本探究测定分析了活动性肺结核患者治疗前后CRP的变化，以探索CRP在肺结核疗效评估中的临床意义。1资料和方法1.1一般资料选自2005年1月至2007年8月我院住院的活动性肺结核患者120例，所有病例均根据临床表现、X线胸片或胸部CT结合痰涂片、细菌培养等确诊，所选病例均符合中华医学会结核病学会分会制定的肺结核诊断和治疗指南[2]，120例均为初治活动性肺结核患者，男78例，女42例；年龄18～73岁，平均年龄(40±6)岁。1.2方法所有患者清晨空腹静脉采血，分离血清后采用美国雅培Ci8200全自动生化分析仪测定，采用散射免疫比浊法测定血清CRP水平，且同时测定红细胞沉降率，痰结核菌检查。。。

This thesis is composed of five parts:(1)systematic evaluation for the first time of the ability of five defferent PCR methods to detect M. tuberculosis;(2) using the endonuclease and southern blot by a radioactive and a nonradioactive labeled probe, to confirm the specificity of the PCR products and to analyse the sensitivity of detection by southern blot method using digoxigenin-labeled probe;(3) evaluation of acid-fast staining, bacterial culture and PCR for the detection of M. tuberculosis in sputum, pleural and peritoneal effusion samples;(4) The feasibility of PCR for detection of M tuberculosis on formalin-fixed paraffin embedded tissues;(5) 26 cases of sarcoidosis were retrospectively studiedwith PCR for M. tuberculosis and nontuberculosis Mycobacterium and the problems on histopatholoical diagnosis of sarcoidosis are discussed.

论文共分为五个部分，(1)首次系统地比较了五种不同引物序列的PCR检测结核杆菌方法的特异性和敏感性等情况；(2)利用限制性内切酶酶切分析和同位素及非同位素标记DNA探针进行Southern杂交的办法，分析PCR产物的序列与原设计是否相符及杂交检测PCR产物对提高检测结核杆菌敏感性的情况；(3)并对PCR用于快速检测临床标本中的结核杆菌进行了初步评价；(4)探讨了PCR用于检测石蜡包埋组织中结核杆菌的作用；(5)并利用PCR检测结核杆菌和非结核分枝杆菌的方法对26例原病理诊断为结节病的病例进行了PCR检测及组织病理学诊断的探讨。

In order to investigate new methods to the detection of Mycobacterium tuberculosis and diagnostic value of FQ-PCR in the detection of DNA in Mycobacterium tuberculosis, FQ-PCR was used to detect DNA of Mycobacterium tuberculosis in body fluid from 68 patients with tuberculosis and 68 patients without tuberculosis and it was also compared with the two methods of smear acid-fast stainning and antituberculotic antibody.

为探讨结核分枝杆菌检测的新途径及其FQ-PCR技术在结核分枝杆菌DNA诊断中的实用价值，运用FQ-PCR技术对68例结核病患者和68例非结核病患者体液进行结核分枝杆菌DNA测定，并与涂片抗酸染色及ELISA—抗结核抗体法进行比较。

Results:Compared with the simple pulmonary tuberculosis patients,the patients with infiltrative pulmonary tuberculosis complicated with diabetes were more serious,more caseous foci,more pulmonary cava,more bacilli in sputum and more recurrent cases.The therapeutic effect of the patients with infiltrative pulmonary tuberculosis complicated with diabetes was similar to those with simple pulmonary tuberculosis in the sputum bacillus disappearance rate and the tuberculosis focus absorption,if the diabetes was stable and the blood glucose was less than 8.4 mmol/L.

结果：浸润型肺结核并发糖尿病的患者比单纯肺结核病情较重、干酪病灶多、空洞多、排菌和复治病例比例大的临床特点；在糖尿病稳定，血糖控制在8.4 mmol/L以下，其痰菌阴转率、病灶显吸率与单纯性肺结核的疗效相似。

Results Tuberculosis lymph gland inflammation coverd highest rate:546 cases (36%) in tuberculosis node was 252 cases (46.2%),lymphonode period was 184 cases (33.7%),early tuberculosis lymphagland inflammation was 233 cases (44.7%).

