objective to assess the effects to utilize minitype plate internal fixation in radial head fracture of mason ⅲ.methods 10 cases of patients with closed radial head fracture under went minitype plate internal fixation by open reduction,and the functional evaluation of elbow was carried out by the pain,stability,myodynamia of elbow,together with the elbow joint score of broberg and morrey.results in 10 case of patients with 12～24 months follow up(average time 20 months),the elbow joint mean score is 58(30～90) with excellent case 1,good 4,poor 5,and the fineness rate is 50％.conclusion it is failed to treat complicated radial head fracture by minitype plate internal fixation,while maybe it is better to resect or replace radial head for these types.

目的 评价微型钢板治疗masonⅲ型桡骨小头骨折的临床效果。方法对10 例masonⅲ型骨折进行微型钢板内固定，根据患者肘关节活动疼痛、稳定性和肌力情况，依照broberg和morrey肘关节评分标准进行功能评价。结果 10 例患者随访12～24个月，平均20个月，肘关节平均评分58 分(30～90)分，其中优1 例，良4 例，差5 例，优良率50％。结论切开复位微型钢板内固定治疗复杂的masonⅲ型桡骨小头骨折，不能取得良好的临床效果，如果要获得较好的肘关节活动和早期康复，可考虑桡骨小头切除或置换。

Results The anterior bundle of ulnar collateral ligament originates from the inferior aspect of the medial epicondyle and inserts immediately adjacent to the joint surface on the ulna near the sublimis tubercle. The posterior bundle originates from the medial epicondyle slightly posterior to its most inferior portion and inserts broadly on the olecranon process. The lateral collateral ligament arises from the inferior aspect of the lateral epicondyle. Two types of conjoined lateral collateral and annular ligamentous insertions on the ulna were observed. Type Ⅰ(61.2％) was bilobate and type Ⅱ(38.8％) was a single broad conjoined type with insertion on the ulna. The anterior band of anterior bundle was more tighten than the posterior band as the elbow flexed less than 60°. When the elbow flexed over 60°, the two parts of anterior bundle were equally tightened. The posterior bundle was tightened as the elbow was flexed more than 90°. The lateral collateral ligament was tightened gradually as the elbow moved in flexion.

结果(1)尺侧副韧带前束起于肱骨内上髁的前下方，止于尺骨冠突内侧的小结节；后束起于肱骨内上髁的内下方，止于尺骨鹰嘴内侧的骨面；(2)桡侧副韧带起于肱骨外上髁的外下方，其纤维部分止于环状韧带，部分止于尺骨冠突的外下方；桡侧副韧带和桡骨环状韧带在尺骨上的止点有两种类型：Ⅰ型占61.2％，桡侧副韧带的部分纤维汇于环状韧带的尺骨止点，另一部分纤维单独止于稍远的尺骨上；Ⅱ型占38.8％，桡侧副韧带和环状韧带形成一宽的纤维止于尺骨上；(3)在肘关节屈曲60°以前，尺侧副韧带前束的外侧部紧张而内侧部较松弛，肘关节屈曲超过60°后，前束内外侧处于同程度的紧张状态；尺侧副韧带的后束在肘关节屈曲超过90°后才被拉紧；(4)桡侧副韧带在肘关节不同屈曲状态时其紧张度逐渐增加，当肘关节屈曲超过90°时，桡侧副韧带被明显拉长。

The results indicate that weight factor had significantly effect on gait speed, steps per minute, gait cycle time, the ratio of stance time and swing time, maximal flexion angle of knee joint and the range of motion of elbow joint. Shelf height had significantly effects on hip flexion and elbow ROM. Box size factor had significantly effect on stride length, gait cycle time, the ratio of stance time and swing time, knee joint elbow ROM. For the muscle activity levels with EMG, load weight and shelf height both affected the %MVC significantly. When the subjects carrying a load of 14kg, the muscle activity and the subjective body part discomfort rating are both higher than that of 7kg. The box size had effect on the body part discomfort rating of upper extremity and back. For the kinetic and kinematics analysis, the shelf height around elbow height was better than the other two height levels.

研究结果显示，对步态时间与空间参数而言，重量因子显著影响步速、步频、步态周期时间与站立期摆荡期比值，箱型因子显著影响步长、步态周期时间与站立期摆荡期比值；关节活动度而言，重量显著影响膝关节最大弯曲角度、肘关节屈伸角度范围，高度因子显著影响髋关节前后倾角度、肘关节活动各变项，箱型因子影响膝关节弯曲最大最小值以及肘关节活动各变项，对肌肉活动度而言，重量与高度因子皆显著影响MVC%值，14公斤重者肌肉活动度与主观不舒适程度皆较7公斤者高，箱型因子虽无明显影响肌肉活动度，但是在主观不舒适程度上却明显影响上肢与背部，终点放置高度应设在肘高附近为佳。

METHODS: From June 2007 to March 2009, 8 patients with terrible triad of the elbow were treated. The fracture of the radial head was classified according to the system of Mason, with typeⅠ in 1 patients, type Ⅱ in 6 and type Ⅲ in 1; fracture of the coronoid process was classified according to the system of Regan and Morrey, with type Ⅰin 2 patients, type Ⅱ in 5, and type Ⅲ in 1. The hollow lag screws and Herbert screws were used to fixe coronoid process and radial head fractures, and repair Annular ligament. Lateral collateral ligament was repaired in 1 case. All injured extremities were treated with a posterior plaster splint with the elbow joint held in 90° of flexion and the forearm in neutral rotation for 2-3 weeks followed by elbow rehabilitation training.

于2007-06/2009-03共收治肘关节恐怖三联征患者8例，其中尺骨冠突骨折按 Regan-Morrey法分类：Ⅰ型2例，Ⅱ型5例，Ⅲ型1例；桡骨头骨折按Mason法分类：Ⅰ型1例，Ⅱ型6例，Ⅲ型1例。8例患者均采取了内固定治疗，采用中空拉力螺钉或Herbert螺钉固定冠突和桡骨头骨折，修补环状韧带，1例缝合修复肘外侧副韧带，术后屈肘90°旋前中立位，以塑形良好的高分子管型石膏固定两三周后开始屈伸和旋转康复训练。
