The urinary fistula is one of the serious complications following renal homotransplantation,especially the uretero-cutaneous fistula.

objectiveto investigate the diagnosis and treatment of traumatic duodenal rupture.methodthe clinical data of 13 patients with traumatic duodenal rupture hospitalized from may 1991 to december 2008 were retrospectively analyzed,and the material cause was automobile accident mainly.resultsin these 13 patients,after hospitalization,6 cases were diagnosed in 3 hours,4 cases were diagnosed in 24～72 hours,and 3 cases were diagnosed after 72 hours.three cases with duodenal cleft repair,2 cases with duodenal ostomy,2 cases with duodenum and jejunum roux-en-y anastomosis,1 case with duodenal diverticular surgery,1 case with meliorative duodenal diverticular surgery,1 case with duodenal broken ends closure and anastomosis of stomach with jejunum,1 case with duodenal distal closure and anastomosis of duodenal local with jejunum,and 2 cases with resections of pancreas duodenum.after the operation,6 cases (46.2%) appeared some complications of wound infection,duodenal fistula,digestive tract hemorrhage,pancreatic fistula,abdomen infection and so on.two cases (15.4%) died from multiple system organ failure and septic shock.conclusionthe early diagnosis and correct operation style selection are the keys for duodenal rupture.

目的：探讨外伤性十二指肠破裂的诊断和治疗方法。方法：回顾分析外伤性十二指肠破裂患者13例临床资料。结果：入院后3 h内确诊6例，24～72 h内确诊4例，72 h后确诊3例。3例行十二指肠裂口修补，2例行十二指肠造瘘，2例行十二指肠空肠roux-en-y吻合，1例行十二指肠憩室化手术，1例行改良十二指肠憩室化手术，1例行十二指肠两断端关闭、胃空肠吻合，1例行十二指肠远端关闭、近端与空肠端侧吻合，2例行胰十二指肠切除术。术后6例(46.2%)出现切口感染、十二指肠瘘、消化道出血、胰瘘或腹腔感染等并发症，2例(15.4%)患者死亡，分别死于多器官系统衰竭和感染中毒性休克。结论：早期诊断与正确的术式选择是成功救治外伤性十二指肠破裂的关键。

Results In all these cases, there are 83 cases origin from mucous membrane which include inflammation, tumor and anabrosis; 24 cases origin below mucous membrane which include interstitialoma, lymphoma and scleroderma;38 cases mainly changed in the shape, structure and position of organs which include hiatus hernia, Oblique inguinal hernia, diverticulum, malrotation of intestine and megacolon; 29 cases mainly changed in the power and function of organs which include functional disorder of esophagus, mesentery-compressed syndrome funtional disorder of small intestine and irritable colon; 17 cases' performance caused by the factor outside intestinal canal which include mesentery shift, stomach shift and small intestine shift, and other cases which include fistula between esophagus and mediastinum, rectovaginal fistula, foreign body in esophagus and small intestine ascariasis.

结果：197例患者中，源于黏膜的病变83例，源于黏膜下的病变24例（包括间质瘤、淋巴瘤、硬皮病），以器官形态、结构、位置改变为主的病变38例（包括食管裂孔疝、腹股沟斜疝、憩室、肠旋转不良、巨结肠），以器官动力、功能改变为主的病变29例（包括食管功能紊乱、肠系膜压迫综合征、小肠功能紊乱、结肠过敏），因管腔外因素的影响而引起的改变17例（包括食管移位、胃移位、小肠移位），其他病变6例（包括食管纵隔瘘、直肠阴道瘘、食管异物、小肠蛔虫症）。

Objective To investigate the cause of native arterio-venous fistula aneurysm and prevention measures. Methods There were 7 cases with snuff-box arteriovenous fistula and 53 cases with brachiocephalic fistulas. Among them, 14 patients had to made second fistulas in the same arm .Spring tape around the arm was used to stop bleeding after dialysis.

Results:Seventysix cases were successfully performed LC,three cases were Mirizzi syndrome type Ⅰ,and one case was converted to open surgery because of gallbladdercolon fistula and gallbladderright hepatic duct fistula,and one was converted to choledocholithotomy with Ttube drainage because of Mirizzi syndrome combined with injury of common bile duct.

结果：76例顺利完成LC，其中3例为Mirizzi综合征Ⅰ型；1例因合并胆囊结肠漏、胆囊右肝管漏中转开腹，另1例为Mirizzi综合征Ⅱ型，术中胆总管损伤中转手术行胆总管对端吻合&T&管支撑引流术。

Result: the occurrence rate of the chylous fistula after neck dissection was 3.47%; 4 cases of them in operation were cured after ligation in, sewing firm, the ramming of local omohyroid, the chylous fistula after neck dissection did not appear; 6 cases of them after operation was cured by conservative treatment,8 cases were cured through the surgical operation again.

结果：乳糜漏的发生率为3.47%；术中并发乳糜漏4例经手术治疗未出现乳糜漏；术后并发乳糜漏14例中6例经保守治疗治愈，术后8例经再次手术治愈。
