Abstract］objective: effects of a single period of mechanical strain on gene expression patterns of two osteogenic transcription factors, ets1 and cbfa1, in rat calvarial sutural osteoblast-like cells were studied in this study to elucidate the molecular mechanism of suture distraction.

摘要］ 目的：探讨大鼠颅骨缝成骨细胞在张应力作用下ets1和cbfa1基因表达的变化，以揭示骨缝牵张成骨的分子机制。

Objective: Effects of a single period of mechanical strain on gene expression patterns of two osteogenic transcription factors, Ets1 and Cbfa1, in rat calvarial sutural osteoblast-like cells were studied in this study to elucidate the molecular mechanism of suture distraction.

结果：Ets1和Cbfa1的mRNA水平分别在机械张应力作用后6 h及12 h内显著增高，随后，mRNA水平逐渐恢复到对照组水平，Ets1先于Cbfa1表达，在加力后表达立即升高，并在0.5h达到最高水平，而Cbfa1则首先经历一个短暂的潜伏期，后表达逐渐升高，在6 h达到最高水平，Cbfa1的最高表达水平约为Ets1的2.58倍。

Results: During the period of distraction and fixation, positive staining of BMP-2 was found in the proliferated preosteoblasts, the mesenchymal cells and the newly formed bone matrix at the sutural margin of the experimental dogs; weak expression was also noticed in the control dogs.

对67例下颌骨骨折患者的临床资料进行回顾总结，所有患者应用小型接骨板行坚固内固定治疗。其中7例颏部骨折，12例颏旁骨折，16例颏孔区骨折，9例体部多发性骨折，5例体部粉碎性骨折；18例合并其他部位骨折。所有患者术前经临床检查和影像学检查明确骨折的位置和骨折线的数目后，分段牙弓夹板结扎、颌间弹性牵引，使移位的骨折段逐渐复位；术中经口内切开复位、细钢丝颌间结扎、小型接骨板坚固内固定；术后行临床检查和影像学复查，必要时颌间弹性牵引7～10 d。结果所有患者手术切口ⅰ期愈合，治疗效果满意；临床检查所有患者牙合关系良好、面部对称；影像学复查证实骨折线对位良好。结论经口内切开小型接骨板坚固内固定辅以颌间固定治疗下颌骨骨折效果可靠，术后并发症少，是一种理想的手术方法。
