目的 探讨消化性溃疡急性穿孔的治疗方法，旨在提高ⅰ期手术治愈的成功率。方法回顾性总结分析20年间手术治疗消化性溃疡急性穿孔208例，其中86例（41.35%）采用胃大部连同病灶切除ⅰ期治愈，胃肠重建采用billrothⅰ式吻合18例（20.93%），billrothⅱ式吻合68例（79.07%），billrothⅱ式结肠前吻合42例（61.76%），结肠后吻合26例（38.24%）。结果 86例中除1例billrothⅱ式吻合术后发生胃瘫并发症，经保守治疗月余治愈外，其余病例术后恢复顺利，均随访5年以上情况良好。结论消化性溃疡急性穿孔的ⅰ期手术治疗，解决了因溃疡穿孔单行修补术后再次复发穿孔、出血、幽门梗阻、粘连、溃疡恶变而重复手术等问题，缩短了治愈周期，提高了生存质量。

Results: thirty cases were cured and 7 cases were improved, accounting for 83% and 13.7% respectively; 8 cases gave up treatment, accounting for 15.7%; 6 cases died, accounting for 11.8%.of the 6 died infants, 5 cases died of respiratory failure and pulmonary hemorrhage; 3 cases were sclerema neonatorum, 3 cases were very low birth weight infants; 3 cases' gestational age was less than 32 weeks, 1 case had intrauterine growth retardation, 1 case had asphyxia history of birth, 1 case had infection history , 1 case had convulsion history (suspected intracranial hemorrhage).

结果：治愈30例，占58.8％；好转7例，占13.7％；自动出院8例，占15.7％；死亡6例，占11.8％，其中5例死于呼吸衰竭，肺出血。其中3例有硬肿症，3例为极低体重儿；孕周2周者3例；1例有宫内发育迟缓，1例有新生儿窒息史，1例有感染史，1例有抽搐史。

Results In 136 patients, a total of 72cases sputum culture positive, the positive rate of 52.9% sputum culture for three consecutive single bacteria were 16 cases, the positive rate of 11.8%.16 patients sputum results were:hemolytic streptococcus 6 cases, 4cases of Pseudomonas aeruginosa , Klebsiella pneumoniae in 2 cases and gas production Bacillus 2 cases , Enterobacter Bacillus 2 cases, 136 patients with 386 cases sputum culture times , 106 times positive(27.5%), and consecutive three or more times a train 4 the same bacteria ,only 42 cases(10.8%); the same single-64 cases sputum culture , which amounts to 16.6%.

结果 24例枪伤中，20例一期缝合伤口，均一期愈合，另外4例只清创未一期缝合的伤口，3例愈合，1例感染；4例骨折内固定均愈合良好，无骨髓炎发生。16例炸伤病人局部软组织损伤及污染比枪弹伤重，14例一期缝合伤口中，10例一期愈合，4例感染，骨折内固定2例，均愈合良好，无骨髓炎发生。结论寒冷干燥地区火器伤早期伤道清创时限可适当延长，在平时火器伤的救治中，对软组织条件较好的创面，早期清创后可一期闭合伤口和骨折内固定，这样可减少感染和伤残，促进康复。寒冷干燥地区；火器伤；早期救治