利用改良一次性5～10ml注射器(22G、7号注射器，外径＜0.7mm)，针吸取材、涂片、行改良瑞-姬双染技术，低倍镜查找特殊异常细胞，油镜仔细观察细胞形态结构特征，对于未见异常细胞要进行分类检查，对疑难标本，同时做结核抗体、抗酸菌染色、流式细胞仪、CD系列、基因、DNA异倍体、锌原卟啉、肿瘤标志物等检测指标。

Results There were 18 cases with abnormal urine routinew (90%), 10 cases with abnormal ESR (50%), acid-fast bacilli from the urine of 12 cases were positive (60%), ultrasonic examination indicated 12 cases with renal tuberculosis (60%), 10 cases were diagnozed to have renal tuberculosis by CT Scanning (50%) and 14 cases by IVP (70%); 14 operated cases were pathologically confirmed to have renal tuberculosis, and follow-up for 1 to 2 years showed stable curative effect and no occurrence of urinary tuberculosis.

结果 18例尿常规异常(90%)，10例血沉异常(50%)，尿液查找抗酸杆菌12例呈阳性（60%），B超提示肾脏结核12例（60%），CT检查10例诊断肾结核（50%），IVP检查诊断肾结核14例（70%）。14例患者手术后经病理确诊为肾结核，随访1～2年治疗效果稳定，未发现尿路结核。

The germinal center firstly emerges in these three organs at 14 days. With the increase of day age, the characteristic structure peripheral immune organs gradually develop mature. Spleen achieve mature at 21 days and cecal tonsil at 35 days.2. IgM~+ and IgA~+ cells in spleen of embryo emerge at 15 days. IgG~+ cell, CD3~+ and CD8~+T lymphocytes of embryo emerge at 20 days. CD3~+, CD8~+ and IgM~+ cells in cecal tonsil of embryo emerge at 20 days. However CD4~+, IgG~+ and IgA~+ cells all emerge in 1 day age chicken out of crust. CD3~+, CD4~+, CD8~+, IgM~+ and IgG~+ cells in esophago tonsil of embryo all emerge at 20 days. However IgA~+ cells emerge in 1 day age chicken out of crust.3. The amount of T, B lymphocytes in peripheral immune organs increase follow with the increase of day age, and hold an upgrade tendency. The amount of T, B lymphocytes in spleen achieved stabilization at 21 days, and in tonsil of esophago and appendix at 35 days.

研究结果表明：1、在组织结构方面，脾脏实质内的白髓与红髓在胚胎18日龄后明显可辨，4日龄雏鸡脾脏中形成明显可辨的动脉周围淋巴鞘和椭球周围淋巴鞘；盲肠基部T、B淋巴细胞在胚胎20日龄时开始出现，即初步形成盲肠扁桃体；食管与腺胃结合处在4日龄时形成明显的隐窝结构，即食管扁桃体初步形成；14日龄时，三种器官中首次出现生发中心；随着日龄的增长，外周免疫器官特征结构不断发育成熟，脾脏在21日龄时达到成熟水平，盲肠扁桃体和食管扁桃体在35日龄时达到成熟水平。2、在T、B淋巴细胞出现时间方面，脾脏中IgM~+和IgA~+细胞在胚胎15日龄时开始出现，IgG~+细胞、CD3~+和CD8~+T淋巴细胞在胚胎18日龄时出现，CD4~+细胞在胚胎20日龄时出现；盲肠扁桃体中CD3~+、CD8~+和IgM~+细胞在胚胎20日龄时开始出现，而CD4~+、IgG~+和IgA~+细胞均在雏鸡出壳后1日龄时出现；食管扁桃体中CD3~+、CD4~+、CD8~+、IgM~+和IgG~+细胞均在胚胎20日龄时开始出现，而IgA~+细胞则在雏鸡出壳后1日龄时出现。3、在T、B淋巴细胞数量变化方面，外周免疫器官中T、B淋巴细胞的数量随日龄增长，整体均呈上升趋势。
