和男女合计恶性肿瘤死亡率均呈现上升趋势且有继续保持的趋势。到2000年将分别上升为121.05/10万和83.92/10万左右；女性恶性肿瘤死亡率呈现无趋势的随机波动且将继续保持下去，到2000年保持在43.59/10万左右。结论：中山市男女恶性肿瘤死亡率呈不同变化趋势，男性恶性肿瘤死亡率呈现上升的趋势，女性恶性肿瘤死亡率呈现无趋势的随机波动。恶性肿瘤死亡率的上升对人群的健康构成重大威胁。

Materials and Methods:236 pathology-confirmed breast lesions(155 malignanty,81 benignancy) and 221 disease-free contralateral breast tissues were examined using EPI-DWIb values=0,1000 s/mm~2TR/TE=8400ms/93.8ms; FOV=30×30cm;matrix=128×128;ASSET=2,slice thickness=4mm.DCE-MR imaging was also performed for accurate location of the lesion using VIBRANT sequence.The mean ADC values of malignant and benign lesions and the disease-flee contralateral breast tissues were calculated from the ADC map for each patient.One-way analysis of variance was applied to compare the mean ADCs of malignant lesions with benign lesions and disease-flee contralateral breast tissues.The threshold ADC value for malignant lesion was determined using a receiver operating characteristic(ROC curve analysis.Each lesion was classified into two major types:mass and non-mass lesion.The diagnostic performance of DWI was compared in breast mass and non-mass lesions.Results:155 malignant and 81 benign lesions were confirmed in 236 lesions.

材料与方法：搜集术前行乳腺MR检查并经穿刺或手术病理证实的236例乳腺病变，以对侧正常乳腺腺体作为正常对照，采用横轴位的平面回波-扩散加权成像序列，TR=8400ms，TE=93.8ms，层厚=4mm，激励次数=2：视野=30×30cm，矩阵=128×128，ASSET=2，b值=0和1000s/mm~2；参考动态增强图像准确定位，测量病变区和对侧正常乳腺腺体的ADC值，应用t检验比较良、恶性病变及正常腺体ADC值的差异，采用接收者工作特征曲线(receiver operating characteristic curve，ROC)确定良恶性病变的ADC界值；根据BI-RADS MRI将乳腺病变按照形态学表现分为肿块性病变和非肿块性病变两组，比较ADC值在肿块和非肿块性病变中定性诊断的效能。

The malignant tumors included osteogenic osteosarcoma,metastasis tumor(six patients,two with pathologic fractures),chondrosarcoma,malignant fibrous histiocytoma (three patients,one with pathologic fracture),fibrosarcoma,and solitary myeloma.all patients received limb savage procedure with custom knee prosthesis including sevnteen cases of hinge joint and twentyone cases of posterior stability joints.Results One of nine patients with benign tumors was lost for followedup,one patient died of unrelated disease,the remained patients have excellent function in five patients,good function in one case,and one poor according to the Enneking evaluating system.the limb salvage rate was 100％.Seven patients were lost for followedup in twentynine of malignant tumors,two patient with malignant fibrous histiocytoma received amputation due to local recurrence nine months postoperatively.Ten patients had excellent function,four good and six poor among the followedup patients.The type of prosthesis in six of all patients with poor function was hinge.

结果 9 例良性肿瘤患者1 例失访，1 例死于其他不相关疾病，其余7 例病人随访时膝关节功能按Enneking制定的标准进行评定，优5 例，良1 例，差1 例，保肢成功率为100％；29 例恶性骨肿瘤患者，骨肉瘤患者失访7 例，2 例患者术后局部复发，行截肢术；其余20 例患者生存期间膝关节功能优10 例，良4 例，差6 例。7 例效果评价差的病人为有6 例为铰链式膝关节假体。

There were 22 malignant tumors including 11 osteosarcomas, 5 chondrosarcomas, 2 malignant fibrous histocytomas, and one each of malignant osteoblastoma, alveolar sarcoma, rhabdomyosarcoma, malignant giant cell tumor of bone. 28 benign tumors included 27 giant cell tumors of bone and 1 benign fibrous histocytoma.

There is not a significant difference in these groups by one-way analysis of variance. 2 Among 51 cases with lung cancer, 7 specimens showed malignant cells in bronchial lavages, including 4 cases in central lung cancer and 3 cases in peripheral lung cancer. Concordant results were observed in 20 cases. 6 specimens showed malignant cells on cytological analysis, and NMSP was positive in at least one gene tested. 14 samples did not show malignant cells on cytological analysis, and the NMSP results were correspondingly negative. The results of cytology and NMSP were discordant in 31 samples. 30 samples were cytologically negative, but NMSP positive in one or more genes. In addition, there was one case that the cytology was positive for malignant cells, but the NMSP was negative in all of the three gene tested.

51例肺癌患者支气管灌洗液细胞学检查发现肺癌细胞7例，其中27例中心型肺癌患者4例支气管灌洗液发现肺癌细胞，24例周围型肺癌患者3例支气管灌洗液发现肺癌细胞。20例支气管灌洗液细胞学检查和过甲基化检测得到一致结论。6例支气管灌洗液发现肿瘤细胞，过甲基化分析至少一个基因阳性。14例灌洗液未发现肿瘤细胞，三个基因过甲基化分析均为阴性。31例支气管灌洗液细胞学检查和过甲基化检测得到不一致结论，30例细胞学检查为阴性，但过甲基化分析一个或一个以上基因为阳性。1例细胞学检查阳性但过甲基化分析阴性。

Malignant melanoma can occur from skin, or any internal organs, among which skin melanoma is more frequently seen, especially in the skin of legs, cunnus, and crissum. However they have been rarely reported. Malignant melanomas of the internal organs can be easily diagnosed with its specitic MRI features, while the non-melanin contained melanoma which has no imageological specific feature are more difficult to be diagnosed. Because CT examination has no specific feature, MRI should be the first choice, for the suspect patient of malignant melanoma.

Results The tumors were divided into three types according to the CT findings. Type I: regular, homogeneous density and well-defined,there were 30 patients, included benign mixed tumor(n=29) and adenolymphomas(n=1); Type II:irregular, heterogeneous dentsity, poordefined, there were 15 patients, included benign mixed tumor(n=7), malignant mixed tumor (n=2),adenolymphomas(n=3), hemagioma(n=2),mucoepidermoid carcinoma(n=1). 结果 肿瘤按CT表现分三类：52例患者中，I类30例，形态规则呈类圆形，密度均匀，边缘清楚，除1例腺淋巴瘤外，29例均为良性混合瘤；II类15例，形态不规则，呈分叶状，密度欠均，边界较清或模糊，其中7例良性混合瘤，2例恶性混合瘤，3例腺淋巴瘤，2例血管瘤，1例黏液表皮样癌；III类7例，形态不规则，边缘无法确定，其中恶性混合瘤2例，黏液表皮样癌2例，囊腺癌1例，恶性淋巴瘤1例，腺泡细胞癌1例。

Results Of 62 cases of PMLGI, 6 cases were low grade malignant B-cell lymphoma of MALT type, 14 cases high grade malignant B-cell lymphoma of MALT type with a low grade malignant component, 1 case lymphomatous polyposes, 2 cases Butkitt-like lymphoma, 38 cases B-cell large cell lymphoma and 1 case intestinal T-cell lymphoma.
