A pure tone audiogram identified severe bilateral symmetric down-sloping sensorineural hearing loss with otherwise normal caloric testing result. Bilateral optic fundus examination using fluorescein angiography identified retinitis pigmentosa. A diagnosis of Usher syndrome Type Ⅱ was diagnosed based on these findings.
