子宫肌瘤对生育有较大影响，妊娠合并子宫肌瘤属于高危妊娠应当引起足够的重视并积极治疗，剖宫产行子宫肌瘤剔除术是一种积极可行的方法，临床效果较好。

Objective To promote the development of rational drug use,the intervention study for rational antibiotics use on perioperative period of hysteromyomectomy was undertaked in our hospital.

目的 干预研究本院剖宫产抗菌药物应用，推动本院合理用药工作的开展。

Results: 8 cases of patients were vaginal delivery; cesarean section delivery in parallel with hysteromyomectomy were 21 cases;1 case was multiple fibroids and the diameters were＞5 cm, with more bleeding and a larger wound during the operation d, so we took total hysterectomy to avoid infection; 1 case of cervical fibroids, which did not be removed in the cesarean section because of its special position.

结果：患者阴道分娩8例；行剖宫产术分娩并行子宫肌瘤剔除术21例；1例为肌瘤多发，且直径均＞5 cm，术中出血较多，且创面较大，为避免感染和出血行全子宫切除；1例为子宫颈肌瘤，因其特殊位置，剖宫产时未行剔除术。

There was no complication after operation.Conclusion Mutiple Hysteromyomectomy by blocking uterine artery in laparoseopy is an ideal,safe and effective procedure charaterized by minimal invasion,rapid postoperative recovery and exerllent clinical efficacy.

腹腔镜下阻断子宫动脉对多发性子宫肌瘤切除术是目前较为理想、安全有效的保留子宫的手术方法，它具有创伤小、手术快、术后并发症少等优点。

妊娠中期子宫肌瘤的处理：①肌瘤直径小于6厘米，且无症状者，定期产前检查，绝大多数不需特殊处理；②肌瘤直径大于6厘米，随着子宫的增长肌瘤还可能继续增大，而大型肌瘤易有红色样变而刺激子宫收缩或有腹膜刺激症状，介时产科医生只是建议患者卧床休息及应用止痛剂等进行保守治疗，很少建议在妊娠期行子宫肌瘤剔除手术，只有在不得已情况下才行肌瘤剔除术。

Methods Forty-five patients with hysteromyoma undergone hysteromyomectomy were randomly assigned to three groups (n=15,each group): group Ⅰ, control group without pre-emptive analgesia; group Ⅱ, the patients were administrated with 0.2 mg·kg-1 ketamine intravenously 30 min before operation; group Ⅲ, 30 mg ketamine was administrated into epidural analgesia 30 min before operation.

45例子宫肌瘤患者均于硬膜外麻醉行全子宫切除术，随机分为3组，即Ⅰ组：对照组，术前不施行超前镇痛；Ⅱ组：切皮前30 min静脉注入氯胺酮0.2 mg·kg-1；Ⅲ组：切皮前30 min将氯胺酮30 mg加入实验剂量(2%利多卡因4或5 mL)中注入硬膜外腔。3组术后均连接Graseby9300镇痛泵行患者自控硬腺外镇痛，镇痛药物为0.2%罗哌卡因加50 mg·L-1吗啡。

Based on selection of patients,electric coagulation,separation and cutting was used for intramural myoma,broad ligament myoma,cervical myoma and subserous myoma respectively.It suggests that laparoscopic hysteromyomectomy is safe and feasible with little blood loss and short recovery in treating myoma.
